Driman, MBChB * Objective: To survey and imrove the athological reorting of colorectal cancer (CRC) secimens in a tertiary care athology deartment. Methods: We identified CRC secimens reorted in a 6-month eriod before and after educational sessions and the introduction of a standardized CRC synotic reorting rotocol. Gross and microscoic descritions were analyzed according to ublished guidelines for imortant staging and rognostic features. We then reexamined these arameters for a further 6-month eriod 15 months later to ensure that the quality of reorting had been maintained. Results: In total, 108 and 166 cases were analyzed before and after standardization, resectively. Many features were reorted aroriately, including tumour size, tye and grade, deth of invasion, nodal status and roximal and distal margin status. Several underreorted features showed significant imrovement after standardization, including serosal involvement (reorting increased from 22% to 84%), distance to radial margin (from 14% to 64%), extramural venous invasion (from 18% to 88%), host resonse (from 19% to 94%) and mean number of nodes retrieved (mean numbers retreived increased from 11 to 16). The subsequent review 15 months later showed continued long-term imrovement in these areas. Conclusion: Education and synotic reorting significantly imroved CRC reorting at our centre. Objectif : Examiner et améliorer la roduction de raorts de athologie ortant sur des sécimens de cancer colorectal (CCR) dans un déartement de athologie en soins tertiaires. Méthodes : Nous avons relevé les sécimens de CCR qui ont fait l objet d un raort au cours d une ériode de six mois avant et arès les séances de formation et la mise en œuvre d un rotocole normalisé de roduction de raorts synotiques sur le CCR. Nous avons analysé les descritions macroscoiques et microscoiques en fonction des lignes directrices ubliées our les caractéristiques rinciales de la détermination du stade et du ronostic. Nous avons ensuite réexaminé ces aramètres endant 6 autres mois 15 mois lus tard our assurer que la qualité des raorts s était maintenue. Résultats : Au total, nous avons a analysé 108 et 166 cas avant et arès la normalisation, resectivement. Our objective in this study was to assess the athological reorting of colorectal cancer secimens in a high-volume, tertiary care, academic athology deartment, from the ersective of the recommendations of the College of American Pathologists (CAP) 2 and to assess the effects of an educational rogram and introduction of standardized, synotic-based reorting. Methods The initial retrosective review included all colorectal carcinoma secimens reorted at our institution in a 6-month eriod ending in November 2002 and identified by a comuter search. We analyzed the reorts for the secific staging and rognostic features included in the CAP guidelines for the reorting of CRC. 2 These arameters were classified as either gross (macroscoic) or microscoic, reflecting the 2 main stages of secimen rocessing. Gross features examined were tumour size, status of the serosa, measured distance of tumour to roximal, distal and radial margins and deth of mesenteric or erirectal soft tissue. Microscoic features assessed were histological tye, histological grade, deth of invasion, serosal involvement, lymhovascular sace (small vessel) invasion, extramural venous (large vessel) invasion, erineural invasion, host resonse, roximity of tumour to roximal, distal and radial resection margins and lymh node status. Included in this list of arameters are some that are not secifically required by the CAP. We included some, such as host resonse, for the sake of academic comleteness. We felt it was imortant to include others such as serosal status and deth of mesenteric or erirectal soft tissue. Because of the imortance of accurate documentation of serosal involvement in colonic cancer, it is necessary to secifically target areas susicious for this at gross examination so that aroriate histological sections can be made. Recording the deth of mesenteric or erirectal fat is a otentially useful gross descritor that may correlate with lymh node yield. we evaluated these reorts, educational sessions were given to athologists, residents and athologists assistants. Imortant asects of gross examination and microscoic reorting as outlined in the CAP guidelines were reviewed. Anatomical diagrams showing the relation of the serosal coverings of the colon, 3 checklists and grossing temlates for CRC were laced at each rosector s bench to act as visual aids. Continued reminders for reorting of various arameters were discussed at deartmental rounds. To facilitate the reorting of microscoic findings, we constructed synotic reorts in which a checklist highlighted the imortant staging and rognostic features. Initially, the checklist was a aer document comleted by the athologist and then transcribed by the transcritionist, who incororated it into a synotic form that was inserted into the athology reort. Our deartment has subsequently moved to an electronic synotic reorting form incororated into our laboratory information system. Educational sessions also focused on the imortance of finding as many lymh nodes as ossible in a resection secimen; it was stressed that at least 12 nodes were needed to accurately redict regional node status. When fewer than 12 nodes were found, redissection of the secimen by either the same or a different rosector was encouraged. A lymh node highlighting solution was also made available for use when fewer than 12 lymh nodes were found. This solution is used after formalin fixation and has been reviously demonstrated to have low toxicity and good efficacy. 4 imlementing the standardization strategies, we reviewed CRC secimens in a subsequent 6-month eriod ending August 2003 for the same gross and microscoic features. Using χ 2 analysis, we comared our findings with the original outcomes. Using a aired t test analysis, we comared the mean number of lymh nodes found before and after standardization. We then reeated the rocess 2 years later (ending May 2006) to assess whether the quality of reorting had been maintained over time. Results For the 6-month eriod ending in November 2002, 108 cases of CRC were retrieved: 64 colonic and 44 rectal tumours. The gross examinations were erformed by athologists, residents and athologists assistants. Although features such as tumour size and the distances of tumour from roximal and distal resection margins were well reorted (in 100% of the cases), other features such as involvement of serosa (in 22%), distance of tumour from resection radial margin (in 14%) and deth of mesenteric or erirectal soft tissue (in 9%) were underreorted (Table 1). Microscoic features that were consistently well reorted (in 98% 100% of the cases) included tumour histological tye, deth of invasion, lymh node status, assessment of roximal and distal resection margins and histological tumour grade. Features that were underreorted (in 7% 81%) included lymhovascular sace invasion, extramural venous invasion, erineural invasion, host resonse and radial resection margin status (Table 2). standardization, 116 CRC cases were retrieved: 81 colonic and 35 rectal tumours. Gross examinations were erformed by a similar mix of athologists, residents and athologists assistants. There were statistically significant imrovements in all underreorted gross arameters (64% 84%), which included involvement of serosa, distance of tumour to radial resection margin and deth of mesentery or erirectal soft tissue (Table 1). 