2 Executive Summary This study explores the functions that the liver contributes to the human body, and how these processes could be jeopardised by the hepatitis B virus. The liver has many functions, but it is especially important for removing toxins from the body. While acute hepatitis B infections are rarely fatal, chronic hepatitis B infections can disrupt the effectiveness of the liver in detoxification processes, and even lead to the development of cirrhosis and liver cancer. Autologous Stem Cell Treatment (ASCTreatment) has developed a unique stem cell product, Autologous Multi-lineage Potential Cells (AMPCs) that could potentially be beneficial for hepatitis B. Two different cases of AMPC intervention have demonstrated its ability to control hepatitis B virus activity within the infected patients. This opens up a promising pathway for hepatitis B treatment research, and further research could be conducted to determine its efficacy and feasibility. The liver and its functions The liver is an important part of the human body as it conducts a wide range of functions that maintain health. The roles the liver plays are so numerous that those with liver failure may only live up to two days. At a glance, the liver appears to manage nutrients by storing vitamins and sugars, controlling cholesterol levels, and secreting bile that aids in digestion. But on top of all this, it also carries out immune functions by producing immune factors that eliminate bacteria from the bloodstream, and by producing clotting factors that stop excessive bleeding from open wounds. The most important function of the liver, however, is to remove unwanted substances such as drugs from the body. The liver is able to carry out detoxification processes as it has the ability to replace its own cells. Liver cells are constantly being regenerated as the liver sustains continuous damage from removing poisonous substances from the body. In spite of this, the liver is highly resilient as it can still function even if 75% of the liver becomes damaged. However, excessive damage to the liver could result in hepatitis the inflammation of the liver that may subsequently cause liver cancer and liver failure. The liver can be damaged by lifestyle factors such as smoking or excessive alcohol consumption, but chronic hepatitis B infections can dramatically increase the risk of liver cancer development. 1
3 Hepatitis B Hepatitis B is a virus that attacks the human liver. It is the most common form of infection that contributes to severe liver damage. It is also transmissible among humans. The hepatitis B virus is transmitted through bodily fluids such as infected blood and semen. Common occurrences of hepatitis B transmission include sexual contact, needle-stick injuries, sharing of needles, and through mother-infant transmission. There are no known cures for hepatitis B current hepatitis B treatments do not eliminate the virus from the body and merely stops viral replication but it is preventable through vaccination. These vaccinations usually occur at birth, and are readministered as the infant grows. Though hepatitis B infections are common, most cases of acute hepatitis B infections are not fatal. Acute hepatitis B infections are usually symptomatic where symptoms of infection are visible and the body is able to eliminate the virus on its own accord. Only in rare cases with immunosuppressed individuals are acute hepatitis B infections fatal. Chronic hepatitis B infections occur commonly in unimmunised individuals and could lead to severe damage to the liver. Those with chronic infections are usually treated with antivirals that stop viral replication so that further damage to the liver can be prevented. Hepatitis B and liver cancer The hepatitis B vaccine is coined as the world s first anti-cancer vaccine as chronic hepatitis B infections can dramatically increase the risk of developing liver cancer. Chronic hepatitis B infections are very strenuous on the liver as the liver is constantly under attack by viruses. The soft, flexible tissue of the liver eventually becomes hardened as it sustains increasing damage. This is compounded by unhealthy lifestyle choices such as smoking and alcohol consumption that accelerate the deterioration of the liver. The nature of chronic hepatitis B infections also adds to this danger as they are usually asymptomatic the symptoms of which cannot be seen or felt. Preliminary damage to the liver causes scars to form on the liver tissue. This process, called fibrosis, prevents the liver from functioning optimally. As more damage is sustained, the numerous scars begin to overlap, leading to the next stage of deterioration, cirrhosis. Cirrhosis occurs when scars on the liver tissue become so severe that blood flow within the liver is disrupted. This leaves some liver tissue with poor blood supply, and these tissue begin to shrink and harden. From this point, cirrhosis can deteriorate into liver cancer or cause liver failure. Liver failure occurs when cirrhosis becomes so severe, and blood flow becomes so stunted, that the detoxification function is made impossible. Blood clotting factors are also no longer produced, which could cause excessive blood loss if other parts of the body are wounded. Liver failure is often fatal. Cirrhosis can also lead to liver cancer. The weakened state of the liver is still open to attack from hepatitis B viruses, but is no longer as resilient as its healthy state. The continuous attack may alter the genes within the liver cells that make them cancerous, eventually causing liver cancer. Since chronic hepatitis B infections are asymptomatic, those infected should regularly undergo screening to detect any occurrences of cancer development. 2 issue 1:
