2 2 Zimmer Modular Unipolar Femoral Head Surgical Technique Introduction The Modular Unipolar Femoral Head construct consists of a CoCr Unipolar Head, and a CoCr Unipolar Head Adapter. The Unipolar Heads are available in 1mm increments from 38mm to 60mm and the Unipolar Head Adapters are available in -4.0mm, +0mm, +4mm, and +8mm offsets. Preparing the Acetabulum Examine the hip joint for congruency and articular lesions, and leave the labrum and transverse ligament intact. Once the femoral head has been resected, measure it using the head gauge or calipers (Fig. 1) and select an endoprosthesis trial. Try measuring 1mm over and under to obtain the closest fit. Femoral Preparation and Implantation For detailed instructions on the osteotomy of the femoral neck, preparation of the femur, trial reduction of the femoral implant, insertion of the femoral stem, and post-operative management, please refer to the femoral implant s specific surgical technique, or contact your Zimmer representative. 3 Zimmer Modular Unipolar Femoral Head Surgical Technique 3 Trial Fit Connect the appropriate endoprosthesis trial to the trial handle using the snap-fit tip that is threaded onto the handle (Fig. 2). Trial Reduction Place the appropriate Unipolar Trial Adapter into the endoprosthesis trial, and attach it to the trial stem and perform a trial reduction (Figs. 4 & 5). Note: The adapter of the trial handle comes in 22mm and 26mm sizes. At this time, make sure that the endoprosthesis trial is appropriately fit (Fig. 3). There should be no play in any direction. 4 4 Zimmer Modular Unipolar Femoral Head Surgical Technique Unipolar Assembly There are two options to assemble the Unipolar Head and the Unipolar Head Adapter. Option 1 Ensure that the Unipolar Head taper, and the exterior taper of the Unipolar Head Adapter are clean and dry. Place the Unipolar Head Adapter into the Unipolar Head (Fig. 6). Ensure that the inner taper of the Unipolar Head Adapter and the neck of the femoral stem are clean and dry. Assemble the Unipolar Head Construct onto the femoral neck and impact it with the femoral head impactor and mallet. Test the security of the Unipolar Head Construct fixation by trying to remove it by hand (Fig. 7). Fig. 6 Fig. 7

5 Zimmer Modular Unipolar Femoral Head Surgical Technique 5 Option 2 Ensure that the inner taper of the Unipolar Head Adapter and the femoral neck are clean and dry. Place the Unipolar Head Adapter onto the femoral neck (Fig. 8). Ensure that the tapers of the Unipolar Head and the Unipolar Head Adapter are clean and dry. Assemble the Unipolar Head onto the Unipolar Head Adapter (Fig. 9) and impact the head with the femoral head impactor and mallet. Test the security of the Unipolar Head Construct fixation by trying to remove it by hand. Fig. 8 Fig. 9

6 6 Zimmer Modular Unipolar Femoral Head Surgical Technique Disassembly of the Unipolar Components Position the rod of the Disassembly Tool approximately one inch from the end of the tool by unthreading the rod (Fig. 10). Compress the tip and place the Disassembly Tool into the Unipolar Head Adapter. Fig. 10

7 Zimmer Modular Unipolar Femoral Head Surgical Technique 7 Twist the T-handle to reposition the rod downward and engage the end of the Disassembly Tool with the Unipolar Head Adapter. Additional twisting will disengage the Unipolar Head Adapter from the Unipolar Head (Figs. 11 & 12). Endoprosthesis Removal Disassociation of the Unipolar Head Construct from the femoral component can be accomplished by striking the inferior surface of the head with a punch and mallet.
