2 What is the Accueil Clinique? A new organizational model established to help patients with specific sub-acute and semi-urgent clinical symptoms and conditions gain rapid access to Jewish General Hospital diagnostic testing, Specialist consultation, and treatments Patients are referred to the Accueil Clinique by Family Doctors from within the CIUSSS Centre-Ouest de L Ile de Montreal territory

5 Accueil Clinique Referral Process Family Doctors from our CIUSS may refer patients with first trimester bleeding, suspected TIAs, and suspected DVTs to the clinic In order to be accepted into the clinic, patients must meet the clinic eligibility criteria (general and conditionspecific) The Family Doctor completes and signs the condition-specific referral form *** Signed referral forms include medical orders for blood tests, imaging, and specialty consultation. These medical orders will be utilized by the Accueil Clinique Nurse to provide patient care using condition-specific clinical algorithms. *** The Family Doctor faxes or s the completed referral form to the Accueil Clinique

6 Accueil Clinique General Eligibility Criteria 16 years or older Independent with ambulation or ambulatory with the assistance of one person Patients with unstable mental health or cognitive impairments must be accompanied to appointment Does not require urgent medical care (<24 hours) Does not have condition specific exclusion criteria Referring doctor has duly completed a referral form and has assured follow-up or has identified the doctor agreeing to provide patient follow-up

7 Accueil Clinique Process Once the referral form is received, the clinic Nurse will confirm its receipt with the Family Doctor by telephone or by The Nurse then calls the patient to book a same day or next day appointment at the Accueil Clinique with the clinic Nurse At the appointment, patient care requirements are determined using the condition specific referral form and clinical algorithm, and may include blood tests, imaging, treatments, and specialty consults within a specified time frame Family Doctor referral form functions as order for testing, medication administration, specialty consultation Diagnostic imaging is arranged using the clinic s pre-determined slots Once testing and consultations are completed, results will be faxed or ed to the referring physician by the Accueil Clinique nurse If a patient becomes medically unstable during process, the patient is referred directly to the JGH Emergency Department
