Penicilliosis marneffei is one of the commonest opportunistic infections in AIDS patients in Thailand. Most patients present with chronic course of fever, lymphadenopathy, hepatosplenomegaly, and cutaneous lesions. Even though P. marneffei infection in non-hiv-infected patients is rarely reported, the incidence has constantly increased especially those living in North Thailand since This review thus aims to describe P. marneffei infection in non-hiv-infected patients regarding the epidemiology, clinical manifestations, diagnosis, treatment, and outcomes, in comparison with that in HIV-infected patients. EPIDEMIOLOGY P. marneffei was first isolated from the lesions in the liver of a bamboo rat, Rhizomys sinensis, at the Pasteur Institute of Indochina, Dalat, South Vietnam, in The fungus was named in honor of Hubert Marneffe, a director of the Pasteur Institute of Indochina. Three years later, in 1959, the first patient with Penicilliosis marneffei was reported as a laboratory-acquired infection due to a needle stick injury. Until 1973, the first natural infection in man was described in a 61-year-old American man with Hodgkin s lymphoma undergoing splenectomy. 2 Before the HIV era, there were fewer than 30 patients from the endemic area including Thailand, Guangxi region in South China, and Hong Kong. 3,4 The incidence of human Penicilliosis marneffei has markedly increased due to expansion of the AIDS epidemic in Southeast Asia. 5 The incidence of P. marneffei infection in Thailand among AIDS patients is 2.4 percent-4.2 percent. In the North, it is as high as 6.8 percent. 6 Even though, the high incidence is noted in AIDS patients, the infection in non-hiv-infected patients remains sporadic. 5 The only known non-human host of P. marneffei is the bamboo rat. The role of these bamboo rats in human infection is unknown. In a case-control study of 80 AIDS patients with Penicilliosis marneffei, an association between the infection and a recent history of occupational or other exposure to the soil especially during the rainy season [odds ratio (OR) = 1.91, 95% CI = ] was observed. 7 However, a history of exposure to or consumption of bamboo rats was not a risk factor for infection in this study. 7 Chaiyalertsak and colleagues have shown that the infection occurred more commonly during the rainy season in North Thailand, probably due to the inhalation of fungal conidia from the environmental source including the soil. 8 CLINICAL MANIFESTATIONS There is something different regarding the clinical manifestations between HIV and non-hivinfected patients. Five non-hiv-infected patients with the age ranging from 6 to 50 years were reported from Ramathibodi Hospital, Bangkok, Thailand in Three patients had associated conditions including tuberculosis, systemic lupus erythematosus, and lymphoproliferative disorder. The other two patients were non-immunocompromised. The common manifestations included fever, generalized lymphadenopathy, and cutaneous lesions characteristic as nodules or abscesses. The osteolytic lesions of the flat and long bones were noted in two patients. The pulmonary involvement was a common feature in this study. In addition, the poor clinical outcome was observed despite appropriate antifungal treatment by amphotericin B, likely due to the delayed diagnosis in most patients. Another study was retrospectively carried out by Wortman and colleagues in 28 non-hiv-infected patients, in comparison with 38 HIV-infected patients. 3 Fever, skin lesions, lymphadenopathy, hepatomegaly, and pulmonary abnormalities were frequently observed in the 2 groups. However, osteoarticular involvement was more common among non-hiv-infected patients (18%) than HIV-infected patients (0%). The characteristics of the cutaneous lesions were different between the 2 groups. Most non-hiv-infected patients had abscesses (43%), ulcers (11%), or pustules (14%), whereas HIV-infected patients tended to have umbilicated papules or molluscum contagiosum-like lesions (18%), papules (45%), and pharyngeal lesions (26%). These lesions were correlated well with the histopathologic findings that granulomatous or suppurative inflammation was noted in most non-hivinfected patients. In contrast, HIV-infected patients were more likely to have necrotizing inflammation characterized by the focal necrosis with surrounding histiocytes in association with both intracellular and extracellular fungi. The majority of non-hiv-infected patients