Dear Team Captain, Thank you for STEPPING UP for Down Syndrome! We are glad that you are taking STEPS to support and empower individuals with Down syndrome and their families. Enclosed you will find information about how to organize your Fundraiser, as well as information about the Down Syndrome Association for Families of Nebraska and the services DSAF of NE provides to our community. The Down Syndrome Association for Families of Nebraska is a nonprofit organization, a 501(c) (3), whose mission is to support and empower individuals with Down syndrome and their families. DSAF promotes the inclusion of all people with Down syndrome by providing families and our communities with up-to-date information and education. To that same end, DSAF strives to foster positive attitudes and enhance public awareness within our society of the inherent worth, value and contributions of people with Down syndrome. Some of the ways DSAF fulfills our mission is through: Bi-Monthly e-newsletters and organization website Resource guides, information / referrals for families of newborn babies diagnosed with Down syndrome Educational seminars and training workshops Local and National conference stipends Social events, educational programs and networking opportunities Advocating for the rights of individuals with Down syndrome Support services and referrals for families Leadership and volunteer development Staffing Should you have any questions, please feel free to contact the office at (402)421-1338. Steps to Having a Fundraiser to Benefit the Down Syndrome Association for Families of Nebraska (DSAF of NE) 1. Have an Idea! 2. Complete the Third Party Fundraiser Proposal Form along with the Third Party Agreement Form and return them to: Down Syndrome Association For Families of Nebraska Attn. Step UP Walk or email to stepupwalk@dsafnebraska.org Once the form is submitted, we will review it. We will do our best to contact you within 7 days with our decision. Please do not move forward with your plans until you have been contacted by a DSAF representative. 3. Wait for written approval from DSAF. 4. Plan and execute your fundraiser! 5. Contact DSAF with any questions you may have or visit our website at www.dsafnebraska.org to learn more. 6. Remember The final deadline for accepting corporate donations and Early Bird Fundraising to earn a fact poster for every $225 250 you raise (limit 3) must be turned in by August 30, 28 2015 6 so plan your fundraiser accordingly to take advantage of these dates. What is a Fundraiser? Commonly Asked Questions A Fundraising event is when a business, organization, community group, school or individual hosts an event or program to benefit an organization. The event or program is planned and executed by the business, organization, community group, school or individual hosting the activity. I want to organize a fundraiser for my STEP UP Team but I m not sure what to do. Listed below are some ideas that you may use to help raise funds and awareness for your STEP UP Team. This list is by no means limited to what you can do; let your creativity guide you! Remember, your team will get credit for the funds you raise but all proceeds of these fundraisers/events benefit the DSAF of NE and they make it possible for us to carry out our mission within our community to support and empower individuals with Down syndrome and their families. Collect Donations: Set out collection canisters next to cash registers or around businesses throughout a week or a month, to encourage patrons and customers to help you support your Team. Host a fundraiser: Basketball Tournament, bake sale, car wash, a Date Auction, etc the list is endless. Host a Special Event: Host a benefit party/concert, a silent auction benefit, a talent show, etc. Speak with your employer about having a Dress Down Day. Selling items like t-shirts, jewelry; Sporting event such as a tournament; Online solicitations Be sure to advertise the fundraiser as, [Team Name] FUNDRAISER to benefit the DSAF of NE. What can the Down Syndrome Association for Families of NE do to help with my fundraiser? The DSAF may assist the fundraiser by: Giving permission to use the DSAF logo Advertising to families in monthly newsletter, e-newsletters, website or other means of advertising as deemed appropriate by the DSAF Providing DSAF materials as requested Assistance in the design of materials/flyers Guidance and suggestions with event planning as requested What if those contributing to our Fundraiser would like a receipt? Receipts will be provided whenever requested and on all donations over $250. Please make sure checks are made payable to the DSAF of NE and you provide us with the donor name and current address information for all donations. Remember, the Step Up Walk Website can be used to organize your team s donations and email receipts to your donors. Can I do more than one fundraiser? You can do as many fundraisers as you like, but each fundraiser needs its own agreement signed and approved by the DSAF of NE. Page 4