3 Chan et al. imrovements in reorting of microscoic features (88% 99%) included lymhovascular sace invasion, radial resection margin status, extramural venous invasion, erineural invasion and host inflammatory resonse (Table 2). Prior to standardization, the mean number of nodes recovered from each resection secimen was 11 (standard deviation [SD] 7); this increased to 16 (SD 8) ( < 0.001) after standardization. The ercentage of cases with 12 or more lymh nodes found was 39% before and 68% after standardization ( < 0.001, Table 3). A subsequent 6-month review undertaken 15 months later revealed 113 cases, including 87 colonic and 26 rectal adenocarcinomas. Reorting of gross arameters continued to imrove in comarison with results shortly after standardization (Table 1). An imrovement in reorting of microscoic features was achieved, with 100% of cases meeting arameters on the synotic reort (Table 2). The mean number of lymh nodes recovered from each secimen increased to 18 (SD 10); in 81% of the cases, 12 or more lymh nodes were found. Discussion Pathologists lay an imortant role in the care of colorectal cancer atients, since accurate staging, rognostic information and treatment decisions are based on the results of athological examination and reorts. Our study goals were to review colorectal cancer athology reorts at a high-volume, tertiary care academic athology deartment, from the ersective of the CAP recommendations and to standardize CRC reorting. Our results show that education and the use of grossing temlates and synotic reorts significantly imroved CRC athology reorting. Gross features such as the resence or absence of serosal involvement, distance to the radial margin and deth of mesenteric or erirectal soft tissue were underreorted. 4 Colorectal cancer athology reorting Underreorted microscoic arameters included radial resection margin status, extramural venous invasion and host inflammatory resonse. Large venous invasion should be reorted searately from small lymhovascular sace invasion because venous invasion indicates risk for heatic metastases, while lymhovascular invasion indicates risk for lymh node metastases. Perineural invasion reflects infiltrative growth and has been shown to be an indeendent indicator of oor rognosis. Host resonse, an indicator of good rognosis, includes lymhocytes at the edge of the tumour, Crohn-like lymhocytic aggregates and intratumoral lymhocytes. 5 Prior to standardization, the number of lymh nodes collected from each secimen was just below the recommended minimum of 12; after standardization, this imroved to a mean of 16 (and then 18) nodes, and the roortion of cases with 12 or more nodes found more then doubled. This arameter is of articular imortance because studies have shown that a minimum of 12 nodes is needed to accurately redict regional node status. 6 8 The detection of even a single ositive lymh node is sufficient to refer the atient for adjuvant chemotheray and is the most imortant determinant of survival. 9 The variability in the number of nodes recovered may reflect secimen size, the amount of ericolic or erirectal tissue, individual anatomical variability, surgical technique and the skill of the rosector; however, many lymh nodes, articularly those 1 2 mm in size are often missed. 10 The increase in lymh node yield was due to recognition of the minimum number of lymh nodes recommended for reorting, the re-examination of secimens with fewer than 12 lymh nodes and the use of a lymh node highlighting solution, a simle and inexensive aid in this task. 4 At our institution, the need for continued imrovement in the reorting of some arameters, esecially various gross features, is being addressed by continued education of staff through rounds, diagrams at each rosector s desk and reminders to take a second look at all CRC resection secimens with fewer than 12 lymh nodes. A recent survey of Ontario athologists (ublished in 2004) 11 showed that only 57.9% of athologists were aware of guidelines for lymh node retrieval in CRC secimens, with only 25.0% able to identify that a minimum 12 nodes are necessary to redict node negativity. Another reort showed that 73% of 8848 CRC cases were designated as node-negative from the assessment of fewer than 12 nodes. 12 Barriers secific to retrieval of greater numbers of lymh nodes in CRC secimens have included not only unfamiliarity with recommendations but also time ressures and shortage of ersonnel, as well as secimens with minimal mesenteric or erirectal soft tissue. 11 Continued education regarding udated guidelines for roer assessment of surgical secimens is imortant; however, it may not always translate into altered or imroved ractices. If new guidelines are to be imlemented successfully, hysician and worklace factors necessary for their adotion must be considered. 13 At our institution, we successfully imlemented the new guidelines for reorting CRC by resenting them with suortive data, by continued reminders for reorting various arameters at deartmental rounds and by introducing a user-friendly synotic checklist. It is also ossible that audits administered by Cancer Care Ontario (the rovincial body through which the Ontario Cancer Registry collects data on cancer atients and cancer reorting) introduced during the second follow-u eriod further enhanced comliance. Surveys of surgeons and oncologists at our institution have shown that the synotic reorts are well acceted as standardized reorts because they rovide easy access to all relevant information. Feedback from athologists, residents and secretarial staff has also been favourable. The effectiveness of our educational rogram and the introduction of synotic reorting has also been demonstrated by data from Cancer Care Ontario. 14 In conclusion, for the rocessing and diagnosis of CRC cases, our data demonstrate the clinical value of imlementing a standardized rocedure including synotic reorting and illustrate how education and worklace modifications lay a role in imroving cancer reorting.
