Persons with an AIDS-indicator condition (Category C) as well as those with CD4+ T-lymphocyte counts <200/FL (Categories A3 or B3) are reportable as AIDS cases in the United States and Territories, effective January 1, PGL=persistent generalized lymphadenopathy. Clinical Conditions Included in the 1993 AIDS Surveillance Case Definition! Candidiasis of bronchi, trachea, or lungs! Candidiasis, esophageal! Cervical cancer, invasive*! Coccidioidomycosis, disseminated or extrapulmonary! Cryptococcosis, extrapulmonary! Cryptosporidiosis, chronic intestinal (>1 month duration)! Cytomegalovirus disease (other than liver, spleen, or nodes)! Cytomegalovirus retinitis (with loss of vision)! Encephalopathy, HIV-related! Herpes simplex: chronic ulcer(s) (>1 month duration) or bronchitis, pneumonitis, or esophagitis! Histoplasmosis, disseminated or extrapulmonary! Isosporiasis, chronic intestinal (>1 month duration)! Kaposi's sarcoma! Lymphoma, Burkitt's (or equivalent term)! Lymphoma, immunoblastic (or equivalent term)! Lymphoma, primary in brain! Mycobacterium avium complex or M. kansasii, disseminated or extrapulmonary! Mycobacterium tuberculosis, any site (pulmonary* or extrapulmonary)! Pneumocystis carinii pneumonia! Pneumonia, recurrent*! Progressive multifocal leukoencephalopathy! Salmonella septicemia, recurrent! Toxoplasmosis of brain! Wasting syndrome due to HIV * Added in the 1993 expansion of the AIDS surveillance case definition. 25

28 Appendix 2: Surveillance Notes Report Delay Report delay is defined as the time interval between the date an AIDS diagnosis was made and the date the case was reported. The median report delay for all cases reported in 1999 was 3 months (range 0 to 158 months). The impact of report delay must be considered when evaluating trends in case numbers and rates over time (Figure 1 and Tables 1, 4-8, 10, 11, 15-16, 18-19, 21). Rates For Table 4, Table 15, Table 16, Table 18, Table 19, and Table 20, AIDS case rates for cases diagnosed in 1990 or after are calculated using population estimates derived from the 1990 census. Exposure Categories Exposure categories are assigned in a hierarchical fashion (listed from highest to lowest in the column headed "Exposure Category"). Cases for which more than one exposure category have been identified are assigned to the category listed highest in the hierarchy. For example, a man who reports sexual contact with another man and heterosexual contact with an HIV-positive women would be classified as "male-male sexual contact." The only exception to this rule includes those men who report both sexual contact with another man and injection-drug use. A separate exposure category has been created for these cases. The "Undetermined" exposure category includes persons with no history of exposure to HIV through one of the defined exposure categories. Cases reported as undetermined include some persons still under investigation. If the investigation identifies a mode of exposure, the case is reclassified into the corresponding exposure category. AIDS-Indicator Conditions At the time of initial case report, all available information about AIDS-defining conditions is collected. However, after a case has been reported, systematic follow-up is not done to identify subsequent AIDS-defining conditions. Therefore, the information presented in Table 14 reflects initial AIDS-defining condition(s) and should be considered a minimum estimate of all AIDS-defining conditions. All persons meeting the CDC AIDS case definition are reportable as required by the California Code of Regulations, Title 17, Section If you have questions on how to report a case, would like a case report form and/or information on the case definition, would like assistance establishing an AIDS surveillance system, or to receive the quarterly surveillance summary and other periodic reports, contact the HIV Epidemiology Program at the address below: HIV Epidemiology Program Los Angeles County Department of Health Services 600 South Commonwealth Avenue, Room 805 Los Angeles, California Phone: (213) FAX: (213)
