Cerner COMPASS ICD-10 Transition Guide Dx Assistant Purpose: To educate Seton clinicians regarding workflow changes within Cerner COMPASS subsequent to ICD-10 transition. Scope: Basic modules and functionality will remain largely unchanged in Cerner as a result of the ICD-10 transition. However, one significant change will be the addition of Dx Assistant, a native tool that facilitates ICD-10 code selection using filters with explicit clinical descriptions. This document will address: Adding the Dx Assistant tool Accessing Dx Assistant (multiple pathways) Navigating within Dx Assistant Adding Dx Assistant: The Dx Assistant tool will be built into the COMPASS platform at a date TBD. In the meantime, each physician will need to add Diagnosis Assistant Specificity using the following steps: 1. After navigating to the Diagnosis and Problems tab within COMPASS, right click anywhere in the Diagnosis field. 2. Click on Properties. 3. Click Diagnosis Assistant Specificity in the left column, then click Add (illustrated in diagram below).

4. Once Diagnosis Assistant Specificity is added to the right column, click Move Up until it sits in between Date and Clinical Dx. This is the recommended placement, so that the tool is adjacent to the Clinical Dx field. 5. Click OK. 6. After adding Diagnosis Assistant Specificity, an icon should appear in an additional column between Date and Clinical Dx 7. The Dx Assistant icon will exhibit one of two states: specified or unspecified, as illustrated below: Accessing Dx Assistant 1. Impression and Plan The suggested primary access point for Dx Assistant is via the Impression and Plan (I&P) paragraph in PowerNote. It is strongly encouraged that physicians utilize this point of access, as diagnoses designated using this pathway can be subsequently abstracted from the Dx and Problems window into the Impression and Plan as illustrated below. After navigating to this module, double click on OTHER DIAGNOSIS to add a diagnosis from the Dx and Problems module:

After adding the relevant code using Dx Assistant (process outlined in next section) and highlighting the appropriate diagnosis, click Include to abstract the diagnosis into Impression and Plan, as shown in the diagram below: 2. Diagnoses and Problems

Another pathway to access Dx Assistant is by clicking on the Diagnoses and Problems module on the left-hand menu within COMPASS (see diagram below). After navigating to the Diagnoses and Problems tab, click Add or start typing in the IMO search field to initiate the code selection process. **It is highly encouraged that physicians access Dx Assistant via one of the above pathways. However, there are three other pathways for accessing Dx Assistant, as discussed below.** 3. Orders Page Navigate to the Orders page using the left-hand menu. Once the pop-up window for orders opens, click on the circular icon as indicated below to access Dx Assistant :

4. Patient Summary Click the Patient Summary paragraph on the left-hand menu, then click Diagnoses / Problems List as illustrated below: 5. Depart Click Depart located in COMPASS banner bar:

Then click on the note icon next to Diagnosis and Problems as shown below: Next, click on the Dx Assistant icon as shown below:

Navigating Dx Assistant Once a diagnosis is added (see Section #2 above), click on the Dx Assistant icon within in the Diagnosis and Problems window as displayed below to open the Dx Assistant window: After the Dx Assistant window opens, use the displayed fields (e.g., Clinical Core, Clinical Subset, Temporality, Anatomy, et al) to filter range of potential appropriate diagnoses as shown below:

Choose the most appropriate diagnosis from the list in the bottom half of the Dx Assistant window: Once the appropriate diagnosis is chosen, click Save and the Dx Assistant window will close. As a reminder, if you access Dx Assistant via the Impression and Plan pathway, the selected diagnosis can be easily abstracted into the Impression and Plan section of PowerNote (described in detail in previous section). **Note: To unselect an item within the filter fields, simply double click on the highlighted item. This will expand the list of potential codes.

Summary Dx Assistant is a COMPASS-native tool that will enable physicians to easily select a diagnosis code with the highest degree of specificity. The tool enables physicians to narrow the list of potential ICD-10 codes using multiple filter fields (e.g., Clinical Core, Laterality, Temporality, et al). As a reminder, it is incumbent on the individual provider to ensure that ICD-10 code specificity is supported by clinical documentation. The intended outcome of this document is for physicians to become proficient with the functionality and user interface of Dx Assistant. Diagnosis / Problem Conversion Appendix To transfer a diagnosis to the problem list while in the Diagnosis and Problems module, simply highlight the desired diagnosis and click Convert as illustrated below: Also, physicians can click the Dx icon adjacent to the Name of Problem field to convert the problem to the Diagnosis list, as illustrated below:

Finally, physicians can drag and drop diagnoses / problems, which aligns with current state functionality.
