Its is Important to remark that the therapeutical effectiveness of these medications are seen in the new colonoscopy, performed 7 days after the initiating the treatment. We could overcome the nodular stenosis previously described, by finding the ileocecal valve destroyed, by the disease; after passing throughout the valve, we could exam around 30 cm of the terminal ileum finding an active disease. The video clip displays passing of the colonoscope through to the first nodular stenosis, displaying a second transverse colon lesion which is nodular and ulcerated also. terminal ileum in particular--is most likely to be involved. while African Americans are at decreased risk, which. Various ulcerations with nodules through the right colon near the cecum were found. The colon was found to measure 120 CM. long. Button batteries are an occasional source of foreign body ingestion, most frequently by TEENren, but occasionally by adults. The chemical composition of the batteries is variable, but they may contain potassium or sodium hydroxide, and salts of mercury, zinc, or other metals. A knowledge of the diameter of the battery and the identification of the chemical system is helpful in patient management. Batteries with a diameter of greater than 15 mm usually contain mercuric oxide. Following ingestion, most batteries pass uneventfully through the gastrointestinal (GI) tract. However, if the battery becomes impacted in the esophagus, at some other site, or aspirated into a bronchus, it must be removed. The impacted battery can produce a local pressure necrosis. Furthermore, if the battery opens, the leakage of alkali can result in an area of liquefaction necrosis with subsequent perforation. In addition, the systemic absorption of heavy metal salts can occur and is of concern. The standard recommendation has been to advance the endoscope only as far as the first circumferential second-degree burn or any third-degree burn because the risk of iatrogenic perforation is felt to be high. Some authors advocate the complete evaluation of the esophagus and stomach using a flexible endoscope until an area of obvious necrosis is identified. If frank necrosis is identified, many authors advocate surgical intervention to evaluate the extent of injury and to remove dead tissue. The presence of esophageal or gastric burns is an indication for hospitalization. Patients should have nothing by mouth. Repeated endoscopy during the first 2 to 3 days is helpful in the identification of progressive injury requiring surgical intervention. In addition, it also allows for the insertion of stents, strings, and feeding tubes that may help maintain esophageal lumen patency while healing occurs. Endoscopy should be avoided from the fifth day until one and one half weeks following ingestion. At this time the risk of perforation increases due to wound softening that occurs as healing progresses. Endoscopy is contraindicated in patients with necrosis of the pharynx or in patients in respiratory distress who have not undergone airway management. The ingestion of solid alkaline agents will result in severe oropharyngeal pain. The mouth and pharynx may have a soapy white film over the mucosa that eventually becomes brown and edematous. Perforation of the esophagus can produce severe chest pain from subsequent chemical and later bacterial mediastinitis. Imaging Contrast-enhanced visualization of the esophagus is not recommended in the initial evaluation of esophageal or gastric burns. Most authors agree that these studies are rarely sensitive enough for the detection of mucosal injuries. Some features of injury evident from esophagrams are the intramucosal retention of contrast material and gaseous dilatation of esophagus due to air trapping. The blurring of mucosal margins, linear streaking of contrast material and abnormal displacement of the pleural reflection may be seen. Following the third week postingestion, serial barium swallows will identify the development of strictures. or mass. Patients may also have mild anemia, leukocytosis, Prior to further evaluation or diagnostic

In the remaining quarter, the entire colon is involved, with no skip areas. Ulcers and nodules of the sigmoid and descending colon. perirectal abscess. Involvement of the small intestine can. When ingested, alkaline agents, such as sodium hydroxide and potassium hydroxide, may result in severe injury to the gastrointestinal tract. With a ph of 12.5 or greater, they produce an injury described histologically as liquefaction necrosis. Solid alkali, when swallowed, tends to adhere to the mucous membranes of the oropharynx and esophagus, producing severe proximal burns. In contrast, liquid lye, a strong alkali found in household drain cleaners such as Drano, Liquid Plumr and Plunge is easier to swallow, tasteless, and odorless. It causes less damage to the oropharynx and greater damage to the esophagus. In addition, liquids, because of their high specific gravity, can pass rapidly through the esophagus and reach the stomach, injuring tissue on contact. Terminal Ileum shows tiny red spot with scanty fibrinoid areas. Some biopsies are taking, histopathology which are shown below. Nodular stenosis due to Crohn's disease. The transverse colon has two nodular and ulcerated stenosis. However, this stenosis was overcome by passing the colonoscope to the next lesion. The Association of H. pylori infection and patterns of erythematous gastric mucosa. Chronic gastritis implies some degree of atrophy (with loss of function of the mucosa) or metaplasia. It predominantly involves the antrum (with subsequent loss of G cells and decreased gastrin secretion) or the corpus (with loss of oxyntic glands, leading to reduced acid, pepsin, and intrinsic factor). People with Helicobacter pylori infection are 3 to 6 times more likely to develop stomach cancer. Which of the following cancers is NOT associated with H. pylori infection? Gastritis is inflammation of the gastric mucosa caused by any of several conditions, including infection ( Helicobacter pylori ), drugs (NSAIDs, alcohol), stress, and autoimmune phenomena (atrophic gastritis). Many cases are asymptomatic, but dyspepsia and GI bleeding sometimes occur. Diagnosis is by endoscopy. Treatment is directed at the cause but often includes acid suppression and, for Helicobacter pylori infection, antibiotics. Find information on medical topics, symptoms, drugs, procedures, news and more, written for the health care professional. Loss of Heterozygosity Validated As Marker for Oral Cancer. Gastritis is classified as erosive gastritis or nonerosive gastritis based on the severity of mucosal injury. It is also classified according to the site of involvement (ie, cardia, body, antrum). Gastritis can be further classified histologically as acute or chronic based on the inflammatory cell type. No classification scheme matches perfectly with the pathophysiology; a large degree of overlap exists. Some forms of gastritis involve acid-peptic and H. pylori disease (see also Overview of Acid Secretion ). Additionally, the term is often loosely applied to nonspecific (and often undiagnosed)

