Healthy Rewards Program Calendar September 2018 These programs are open to all Saint Francis Healthcare System employees and volunteers. Full-time and part-time employees enrolled in Healthy Rewards Employee Incentive Program receive Wellness points for participation. If you have any questions, please call Wellness at ext. 5970. Program Descriptions There are many ways to earn points toward the Wellness portion of the Healthy Rewards incentive program. This calendar lists all programs and activities that can be attended by employees (as well as spouses and additional adults covered by a Saint Francis Healthcare System insurance plan) to earn Wellness points. For a complete listing of all the ways that Wellness points can be earned, please refer to your Healthy Rewards Goals Packet that was given to you at your annual Wellness appointment. BEEF (Body weight exercise that is Easy, Effective, and Fast) - 25 Wellness or Fitness points* Tuesday, September 18 (Fitness Plus Athletic Republic (second floor of Fitness Plus) Tuesday, September 25 (Fitness Plus Athletic Republic (second floor of Fitness Plus) 8-8:20am or by appointment (call x5970 to schedule a different day/time) This program will review specific guidelines and recommendations for quick and easy bodyweight resistance training that can be done at home, at work, or on the go! A 30-day fitness log will be provided at the end of class. *Completion of the 30-day exercise log is required in order to receive 25 FITNESS points. DASH Diet - 25 Wellness or Blood Pressure points* 9:00-9:45 am Check out the diet plan that has been developed specifically to prevent or lower high blood pressure. The DASH (Dietary Approaches to Stop Hypertension) Diet class will cover the background information on the diet and give you specific guidelines to follow. *This class may be taken for partial credit under the Blood Pressure portion of the Healthy Rewards goal if your blood reading is outside the desirable range (the 30-day challenge handed out by the instructor must be completed), or you may use the 25 points for the Wellness portion. Express Screening (for enrollment/re-enrollment in Healthy Rewards) 25 Wellness points* Thursday, September 6 (Saint Sebastian & Saint Martha Conference Rooms) 7:00-9:00 am First Thursday of each Month These monthly screenings are EXCLUSIVELY for employees whose annual evaluation is due (as well as spouses/ eligible adult dependent who are covered under an SFMC insurance plan) and includes having your blood work drawn (be sure to fast overnight), blood pressure checked, height/weight, etc. Your online Personal Wellness Profile (PWP) MUST be completed prior to participating in the screening. A separate appointment with Wellness is required to review results. Dependents of employees who are enrolled in the SFMC insurance plan are also eligible to participate in a screening during the time of the employee s annual evaluation. *If you need labs drawn for a B or C Follow-Up, you are welcome to attend an Express Screening for blood draw, however the 25 points will only be awarded when you attend your follow up appointment with Wellness.

Fit or Fat?- 15 Wellness points Wednesday, September 19 (Located in the Wellness Office) 7:00-8:00 am (takes about 10 minutes) BODY FAT PERCENTAGE: Learn what your percent body fat is. We will be measuring percent body fat using skinfold calipers, the most widely used method of estimating percent body fat in the fitness field. Percent body fat in healthy percent fat range for your age and gender is worth 50 points in the Body Weight category. Establish a baseline for the coming year! ***NEW*** STEP TEST: Come let us measure your cardiovascular endurance! The Step Test is a 3-minute fitness test that will reveal your personal aerobic capacity. You can use this measurement to establish a baseline for Healthy Rewards goals and possibly earn 50 points in the Fitness category on your next annual visit. Food of the Month- 10 Wellness points Thursday, September 13 (Located at the Tower Gift Shop) 11:30-12:15 pm Come catch the cooking demo on wheels and take a taste of an exciting new recipe! Healthy Bites Luncheon 20 Wellness points Thursday, September 6 (Saint Sebastian & Saint Martha Conference Rooms) 12:10-12:40 pm First Thursday of each month Various health professionals lead this monthly program discussing a variety of health-related topics. It is also open to the public. Feel free to bring your lunch to eat while you listen. Heart & Health Screening Wednesday, September 5 (Saint Martha Conference Room) 7:00-10:00 am First Wednesday of each month Cholesterol ($30) and blood glucose ($7) test 15 Wellness points Results in minutes!! Blood pressure checks are free of charge; cholesterol and blood glucose tests require a small fee (and a 10-12 hour fast, water is encouraged).

Salad Week 5 Wellness points Sunday, September 9 Saturday, September 15 Choose a day this week for your department to have a Salad Luncheon. You and your colleagues organize a potluck of your favorite salad fixings (lettuce, carrots, cucumbers, tomatoes, nuts, cheese, beans, beets, avocado, etc ). Send Wellness a list of all of those participating. 5 Wellness points per participant SideStepping Diabetes 25 Wellness or Blood Glucose points* 7:30-8:15 am Pre-registration is required! Call x5316 With early detection and lifestyle change, you can reduce your risk of developing diabetes. Come learn how! This class is offered once per month; it will be the same information each month. *This class may be taken for partial credit under the Blood Glucose portion of the Healthy Rewards goals if you have glucose/hgba1c levels outside the recommended range (the 30-day challenge handed out by the instructor must be completed), or you may use the 25 points for the Wellness portion. TLC (Therapeutic Lifestyle Change) 25 Wellness or Blood Lipids points* 9:45-10:30 am Does your heart need a little TLC? It may if you have high cholesterol or other risk factors for heart disease. TLC helps you take small steps that can result in big changes by lowering your risk for heart disease. This class is offered once per month; it is the same information each month. *This class may be taken for partial credit under the Blood Lipids (cholesterol) portion of the Healthy Rewards goals if you have cholesterol levels outside the recommended ranges (the 30-day challenge handed out by the instructor must be completed), or you may use the 25 points for the Wellness portion. Walk Till You Drop 25 Blood Glucose points Thursday, September 20 (Wellness Office) Appointments will be available between 8:15 and 9:30 a.m. Participation is limited! Pre-registration is required! Call x5316 Have you ever been told that you are Pre-diabetic or Diabetic? Have you heard that exercise can help you get better control over your blood sugars? If so, come join us on the Fitness Plus track on Thursday September 20 to walk till you drop - your blood sugar, that is! To participate, you must commit to: * Eating a meal about 60-90 minutes before your walking session * Avoiding sugary snacks or beverages after your meal * Walking at least 20 minutes at a BRISK pace during your appointment * Having Wellness staff check your blood sugar by finger-stick before and after your walk

5 Wellness points (maximum 60 points per year) Every Wednesday (Elevators outside Tuscan Sun) 11:15 am-11:45 am This is a weekly weigh-in for anyone trying to lose or maintain weight. You will be provided with a new health tip each week to help keep you on track. Wellness Walk 5 Wellness points OR 5 Fitness points (maximum of 25 points per year) Tuesday, September 11 (Fitness Plus track meet by the Wellness Office) 11:30 am-1 pm (walk as long as you like) Take part of your lunch break to burn off some calories (and stress!). You do not need to be a Fitness Plus member to participate in these Wellness Walks. Walk 4 laps (0.5 mile) and earn 5 Wellness Points. Walk 8 laps (1 mile) and earn 5 Fitness points.
