Heart & Lung Westchester Symposium Saturday, May 5, 2018 8am 2pm Edith Macy Conference Center Statement of Need Multidisciplinary care coordination is essential to the continuation of successful treatment of patients with cardiovascular and thoracic disease, as it often involves the knowledge base of primary care providers, specialists, and surgeons. In addition to the ever changing nature of the technology utilized within the field, providers are often unaware of training and best practices used by other disciplines. Practice gaps exist due to continued evolution of new technology and the lack of provider awareness with respect to the various medical and surgical treatment options to manage their disease. This cutting edge activity is geared towards educating providers on the updated guidelines regarding ongoing screening and referrals to surgical specialists as well as current methods of surgical intervention using the latest endovascular and open surgical techniques. Identification and awareness of risk factors will promote early recognition and management of cardiovascular and thoracic disease. Program Objectives All Northwell Health CME activities are intended to improve patient care, safety and outcomes. At the conclusion of this conference, participants should be able to: - Recognize and implement nationally recognized guidelines for evaluation and treatment of complex cardiovascular and thoracic patients - Develop strategies to implement a multidisciplinary best practice care model to improve evaluation, diagnosis, and management of cardiovascular and thoracic patients - Review advances in cardiothoracic surgical techniques - Assess the most current treatment options for patients with cardiovascular and thoracic disease - Develop strategies for standardization of care in the treatment of cardiovascular and thoracic disease Target Audience Primary Care/Internal Medicine Physicians, Pulmonologists, Gastroenterologists, Medical Oncologists, Cardiologists Vascular Surgeons, and Cardiothoracic Surgeons, Critical Care Intensivists, Physician Assistants, Nurse Practitioners.