had no predisposing factors in this study; only one-third had preexisting or associated conditions including tuberculosis, systemic lupus erythematosus, dermatomyositis, autoimmune hemolytic anemia, lymphoproliferative disorder, renal transplantation, and Hodgkin s lymphoma. Interestingly, half of HIVinfected patients had positive fungal blood cultures, whereas the number was only 7 percent among non- HIV-infected patients. Even though there were some different clinical manifestations between the 2 groups, the response to antifungal treatment in this study was excellent. A study in Hong Kong by Wong and colleagues from 1994 to 1999 compared the clinical manifestations of P. marneffei infection in non-hiv- and HIV-infected patients. They showed that HIV-infected patients had a higher frequency of fungemia than non-hiv-infected patients, 10 consistent with a previous study by Wortman and colleagues mentioned above. 3 In addition, the
3 Vol. 28 No. 1 P. marneffei Infection in Non-HIV:- Plongla R, et al. 73 delayed diagnosis was more frequently observed in non- HIV-infected patients than HIV-infected patients (the median duration of illness before diagnosis was 5.5 and 1 week among non-hiv- and HIV-infected patients, respectively). Approximately 85 percent of non-hivinfected patients in this study had predisposing conditions including hematologic malignancies, corticosteroid use, and immunosuppressive drug use. A recent study in Thailand described the unique clinical syndrome in non-hiv-infected patients with acquired cell-mediated immunity defect who were susceptible to opportunistic infections caused by Salmonella, P. marneffei, Cryptococcus neoformans, Histoplasma capsulatum, and nontuberculous mycobacteria. 11 The syndrome comprised the clinical manifestations of opportunistic infections mostly involving the lymph nodes and the lungs as well as reactive skin diseases like Sweet s syndrome, pustular psoriasis, and acute generalized exanthematous pustulosis. As mentioned earlier, the osteoarticular involvement was more commonly noted in non-hivinfected patients than HIV-infected patients. The affected bones and joints include the ribs, skull, scapula, long bones, lumbar vertebrae, and temporomandibular joints. Arthritis commonly involves both peripheral large and small joints of the fingers. 1 A recent study by Louthrenoo and colleagues has shown that non-hiv-infected patients had the delayed time to diagnosis of osteoarticular penicilliosis, compared to HIVinfected patients (11.75 ± 7.76 months and ± 9.01 days in non-hiv- and HIV-infected patients, respectively). 12 In addition, they found that patients with osteomyelitis usually had associated subcutaneous abscesses or chronic ulcers overlying the corresponding osteolytic lesions. 12 In this study, the radiographic abnormalities included 73 periarticular soft tissue swelling, periarticular osteopenia with the narrowing joint spaces, and multiple osteolytic lesions and minimal sclerotic margins without periosteal reaction. However, these findings are not specific for P. marneffei osteomyelitis since the infections caused by Mycobacterium, Histoplasma, Cryptococcus, Blastomyces, and Coccidioides can give these radiologic abnormalities. DIAGNOSIS The diagnosis is based on the identification of the organism on microscopy with the confirmation by the culture method. P. marneffei has the unique feature among the Penicillium species of being thermally dimorphic. At 25 C it grows as the mycelium form (similar to Aspergillus), but at 37 C it grows as the yeast form. Microscopic examination of all clinical specimens reveals the yeast cells (2 2 to μm in size) with a transverse septum per each yeast cell (asexual fission for schizogony), present both within and outside the macrophages. 13 The cultivation at 37 C, the fungal colonies appear to have soluble red pigment around them and diffused into the agar medium. Several serodiagnosis and molecular methods have been developed, but these have not yet entered to the routine clinical practice due to their relatively low sensitivity and specificity. 14 Interestingly, the currently available galactomannan test for Aspergillus antigen may assist in the diagnosis of P. marneffei infection. A recent study has shown that the sera from 11 of 15 HIV-infected patients with P. marneffei infection were positive (optimal density index of > 0.5) for galactomannan antigen test. However, the serum samples from 9 percent of HIVinfected patients without P. marneffei infection were apparent to be falsely positive. 15 The differential diagnosis of P. marneffei infection includes other fungal infections like