Biopsy sampling of gastric mucosa at diagnostic endoscopy provides information that cannot be obtained by other means. The most common indication for gastric biopsy is the need to know whether or not the patient is infected with Helicobacter pylori, and whether the stomach is gastritic or not. Microscopic examination of gastric biopsy specimens, in addition to H. pylori status, provides information about the grade, extent, and topography of gastritisrelated and atrophy-related lesions in the stomach. This information provides further opportunities for assessing the risk and likelihood of various gastric disorders. These are: a) The predominance or restriction of the H. pylorirelated gastritis in the antrum strongly correlates with an increased risk of peptic ulcer disease, and of duodenal ulcer in particular (the duodenal ulcer phenotype of gastritis). b) The presence of atrophic gastritis (loss of normal glands) in the area of the gastric body indicates a low risk of ulcer and also a reduction in the capacity of the patient to secrete acid. c) The occurrence of advanced atrophic gastritis and intestinal metaplasia multifocally in the stomach (advanced multifocal atrophic gastritis), and in the lesser curvature and angular notch in particular, are features suggestive of an increased risk of gastric neoplasias (the gastric cancer phenotype of gastritis). d) The presence of normal and healthy gastric mucosa indicates, on the other hand, an extremely low risk of both peptic ulcer disease and gastric cancer. In addition to diagnosis of H. pylori-related gastritic lesions, routine gastric biopsies may reveal findings that indicate special forms of gastritis, such as eosinophilic, lymphocytic, reactive, or granulomatous gastritis (e.g., Crohn's gastritis), or Helicobacter heilmannii gastritis. These types of gastritis can be found incidentally in a small percentage of patients who undergo diagnostic gastroscopy for abdominal complaints. or mass. Product identification, when possible, is extremely helpful and allows health care workers to form judgments about the likely quality and extent of damage and will help in the anticipation of potential complications. Additional treatment for esophageal strictures is undergoing further study. Substances with lathyrogenic properties have been reported to prevent stricture formation in animals. ß- Aminopropionitrile inhibits intermolecular covalent cross-linking in newly formed collagen and decreases the tensile strength of scar tissue. Penicillamine has also been used experimentally in animal studies. It inhibits lysine-derived aldehyde groups and prevents the cross-bonding of collagen. Currently, these treatments are not available or recommended for human use. This lesion belong to the second stenosis of the transverse colon, which is nodular and ulcerated. Prophylactic bougienage has been started as early as the week following ingestion. However, many authors believe that dilatation prior to the third week following ingestion is contraindicated because the risk of trauma and perforation is high. If stricture formation is apparent, steroid administration may be reduced and dilitation begun cautiously. Strictures of moderate degree may be dilated successfully by antegrade bougienage. In patients with more severe stricture formation, retrograde dilitation using an indwelling string is considered by some the safest and most successful method, but it requires a gastrostomy. Strictures that do not respond to dilatation may require surgical reconstruction and colonic interposition. Surgical management at this point is controversial. Many authors advocate esophagectomy and colonic interposition as prophylaxis against the later development of esophageal carcinoma. Some believe that the risk of thoracotomy and

The ingestion of acids occurs less frequently but results in a higher rate of complications and mortality. Acids produce coagulation necrosis and eschar formation, which allegedly protects underlying tissue from further caustic damage. On ingestion, acids have their major effect on the columnar epithelium of the stomach. The esophagus tends to be spared. This effect has been attributed to a rapid transit time through the esophagus and to the resistance of esophageal squamous epithelium to acid injury. Once the acid reaches the gastroesophageal junction it tends to follow the lesser curvature of the stomach, pooling in the antrum and eventually inducing pylorospasm and extensive antral damage. Destruction of the gastric mucosa and musculature results in edema, inflammation, the loss of glandular tissue and sometimes immediate or delayed hemorrhage. Eventually, if the initial injury is survived, fibrosis develops and may result in gastric outlet obstruction. Serious ingestions may result in complete gastric necrosis, hemorrhage,

8 early perforation, and the destruction of adjacent structures. In addition, acids are well absorbed and may produce a profound systemic acidemia as well as massive hemolysis.