4 Autologous Multi-lineage Potential Cells (AMPCs) Autologous Stem Cell Treatment (ASCTreatment) is a company that develops autologous stem cells for medical treatments. Stem cells are cells that have the potential to transform (or differentiate) into other specialised cell types within the body (muscles cells, blood cells, etc.). Adult stem cells are only able to differentiate into a small range of cell types; while the stem cells that are found in newborn infants are pluripotent meaning they can differentiate into all cell types. Autologous, within the context of stem cells, means that the cells are harvested from, and then administered to the same individual. This minimises the risk of rejection and eliminates the risk from genetic manipulation. ASCTreatment s Autologous Multi-lineage Potential Cells (AMPCs) are both autologous and pluripotent. These stem cells have the potential to heal the body from deteriorating conditions by regenerating and replacing old and damaged cells. In the case of the liver, AMPCs are able to differentiate into liver cells to replace the damaged and worn liver cells. This provides an additional source of liver cells that could aid in the detoxification process, essentially restoring the liver to its healthiest state. AMPCs have already been administered in over 200 instances, all of which have reported beneficial outcomes. These treatments are administered for a range of different conditions, but there have been two documented instances where AMPCs were shown to affect hepatitis B infections. AMPC and hepatitis B two case studies Two patients who have undergone AMPC treatment for hepatitis B have now had the virus under control. Viral presence in both these cases is measured in hepatitis B surface antigen (HBsAg) and hepatitis B surface antibody (HBsAb) levels. HBsAg is the surface antigen part of the virus molecule of the hepatitis B virus, and presence of these molecules within the blood stream shows that the virus is active and replicating. HBsAb are antibodies produced by the body in response to the presence of HBsAg. The presence of HBsAb will afford the body protection against the hepatitis B virus. 3
5 Patient A : Male, 50 years old BEFORE Patient A is a sufferer of hepatitis B infection who chose to undergo AMPC intervention on 10 August Prior to this, a test to determine hepatitis B infection status was conducted on 22 July 2013, and the HBsAg status was positive, indicating the presence of hepatitis B virus within the patient s body. A blood test conducted on 22 March 2013 also showed a hepatitis B viral load of <20 IU/mL. This is indicative of high viral infectivity within the body. 6 Patient B: Male, 55 years old BEFORE Patient B is a chronic sufferer of hepatitis B. Blood tests for patient B conducted on 5 Jan 2010 and 15 August 2011 showed HBsAg-positive and HBsAb-negative statuses respectively. The presence of hepatitis B antigen and the absence of hepatitis B antibodies indicate high viral activity. 7 AFTER AMPC intervention occurred on 1 August 2013, and screening for hepatitis B status on 23 April 2014 showed HBsAg-negative and HBsAb-positive. These results show that the hepatitis B virus has been successfully controlled by the immune system. Significant damage to the liver from cirrhosis or liver cancer is severely detrimental to the body. Hepatitis B poses a health risk for the public as it may lead to cirrhosis and even liver cancer. While hepatitis B vaccines are effective in preventing acute and chronic infections, those who are unimmunised remain susceptible to the virus. Current treatments of hepatitis B only serve to cease viral activity but not eliminate the virus from the system. AMPC interventions in both cases mentioned above were shown to positively affect hepatitis B infection by reducing viral loads or by stimulating immune function. While the research on AMPC effect and hepatitis B is still preliminary at this point, these results show a potential avenue for hepatitis B treatment research. The potential is greater with AMPC as it also aids in convalescence due to the nature of stem cells. Future research could further examine the effect of AMPC on hepatitis B infections to verify the beneficial effects that it could confer to those ailing with the virus. Disclaimer Note that the results of stem cell treatments administered in these two instances were the culmination of multiple factors that could vary among individual patients. Although stem cell therapy has been demonstrated to alleviate symptoms of multiple disorders, ASCTreatment can neither predict nor guarantee the success of each patient who undergoes the stem cell therapy procedure. This is due to the nature of all medical interventions where there is an inherent risk attached.
