Introduction: Physician office, clinic, and healthcare centers provide primary care function including health education, prevention, and chronic disease management. As recommended by OECD and AHRQ, Prevention Quality Indicators (PQIs) is a set of measures that can be used with hospital inpatient discharge data to identify the quality of care for ambulatory care sensitive conditions. Good outpatient healthcare settings can potentially prevent the need for hospitalization. As the result, Jawda Department of Health has developed the following indicators to track performance for clinic and centers in the Emirate of Abu Dhabi. Clinic and centers are NOT required to submit quarterly submission of Jawda e-notification system. Jawda team will use centrally collected claim data submitted from healthcare providers through Shafafiya portal. Type: Clinics/ Centers Indicator Indicator Number: CC001 Sub- Percentage of Diabetes Hospital Admission Emergency admissions for a principal diagnosis of diabetes with short-term complications. Numerator: Number of patients aged 18 years or older, with an emergency admission for diabetes short-term complications (principal ICD 10-CM diagnosis code). The patients would have visited the clinics at least 2 times within 0 to 182 days of the hospital admission. Denominator: Total number of diabetes patients 18 years and older who visited the clinics with the diagnosis of diabetes at least 2 times in the last six months. : [Numerator] / [Denominator] x 100 Unit Measure comparison if available Admission rate per 100 patients with diabetes who visited the clinics at least 2 times Developed locally by modifying similar indicators used by AHRQ and OECD. 2 P a g e

The patients would have visited the clinics at least 2 times within 0 to 182 days of the hospital admission. Denominator: Total number of heart failure hypertension patients 18 years and older who visited the clinics with the diagnosis of heart failure at least 2 times in the last six months.
