6 INTRODUCTION An association between occupation and health has been recognised for centuries. As early as 1713 Ramazzini, the father of occupational medicine, produced a general work on the health effects of over sixty occupations(1). In 1775, Pott discovered a relationship between scrotal cancer and chimney sweeping. Farr's work in Britain lead to the ten-yearly publication of the Registrar General's Occupational Mortality Supplement in 1851 (1). Epidemiological studies of occupational health have identified a number of health risks in specific occupations. For example, pneumoconiosis in coalworkers-(), lung cancer among chromate workers, mesothelioma, lung and gastrointestinal cancer in asbestos workers (3). The purposes of this study are to describe the pattern of male occupational mortality in NewZealand during , and to provide the stimulus for the future investigation of specific aetiological hypotheses. The study, therefore, extends the previous work by Copplestone and Rose (4) and Pearce et al (5). METHODOLOGY This study was limited to the mortality among males aged -64 years. The numerator data on various causes of death (Appendix 9) were extracted from the National Health Statistics Centre computer tapes containing records of all deaths registered in New Zealand during The cause of death was classified according to the 9th revision of the International Classification of Diseases (ICD-9). The denominator data, i.e. all males aged -64 in the full-time labour force, were obtained from the 1981 New Zealand Census and supplied on computer tape by the Department of Statistics. Occupations in both the numerator and denominator were classified according to the New Zealand Standard Classification of Occupations (6). Differential reporting of a person's occupation on census forms and death certificates has been found to be a minor bias in Britain (7). The general effect is to reduce, rather than increase the observed mortality gradient (8). No assessment could be made of the effects of a previous occupation. The indirect method of standardisation was used to adjust for differences between the age structures of the various occupations. 8 DISCUSSION These comments highlight the salient features of table one (page 7), which summarises the comprehensive tables in the appendices. TOTAL MORTALITY Clerical, sales and related workers, legislative and government administration officials, jurists, teachers, accountants, architects, engineers, life scientists, medical, dental, veterinary and related technicians recorded fewer deaths than the national population. There was also a favourable mortality among housestaff and related housekeeping service workers, protective service workers, building caretakers, charworkers, cleaners, farmers, agricultural, animal husbandry workers, farm managers and supervisors. Fewer than expected deaths were recorded among production supervisors, general foremen, metal processers, chemical processers, tanners, felimongers, peltdressers, electrical fitters and related electrical and electronics workers, rubber and plastic product makers, printers, stationary engine and related equipment operators. In contrast, there was high total mortality among working proprietors (catering and lodging services), cooks, waiters, bartenders, hairdressers, barbers, beauticians, fishermen, hunters and forestry workers, aircraft and ships officers, miners, quarrymen, well drillers, spinners, weavers, knitters, dyers, food and beverage processers, painters, bricklayers, carpenters and other construction workers, transport equipment operators, and labourers not elsewhere classified (n.e.c.). CAUSE SPECIFIC MORTALITY Professional, Technical and Related Workers While aircraft and ships officers had a high number of deaths from injury and poisoning, the mortality was low among accountants, medical, dental, veterinary professions and related technicians. Accountants also experienced a low number of deaths from cancer. Few deaths from injury and poisoning were experienced by teachers, architects, engineers and related technicians. These occupations recorded a low mortality from lung cancer, ischaemic heart disease, motor vehicle accidents, and respiratory diseases. Teachers had few deaths from suicide and cerebrovascular disease, and there was a low risk of death from diseases of the circulatory system among sculptors, painters, photographers and related creative artists. There was a high mortality from melanoma of the skin among architects, engineers and related technicians, and from cancer of the brain among workers in religion. Life scientists, jurists, medical, dental, veterinary and related technicians experienced few deaths from ischaemic heart disease. Lung cancer mortality was low amongst accountants, medical, dental, veterinary and related technicians.