FUNDRAISING PROPOSAL FORM This form serves as an agreement between the fundraiser and the Down Syndrome Association for Families of Nebraska for the purpose of setting forth the terms and conditions of our relationship in respect to the fundraising program described below. Please return to this form to the DSAF of NE. FUNDRAISING AGREEMENT FORM Thank you for your interest in fundraising for the Down Syndrome Association for Families of Nebraska. We appreciate your desire to support your team and STEP UP for Down Syndrome. This letter serves as an agreement between you and the Down Syndrome Association for Families of Nebraska for the purpose of setting forth the terms and conditions regarding the fundraising program described below. 1. At your event on (date) you agree that if donations are collected, you and or your team will forward 100% of the donations collected to the Down Syndrome Association for Families of Nebraska. Unfortunately we do not allow joint fundraisers at this time. 2. Advertise the fundraiser as, [Insert Team Name] FUNDRAISER to benefit DSAF of NE. Although DSAF is extremely appreciate of your efforts, insurance and/or other responsibilities could be presumed if it was actually a DSAF sponsored event. 3. All donations submitted in the form of a check must be made payable to the Down Syndrome Association for Families of Nebraska. In addition, it is your responsibility to ensure that DSAF of NE knows to credit your team with funds received. 4. If donors expect a tax receipt for their donations, please ask donors to make their donation by individual check (make sure donor s name and address are included on the check). The check must be made out to the Down Syndrome Association for Families of Nebraska, not to individuals or another organization. 5. Please realize that in order for individuals donating coins or bills to receive a tax receipt from DSAF, your team must keep a record of the cash donations. An easy way to do this is to purchase a receipt book and write down each donation along with the name and mailing address of the donor. Be sure to submit the cash and records to DSAFof NE so we can issue a tax receipt to the donor. 6. You agree to deliver or mail the money you have collected to the Down Syndrome Association for Families of Nebraska within 3 days after the fundraiser. Please mail money to: Down Syndrome Association for Families of Nebraska Attn. STEP UP Walk 7. You agree to pay all costs associated with advertising, developing and printing promotional and marketing materials for the fundraising program except those that DSAF may voluntarily provide to you at no obligation and you agree that before you use, publish, disseminate or display the materials with the DSAF of NE name on it that you must obtain approval of the materials by the Down Syndrome Association for Families of Nebraska. 8. You agree to comply with all federal, state and local charitable solicitation statutes, regulations and ordinances that affect or apply to you and the fundraising activities authorized hereunder. 9. You agree that any tax credit accrues to the original donor and not to you as a third party conveyer of the donation. 10. You agree that you shall not assign, subcontract or in any way transfer any of the obligations, duties or responsibilities under this Letter Agreement without the prior written permission of the Down Syndrome Association for Families of Nebraska. 11. Either party to this Letter Agreement may terminate this Letter Agreement at its convenience. In the event of termination, all funds collected subject to this Letter Agreement must be remitted to the DSAF of NE. Page 6

12. DSAF and its officers and directors or agents are not liable - paired with a hold harmless and indemnity provision wording pending. 13. This Letter Agreement constitutes the entire agreement and understanding between you/your team and the Down Syndrome Association for Families of Nebraska with regard to the subject matter hereof and no amendment, modification or waiver of any of the terms or conditions herein shall be valid unless in a writing signed by both you/authorized representative of your organization and an authorized representative of the DSAF. DSAF and its officers and agents shall incur no liability in relation or arising from the event. The below-named Applicant Fundraiser shall defend and indemnify and hold harmless DSAF and its officers and agents, against any and all losses, claims, demands, liabilities, suits, judgments, costs, and expenses, incurred by or suffered to DSAF or its officers or agents, by reason of such Applicant Fundraiser's breach of any covenant, representation, or obligation contained herein Once again, we thank you for your commitment to support the Down Syndrome Association for Families of Nebraska. To demonstrate your agreement to the terms and conditions of this Letter Agreement, please sign below as indicated and return the signed copy along with your Fundraising Proposal Form to: I AGREE TO THE TERMS AND CONDITIONS OF THIS LETTER AGREEMENT.
