3 Introduction Fourteen sexually transmitted infections (STIs) are currently notifiable in Ireland: ano-genital warts, candidiasis, chancroid, Chlamydia trachomatis, genital herpes simplex, gonorrhoea, granuloma inguinale, infectious hepatitis B, lymphogranuloma venereum, molluscum contagiosum, non-specific urethritis, Pediculosis pubis, syphilis and trichomoniasis. Aggregate data is collected quarterly from STI clinics including diagnosis, age group, and sex. The number of STIs notified by quarter, health board, age group and sex for quarter 1 (Q1) 2001 are presented in this report. Rates per 100,000 population are based on the 1996 population census and are reported in appendix A. It should be noted that cases of infectious hepatitis B that are sexually transmitted may also be reported through the weekly infectious disease report by NDSC. NDSC would like to thank all those who provided data for this report, particularly the STI Clinics and the Departments of Public Health Summary The total number of notified STIs has increased by 6.93% in Q1 2001, compared to the same quarter in The three most common notified STIs in Q were ano-genital warts, C. trachomatis and non-specific urethritis. The highest increases recorded during Q1 2001, compared to Q were for syphilis, gonorrhoea and genital herpes simplex. Notified STIs for Quarter In Q1 2001, 2377 cases of STIs were notified in Ireland, compared to 2223 during the same quarter in 2000 (table 1), representing a 6.93% increase in STI notifications. Notified cases of ano-genital warts, chancroid, C. trachomatis, genital herpes simplex, gonorrhoea, infectious hepatitis B, molluscum contagiosum and syphilis all increased during Q1 2001, compared to Q Significantly, notified cases of syphilis increased by 24.5 fold. Notified cases of candidiasis, non-specific urethritis, P. pubis and trichomoniasis decreased, whilst no cases of granuloma inguinale or lymphogranuloma venereum were notified during Q The cumulative rate per 100,000 population for all notified STIs increased in Q to per 100,000; compared to per 100,000 in Q (figure 1 & appendix A table 9). It is important to note that STI surveillance is mainly clinic based. At the time these notifications were made there were no STI clinics in the MHB and NEHB. For Q1 2001, 52.25% of all STI notifications were in the ERHA, 0% in the MHB, 10.43% in the MWHB, 0% in the NEHB, 7.70% in the NWHB, 10.10% in the SEHB, 11.02% in the SHB and 8.50% in the WHB. Notified STIs by Age Group for Quarter Notified cases of STIs by age group (years) are presented in table 3. Five percent (129) of notified cases of STIs in Q were 0 to 19 years old, 30.92% (735) were 20 to 29, 8.67% (206) were 30 to 39 and 2.52% (60) were aged over 40 years of age. Data represented in table 3 should be interpreted with caution, as the age group data for 52.46% (1247) of cases notified during Q was unknown. Rates are presented in appendix A table 10. 6 Notified STIs by sex for Quarter During Q1 2001, the majority of all cases of notified STIs were amongst males [53.05% (1261)] whilst 46.61% (1108) were amongst females (table 4). Data was not available for 8 cases. Figure 2 represents the percentage of male and female cases for each of the notifiable STIs, during Q The majority of cases of ano-genital warts, gonorrhoea, infectious hepatitis B, molluscum contagiosum, non-specific urethritis, P. pubis and syphilis were amongst males. The majority of cases of candidiasis, chancroid, C. trachomatis, genital herpes simplex and trichomoniasis were amongst females. Rates are presented in appendix A, table 11. 7 Notified sexually transmitted infections by quarter and year The total number of notified STIs decreased by 1.3% in Q1 2001, compared to Q (table 5). However, the total number of notified STIs for Q reached the highest recorded level for the first quarter (Q1) since 1995, increasing from 1122 in 1995 to 2377 in 2001, an increase of 111.9% (table 6) 1. It is also significant that the total number of STIs notified in Q is 6.69% higher than the total number of STIs notified in all 4 quarters in 1989 (table 7) 2. In Q1 2001, the number of notified ano-genital warts cases reached the highest level recorded for a single quarter 1 (tables 5 & 6). Notified candidiasis cases have reached the lowest level recorded for a single quarter 1. Notified cases of C. trachomatis have increased significantly since Q1 1999, particularly when the first quarter (Q1) is compared from 1995 to 2001 (table 6 & figure 3). Genital herpes simplex notified cases also increased during Q1 2001, following a decrease during Gonorrhoea and infectious hepatitis B notified cases have steadily increased since Q Nonspecific urethritis notified cases have decreased to their lowest level since Q The number of notified cases of P. pubis is higher in Q1 1999, 2000 and 2001 than for previous years. Notified cases of trichomoniasis decreased in Q1 2001, following a decrease in Q4 2000; however, were higher than for the first quarter in 1995 to The number of notified syphilis cases has increased rapidly since Q1 2000; primarily due to an outbreak of syphilis amongst men who have sex with men in Dublin. This is against a low incidence of syphilis throughout the 1990s, which in 1999 reached its lowest levels in 10 years.