9 Administrative and Managerial Workers Managers were at a low risk of death from diseases of the respiratory and nervous systems, endocrine, nutritional and metabolic diseases and other immunity disorders, cirrhosis of the liver, non-ischaemic/non-hypertensive heart disease, and injury and poisoning. However, they had a high mortality from ischaemic heart disease, and cancers of the large bowel and lymphatic system. Clerical and Related Workers There was a low number of deaths from cancer and ischaemic heart disease among clerical supervisors and government executive officials, mail distribution clerks, and bookkeepers, cashiers and related workers. Bookkeepers, cashiers and related workers also experienced a low mortality from injury and poisoning, motor vehicle accidents, diseases of the respiratory system, and lung cancer. The risk from lung cancer was low among government executive officials. Transport and communication supervisors had an elevated risk from cancer of the kidney, but a favourable mortality from injury and poisoning. Clerical and related workers n.e.c. were at low risk from injury and poisoning, motor vehicle accidents, endocrine, nutritional and metabolic diseases, and immunity disorders. Sales Workers The mortality from lung cancer, ischaemic heart disease, injury and poisoning was low among insurance, real estate, securities, business services salespersons, auctioneers, managers (wholesale and retail trade), salespersons, shop assistants and related workers. The risk of death was low among working proprietors (wholesale and retail trade) from injury and poisoning, but the number of deaths from ischaemic heart disease and cirrhosis of the liver exceeded the New Zealand average. Mortality from diseases of the respiratory system was favourable amongst managers (wholesale and retail trade), working proprietors (wholesale and retail trade), salespersons, shop assistants and related workers. Technical salespersons, commercial travellers, and manufacturers agents experienced few deaths from injury and poisoning, ischaemic heart disease, motor vehicle accidents, and suicide. The mortality from motor vehicle accidents was also low among salespersons, shop assistants and related workers. Sales supervisors and buyers recorded few deaths from malignant neoplasms and ischaemic heart disease.
10 Service Workers Mortality from ischaemic heart disease was low among managers (catering and lodging services), building caretakers, charworkers, cleaners, and related workers, but high for working proprietors (catering and lodging services). Building caretakers, charworkers, cleaners and related workers recorded few deaths from malignant neoplasms, diseases of the respiratory system, injury and poisoning, and cancers of the lung and large bowel. Protective service workers (e.g. Fire fighters, Police, warders, guards and those in the armed forces) had a low mortality from motor vehicle accidents, and injury and poisoning, but a high number of deaths from cancers of the stomach, pancreas, bladder, and the large bowel. Cooks, waiters, bartenders and related woikers experienced an adverse mortality from diseases of the circulatory and respiratory system, cancer of the lip, and cirrhosis of the liver. Agricultural, Animal Husbandry and Forestry Workers, Fishermen, Hunters The mortality from malignant neoplasms and ischaemic heart disease was low among farmers, farm managers and supervisors, agricultural, and animal husbandry workers. Farmers experienced a low risk of death from diseases of the respiratory and digestive systems, cerebrovascular disease, pneumonia and influenza, cirrhosis of the liver, and cancers of the lip and lung. This occupational group also had a high risk for cancer of the lymphatic system. Deaths from motor vehicle accidents, suicide, injury and poisoning were above the national average for fishermen, hunters and related workers. There was a high mortality among forestry workers from homicide, malignant neoplasms, respiratory and circulatory system diseases, motor vehicle accidents, and injury and poisoning. Production and Related Workers, Transport Equipment Operators and Labourers Plumbers, welders, sheet metal and structural metal preparers and erectors, spinners, weavers, knitters, dyers and related workers had a high number of deaths from malignant neoplasms. Cancer mortality was low among stationary engine and related equipment operators. Deaths from circulatory system diseases were low for metal processers, electrical fitters and related electrical and electronics workers. However, food and beverage processers experienced a high risk from this cause of death, as well as from cancers of the oesophagus and lung, and non- Ischaemic/non-hypertensive heart disease. The mortality from respiratory system diseases among blacksmiths, toolmakers, machine tool operators and transport equipment operators exceeded the national average. In contrast, production supervisors and general foremen, electrical fitters and related electrical and electronics workers had a favourable mortality from this cause of death. The number of deaths from injury and poisoning were high for transport equipment operators, miners, quarrymen, well drillers and related workers, but low for production supervisors and general foremen, electrical fitters and related electrical and electronics workers. Occupations with a high lung cancer mortality included bricklayers, carpenters and other construction workers, painters, transport equipment operators, miners, quarrymen, and well drillers and related workers. However, there were few deaths from cancer of the lung and motor vehicle accidents among electrical fitters and related electrical and electronics workers.
11 1 The mortality from ischaemic heart disease was low for stationary engine and related equipment operators, but high for machinery fitters, machine assemblers and precision-instrument makers (except electrical). Transport and equipment operators had an adverse mortality from ischaemic heart disease, digestive system diseases, motor vehicle accidents, cerebrovascular disease, and asthma. Bricklayers, carpenters and other construction workers experienced a high number of deaths from stomach cancer and diseases of the arteries. The death rate from cancer of the kidney was high among painters.