In a retrospective study in Thailand, Mootsikapun and colleagues found that histoplasmosis and Penicilliosis marneffei had shared similar clinical presentations, laboratory investigations, and radiologic appearance. 16 The most common clinical findings in both infections included chronic fever, weight loss, cough, anemia, lymphadenopathy, hepatomegaly, and splenomegaly. The occurrence of skin lesions was noted with the same frequency in both fungal infections. The most common findings of chest X-ray in both infections was bilateral interstitial infiltrations. Table 1 demonstrates the difference and similarity among the 3 important yeasts causing human infection including P. marneffei, H. capsulatum, and C. neoformans. itraconazole is the drug of first choice for treatment of P. marneffei infection. 17 The approach to initial treatment depends on the disease s severity and the patient s ability to tolerate the oral therapy. For HIVinfected patients with severe disseminated disease, the intravenous therapy with amphotericin B (0.6 mg/ kg daily) for two weeks followed by oral itraconazole (200 mg twice daily) for 10 weeks is the preferred regimen. However, the duration for non-hiv-infected patients remains unclear, perhaps for years. The patients treated with amphotericin B should be monitored for the infusion-related adverse reactions, nephrotoxicity, and electrolyte disturbances. The new triazoles including voriconazole and posaconazole are also effective and well-tolerated but the clinical studies are limited. 18 TREATMENT Therapeutic intervention of P. marneffei infection has to be timely and optimally appropriate. The delayed diagnosis is associated with the increased mortality and morbidity. Amphotericin B and OUTCOMES The mortality is very high in disseminated P. marneffei infection unless the early diagnosis is made and appropriate treatment is promptly instituted. 5 Vol. 28 No. 1 P. marneffei Infection in Non-HIV:- Plongla R, et al. 75 percent to 77 percent in early studies 1 has increased to approximately 90 percent in recent studies. 19 CONCLUSIONS In conclusion, non-hiv-infected patients with P. marneffei infection have the different and similar clinical manifestations, compared to HIV-infected patients. The clinical features vary from the localized infection to the severe disseminated infection. Localized infection is more commonly noted in non-hiv-infected patients than in HIV-infected patients. The unique skin lesion like abscesses and ulcers as well as osteoarticular involvement are frequently observed in non-hivinfected patients. The under-recognition of Penicilliosis marneffei in non-hiv-infected patients may cause the delayed diagnosis and will lead to the increased mortality and morbidity rates. Even though P. marneffei infection in non-hiv-infected patients is not uncommon, we urge all clinicians to raise the awareness of this infection in their daily clinical practice. References 1. 7 Review Article Vol. 28 No. 1 Sparganosis:- Wongkulab P, et al. 77 Sparganosis: A Brief Review Panuwat Wongkulab, M.D., Kom Sukontason, M.D., Ph.D., Romanee Chaiwarith, MD., MHS INTRODUCTION Sparganosis, first described by Manson in 1882, 1 is a parasitic zoonosis caused by plerocercoid larvae of Pseudophyllidea tapeworms mainly recognized in Spirometra genus. 2-3 Many species of Spirometra are recognized in clinical diseases including Spirometra mansoni, S. ranarum, S. mansonoides, S. erinacei, and the aberrant Sparganum proliferum. 4 The genus identification can only be indicated in adult worms which complete their life cycle in definite host; dogs or cats. 4 Humans are accidental intermediate hosts and worms develop into plerocercoid larvae which migrate into the tissue and cause local inflammation at the final sites. 3,5 Life cycle As shown in Figure 1, mature Spirometra worms inhabit in the intestines of dogs and cats. 1 Eggs are released into fresh water and hatch into the coracidia, which are ingested by copepod (crustaceans). 2-4 Coracidia develop into procercoid larvae in the copepod; primary intermediate host. 4 Infected copepods are ingested by secondary intermediate hosts including reptiles, amphibians, fish, and procercoid develope into plerocercoid. 5 The plerocercoid develops to mature worms in definitive hosts; canine. 