Jefferson Center Follow Up & Monitoring Services The Child and Youth Leadership Commission (CYLC, or HB1451) in Jefferson County started a project in 2014 in the Health/Mental Health/Other Subcommittee that has created a collaborative relationship with Jefferson County School District and Jefferson Center for Mental Health to address suicide ideation in children that have been identified by the schools through a suicide risk assessment as high risk. http://www.jeffersoncountycylc.com/#!partners/mvua0
Jefferson Center Follow Up & Monitoring Services Jefferson County School District conducted 896 suicide risk assessments during school year 2014-2015. Through the collaborative relationship and partnership of the school district and Jefferson Center for Mental Health, 37 of those assessments were identified as high risk and received specialized follow-up treatment and wrap around support from all the systems involved in that child s life. The goal is suicide prevention.
Jefferson Center Follow Up & Monitoring Services Closing the Gap: From micro relationship building with the individuals and families effected by suicide, to the macro relationship building with stakeholders, community members, and providers, the common thread is connectedness and partnership that heals and moves us to grow.
