Clinical Nurse Specialists (CNS) Working alongside Consultants, Cardiac Services has a number of highly specialist nurses who provide excellent support to patients and their families. These include: Arrhythmia CNS Interventional Cardiology CNS Rehabilitation CNS These specialist nurses provide care and a dedicated point of contact from pre-admission to post discharge. Pre-Admission Care Our CNS team will contact patients prior to admission and provide: Individual contact by email, telephone or text message Tailored advice about patient s condition and hospital admission Information and guidance regarding medication to be taken prior to admission Assistance with discharge planning Patient support to answer any questions or queries prior to admission 8

During Your Stay During admission, a dedicated CNS will work alongside the ward nurses and Consultant to keep the patient informed about their ongoing condition, to answer any questions and assist with any complex medical or nursing needs. Post Discharge Care Our CNSs offer post discharge support for patients relating to queries regarding symptoms or medication. The patient will be given a 24-hour helpline number providing direct contact to a cardiac ward nurse. Rehabilitation Our Rehabilitation Specialist Nurse (RSN) is supported by a team of cardiac physiotherapists, dietitians and pharmacists who provide support throughout the rehabilitation process. The RSN provides assessment of cardiac risk factors, education and support for patients undergoing procedures. All patients will be referred to a cardiac phase lll rehabilitation programme which provides ongoing structured exercise, education and physiological support. Nursing and Ward Services Critical Care Unit The Critical Care Unit provides 24-hour specialist care for patients requiring multi-organ support due to complex medical and surgical conditions. It also provides care for patients undergoing complex cardiology procedures e.g. TAVI. The Critical Care Unit submits data to the ICNARC (Intensive Care National Audit Research Centre) database. Critical Care includes: 15 beds providing Level 2 and Level 3 care 24-hour Consultant Intensivist cover 24-hour critical care resident medical officer cover 24-hour radiology/pathology services Highly experienced critical care trained nurses Ward Nursing The ward has a high nurse-to-patient ratio, providing skilled nursing care to patients as they recover in a comfortable and attractive ward environment. Cardiac physiotherapists provide patients with individual tailored exercise programmes to aid the rehabilitation process. Blood Management Services A team of clinical perfusion scientists utilise the most advanced technology available to offer a range of perfusion techniques unmatched in the UK private sector. Blood management strategies: Ability to reduce bleeding, therefore dramatically reducing the need for blood transfusions. London Bridge Hospital is unique in offering blood management to every single patient, not just those undergoing complex procedures. Cardiopulmonary bypass excellence: State-of-the-art equipment minimises post operative complications.
