Children are special We all recognize that children are not small adults and should not be treated as such. We wouldn t give a newborn baby a steak to eat; we would only give a pediatric dose of medicine to a toddler; we would never ask a thirteen-year-old to drive us to work. We have evolved to treat children and adolescents appropriately according to their age, and this understanding guides practically everything we do with and for them to aid their growth and development into happy, successful adults. Hearing loss is no exception. In fact, understanding the unique challenges of hearing impairment in childhood is essential to avoid long-term problems that might hold them back later in life. From initial diagnosis and testing to hearing aid selection and verification, and ongoing support and counselling, the needs of hearing-impaired children are very different from those of adults. Crucially, hearing loss, regardless of degree, can impair the development of spoken language. Therefore, it is vital that children have consistent access to speech to be able to engage fully with education, family life and relationship building in order to establish the foundation of spoken language. It is important for you, as a practicing physician looking after children at such an important time in their young lives, to know that your hearing aid recommendations will make a difference. Introducing SoundRecover2: a revolution in hearing technology Non-linear frequency compression is a form of frequency lowering technology that compresses high-frequency sounds into a lower frequency range in order to improve audibility. Phonak first introduced non-linear frequency compression, called SoundRecover, in instruments for severe to profound hearing loss in 2008, providing better detection of /sh/ and /s/ sounds for those with more severe hearing loss. SoundRecover2 is Phonak s new frequency lowering algorithm, using the very latest adaptive technology to increase the benefits of non-linear frequency compression. It uses two cut-off frequencies to define a region where compression is adaptively applied in real time according to the sound frequencies being detected (Figure 1). This helps to increase audibility of high-frequency sounds and at the same time protect the clarity of low-frequency sounds (e.g. vowels and voiced consonants). SoundRecover2: enabling access to even more sounds Clinical studies of SoundRecover2 show improved audibility of high-frequency information compared with the original SoundRecover while also maintaining the low and midfrequency sounds, giving children access to a broader range of sounds essential for speech and language development. 1,2 By creating an optimal listening experience, SoundRecover2 enables more children than ever before to hear and communicate with confidence. Children already using SoundRecover may be able to switch to SoundRecover2 without a long period of acclimatization, so the benefits of upgrading to the new platform can be experienced sooner rather than later. AutoSense Sky OS Listening made easy Phonak s AutoSense Sky OS (operating system) is based on the successful AutoSense OS. It has been specifically designed to ensure that children have an optimal listening experience whether they are in the classroom or playground, listening to music or spending time with the family. By accurately capturing and analyzing a child s environment in real time, AutoSense Sky OS always selects the right blend of programs and settings to optimize a child s listening and understanding (Figure 2). The new automatic program and feature selector in AutoSense Sky OS is up to 30% more precise for noisier classrooms and 39% more accurate in recognizing yelling and shouting as unwanted noise compared to the adult system. 3 Indicator light Ensuring life is on The new, easy-to-understand indicator light featured on the four Sky V behind-the-ear (BTE) models provides parents and caregivers a clear, visible signal that: the hearing aid is powered on the Roger program is active the battery is running low This new feature can be activated and programmed flexibly by a hearing care professional. In an external usability study of the new indicator light, 100% of parents were easily able to confirm the hearing aid was powered on. 4 Technology With no manual interaction required, children can focus on more of life s great adventures and let their hearing aids do the work. RogerReady Ready to go Adding a Roger receiver is now easier than ever. Any Roger receiver coupled to a Sky V device will automatically detect the signal from a Roger microphone and activate the Roger + Mic program. This is all that is needed to receive the Roger signal. Automatic detection ensures that children have access to the Roger signal with unparalleled signal-to-noise ratio whenever a Roger microphone is in use. There is no additional programming of the hearing aids by the audiologist, giving access to improved hearing quickly and simply. Roger + Mic A choice of customizable modes Available in Phonak Target 4.3 software and higher, Roger + Mic now offers a choice of three microphone modes customizable by the fitter: Omnidirectional Real Ear Sound Automatically activated fixed directional Roger and directional setting Exclusively providing near and far field clarity The Roger and directional setting allows children to simultaneously hear the teacher at a distance and their peers in close proximity (see Figure 3). Phonak Sky V performance: proven benefits Technology Along with a host of new dedicated features, children wearing Sky V will also benefit from the proven performance of Binaural VoiceStream Technology, which uses a four-microphone network to stream simultaneous, bi-directional, full-audio bandwidth signals in real-time. This helps to improve sound quality, resulting in unmatched speech understanding. Binaural VoiceStream Technology has been shown to improve speech recognition. It is featured in CROS II and is available in hearing instrument programs such as Speech in Wind and DuoPhone. Speech in Wind When Speech in Wind is activated, the system can respond appropriately depending on whether the wind noise is asymmetrical, so the wearer has no need to recognize and react to the wind noise. The Speech in Wind program has been shown to make voices clearer while improving comfort. As a result, speech intelligibility increases by up to 28%. 6 DuoPhone Children with hearing loss can find phone calls difficult to follow. Phonak s DuoPhone helps them overcome this challenge by automatically streaming the caller s voice in real-time to the ear opposite the phone. The effect is to allow the speaker to be heard in both ears. A recent study of children aged between 6 and 14 years showed that using the DuoPhone helped improve speech recognition by 30% over monaural hearing in both quiet and noisy conditions. 7 CROS II For children with an unaidable hearing-impaired ear on one side and hearing in the other ear, Phonak now offers a new, discreet CROS transmitter (Contralateral Routing of Signals). Featuring the unique Phonak Binaural VoiceStream Technology, CROS II wirelessly transmits sound from the unaidable ear to the Sky V hearing aid on the better hearing ear. A clinical study has shown that using CROS II results in improved speech understanding in both quiet and noisy environments. 8 Our research has shown that children typically receive a 30% improvement in speech understanding on the telephone with use of the DuoPhone, and as a result, we consider the DuoPhone to be an absolutely essential feature on hearing aids fitted for children. Roger technology: helping bridge the understanding gap Children require a hearing system capable of dealing with noisy listening challenges as they enjoy a life of action, play and education. Any improvement in word recognition scores can have a positive effect on a child s understanding and therefore learning abilities. 9 This is where the Roger portfolio can bridge the understanding gap.
