Please join us for Region VII Diabetes Summit When Friday, November 18, 2016 8:00 AM 4:00 PM Location: John and Mary Pappajohn Education Center 1200 Grand Avenue, Des Moines, IA Purpose: The summit will provide the latest information on the diabetes epidemic, how evidence-based programs are working to reduce this epidemic, and the roles key stakeholders play in prevention and control of diabetes. Objectives for Summit By the end of the program, attendees will be able to: Explain the role federal agencies play in addressing diabetes prevention and control at the state, and local level Discuss the prevalence and incidence of pre-diabetes and diabetes in the United States Explore innovative mechanisms for reaching high risk populations through healthcare referrals, community efforts, and program promotion Describe how evidence-based programs such as the National Diabetes Prevention Program, and the Diabetes Education and Support Programs impact prevention and control of diabetes. Identify challenges and strategies for implementing and/or, utilizing evidence-based programs and services Target Audience The intended audience for the Summit is public health professionals, healthcare providers such as: physicians, nurses, advance practice nurses, health coaches, diabetes educators, and dieticians, pharmacists, diabetes prevention program facilitators, diabetes self-management education coordinators, certified diabetic educators, and third-party payers.

The cost per individual will be $35.00 and will cover the cost of a light breakfast, lunch and afternoon snack. Registration Registration is limited.
