Adaptation of the KOOS questionnnaire for the use in Estonia. 2000-2002. Ann Tamm University of Tartu, Estonia ; ann.tamm@kliinikum.ee 1.The initial material was the PhD thesis by Ewa Roos ( Knee Injury and Osteoarthritis. Development,evaluation and application of patient relevant questionnaires. Lund 1999). 2. The first forward translation from English into Estonian was made by 2 English.-Estonian professional translators. Each problem in the first translation was discussed and consulted by a panel consisting of 4 physicians ( 2 GPs, 1specialist in rehabilitation medicine,1 specialist in internal medicine) and 1 Estonian who had resided in Sweden and knew Swedish conditions (working version 1). 3. Back translation from Estonian into English was made by 2 English-Estonian professional translators. This translation was compared with initial English text. The translation was consideredd very good by the panel (2 GPs, 1specialist in rehabilitation medicine,1 specialist in internal medicine )( working version 2) 4. The elaboration of both working versions (1 and 2) was participated by physicians of different specialities who checked and discussed all subscales and individual items of the questionnaire. Working version 3 was developed which was regarded as ready for being completed by a pilot group. Pilot group 1 was formed of 30 (15+15) 35-60 year old patients in the list of two GPs, living in town Tartu and of 12 voluntary physicians and nurses. 5. In the execution of working version 3 the following circumstances became evident. Problems with understanding appeared in connection with the domain ADL: (i) item A8 going shopping and (ii) item A 13 getting out/in of bath First, substantial analysis and discussion revealed that as super-and hypermarkets did not exist in Estonia in 2000, it was not understood that shopping could be hours-long activity, standing and walking, which may cause an extra load for the knees. Later on the trade settings changed and at present this question is understandable without adaptation. Second, in Estonian tradition, people living in rural areas have a wash in residential houses or in separate sauna buildings where there is no bath (about 5-6 % of the study subjects). (iii) items P 2-3 in the domain Pain. All the questions of KOOS pertain to the past week. Two of the five items P are beyond of that scale : monthly, weekly. The decision: not to change the original questions (working version 4 = 3 ). 6. Following discussion, working version 4 was tested on 22 patients (pilot group 2) attending the rehabilitation unit of the Clinic of the Internal Medicine of Tartu University Hospital (Patients who had among other diagnoses also M 17.0). 7. In working version 4 we recruited 161 subjects (100 women and 61 men), aged 34 55 years, investigated within the Elva population study for establishment of the prevalence of knee OA. Probably it was the first attempt to use KOOS in a population study.