1 Humans are intermediate hosts for Spirometra worms which develop sparganosis, and cannot serve for definite hosts. 4,6 Epidemiology Although several cases have been reported around the world, Eastern and South East Asia are mainly affected. There were sporadic cases in North America and Europe. 3 In Thailand, there were 34 cases reported by Wiwanitkit et al in Males were slightly less affected than females with the ratio of 15:19. The mean age was 34 ± 12.8 years (range years). The risk factors could be identified in 14 cases which included drinking untreated water, and history of eating snake or frog. 6 Mode of transmission Humans are accidental host by 1, Drinking untreated water containing infected copepods 2. Ingesting raw or inadequately cooked second intermediate hosts e.g. flesh of snakes or frogs infected with the Spirometra larvae 3. Applying the flesh of an infected intermediate host as a poultice on the open wounds, lesions, or eyes e.g. for medicinal or ritualistic reasons. They could settle on subcutaneous tissue as nodule, visceral organs including lungs, abdominal viscera, and also vital organs such as the brain. The reported incubation period ranges from a few weeks to years However the larvae can live in the tissue more than decades before symptoms happened After ingesting the infected copepod from untreated water or uncooked intermediate host, the larvae will penetrate the intestinal wall into peritoneum, migrate painlessly systemic to any site of the human body for growing and elicit pain with local inflammation at the final site of invasion. Eyes and skin are the most affected final location. 3 Ocular manifestation was the first leading symptom in the series of Thai patients, followed by subcutaneous tissue and central nervous system which presented with progressive neurodeficit and had the worst prognosis. 6 Others unusual organ involvements e.g. bone, pulmonary, oral cavity and proliferative sparganosis have also been reported in Thailand Sparganosis can be divided into 2 types as follows; 1. Proliferating type is caused by S. proliferum, mainly reported outside the United States including Japan, Venezuela, and Paraquay The larvae could proliferate as a parasite in humans, with more than one plerocercoid in one lesion, and spread to other part of organs. 3,5 Most cases were fatal. 2. Nonproliferation type is usually caused by remaining of species of Spirometra. They could not proliferate and find only one plerocercoid larvae in one lesion. S. mansonoides are the major acquired infection in United States. 3 Diagnosis The definite diagnosis is usually made by direct visualization of parasite, and pathological examination of tissue biopsy. The length of larvae is a few
9 Vol. 28 No. 1 Sparganosis:- Wongkulab P, et al. 79 millimeters to a centimeter, and white, ribbon-like, wrinkles are the gross morphology. The recorded length in Thailand was more than 2 mm. 6 The serological testing by monoclonal antibody IgG with enzyme-linked immunosorbent assay (ELISA) has been documented as optional for diagnosing cutaneous and cerebral sparganosis. Clonorchis and Paragonimus infections may cause positive result for this test. 3,17 Imaging Brain computer tomography and magnetic resonance imaging is a useful tool for guiding the diagnosis, determining severity and excluding other possibilities. Frontoparietal lobe, basal ganglion, and external capsule are the most common invasion sites. Although there are no specific signs for definite diagnosis, there are some suggestive findings as follows. Computer tomography may show an extensive low density of white matter with adjacent ventricular dilatation, an irregular nodular enhanced lesion, and changing in the location of the enhancing nodule punctuate calcifications. 18 Magnetic Resonance imaging may show widespread white matter degeneration, cortical atrophy, ipsilateral ventricular enlargement, decreased volume of the ipsilateral crus cerebri, and tunnel sign on postcontrast MRI or bead-shaped enhancement. 19 Treatments and Preventions Surgical resection and larvae removal are the best treatment. 3,5 Further damage can be prevented when live larvae are removed from the vital organs such as brain. Removing the dead larvae and granuloma might not improve the neurological deficit in cerebral sparganosis cases. 18 The antihelminth including mebendazole, albendazole, and praziquantel have not been shown to be beneficial.
