3 cancer committee Chairman s letter Dear Friend of the St. Mary Mercy Cancer program, This 2012 annual report of oncology services offers you an overview of our program and highlights exciting developments taking place in our care of cancer patients. This has been a busy and productive year for the cancer program. The St. Mary Mercy Livonia cancer program proudly maintains accreditation by the American College of Surgeons Commission on Cancer (CoC) with a three-year commendation status as a Community Hospital Cancer Program. This certifi cation demonstrates the dedication and commitment of St. Mary Mercy Hospital and our team of cancer specialists to provide the highest level of cancer care, right here in our own community. The CoC has substantially revised its accreditation guidelines to ensure that cancer programs are providing a higher level of patient-centered services and measuring the quality of the care delivered. As chair of the Cancer Committee, I am pleased to report our exciting progress highlights! These patient-centered services have been enhanced or newly implemented to improve the patient experience with care, quality of life and treatment outcomes: Registered Dietician available for outpatients to provide nutritional consultation and advice during cancer treatment Social Work services now available for patients through their Medical Oncology office and Radiation Oncology Genetic counseling and testing services available to patients and their families who are considered at high risk for inherited cancer syndromes. Nurse Navigator to assist cancer patients during the entire spectrum of cancer care. Survivorship care planning services for patients who have completed their cancer treatments. Patient Financial Navigation Services Spiritual Care Services High-risk breast disease coordinator Center will also work closely with the Cancer Center, ensuring seamless access to breast cancer treatment services offered by our oncology team. MammoPlus is a new feature available to all patients who obtain mammograms at the Marian Women s Center at St. Mary Mercy. MammoPlus helps to identify patients at high risk for developing breast cancer. RiskPlus is designed to individually counsel patients found to be at high risk and enact a plan to manage their risk with help of a high risk breast disease coordinator. Quality improvement specialists are assisting health care providers to enhance the quality of cancer care provided at St. Mary Mercy. In the past year, we have focused on: Improving timeliness of chemotherapy infusions Implementing a process to improve pain control Expediting results reporting for clinical trial patients Assuring compliance with NCCN guidelines in the management and treatment of resected non-small cell lung cancer (see page 7). The St. Mary Mercy physicians, staff and administration are proud to offer remarkable cancer care and treatment. I look forward to further implementation of our patient centered services and quality care improvements in the new year as we aim to fulfill our mission: To be a truly great hospital, providing comprehensive, coordinated and compassionate care, every time to everyone. The Comprehensive Breast Center located within the Marian Women's Center at St. Mary Mercy, will be dedicated to breast diagnosis and management. 4 Community Outreach Programs for Cancer Services July 2011 June 2012 The St. Mary Mercy Hospital Community Outreach department serves to design and implement cancer-related education and screening programs and supportive services for cancer patients and their families. As a part of the Community Benefi t Ministry, such programs include those that may be low-cost or no-cost and for the poor, underserved and broader community. The Men s Health Day program provides the opportunity for men ages to participate in free blood tests for Prostate Specifi c Antigen, Total Cholesterol and HDL, blood pressure and stroke screening and a physical exam for Prostate disease. Screening is also available for skin cancer. Education information is available for heart health, colorectal cancer, physical activity and nutrition. This program brings in over 225 men, some without health insurance and some without a medical home. During the month of October, Breast Cancer Awareness month, a variety of women s health education and screening was offered, including mammograms, for those women without insurance coverage. Transportation to and from the hospital for infusion or radiation services can be a challenge. Recognizing this, St. Mary Mercy provides shuttle service, cab vouchers or gas cards for those patients in need of the necessary transportation. A team honoring a loved one gathers at the 2012 Embrace Life 5K run/walk for Cancer. Supportive services for patients and their families are provided through the Breast Cancer and Prostate Cancer (Us Too) support groups. The monthly sessions provide emotional, social and educational support along with providing important resources that may be needed. In support of furthering education and research in cancer, St. Mary Mercy participates in the annual American Cancer Society (ACS) Relay for Life events in the local area. Being a sponsor and providing a team of employees, patients and families is a way to support the fi nancial resources of ACS while enhancing the education and support resources available. An annual fund-raising event, Embrace Life 5K run/walk for Cancer brought over 800 people in a celebration of life for cancer survivors and those with cancer. Funds raised are used to help those in need of fi nancial assistance for screenings, transportation and also for products and services available through the Helen Palmer Image Recovery Center. 7 AJCC Stage Comparison The distribution of stages at time of initial lung cancer diagnosis shows a higher percentage of Stage IV patients compared to other community hospitals across the country. 8 Comparison Survival Graph The fi ve-year survival rates for lung cancer patients diagnosed at SMML from are comparable to the national average, with the exception of stage III patients. 9 Site specific study - Lung Cancer The cancer specialists at St. Mary Mercy work as a multi-disciplinary team to offer patients comprehensive and compassionate care. Our patients entrust us to make the appropriate decisions and to provide the best possible care. As health care continues to evolve, reporting outcomes and comparing our results with national benchmarks is becoming increasingly important. At St. Mary Mercy, we strive to ensure compliance with well established, evidence-based national treatment guidelines. Harmesh Naik, MD, medical oncologist, conducted a retrospective review of patients who underwent lung cancer surgery in The purpose was to study whether: diagnostic evaluation was adequate cancer stage was properly determined smoking cessation advice was offered prognostic factors were identified postoperative treatment plan using chemotherapy and radiation followed National Comprehensive Cancer Network (NCCN) guidelines I am pleased to share the findings of his report (Figure 6) showing our high rates of compliance with national guidelines regarding appropriate testing, treatment and supportive care of our cancer patients. It also identifies opportunities for us to help individualize treatments plans for each patient. At St. Mary Mercy, patients can be assured that they receive the proper testing, diagnosis and treatment of cancer. 10 Clinical Trials at St. Mary Mercy Livonia Rose Ermete, RN, BSN, OCN, CCRP Research Nurse Coordinator St. Mary Mercy Livonia (SMML) has been participating in cancer research studies for the last fi ve years (Figure 7). The oncologists at SMML are approved, National Cancer Institute (NCI) Community Investigators. This designation, and our affi liation with the Michigan Cancer Research Consortium, allows SMML to offer NCI Cooperative group and industry research studies to our patients. SMML provides our patients the opportunity to participate in treatment, diagnostic, prevention and symptom management research studies, (clinical trials). Participation in a clinical trial provides patients access to promising treatments that are not yet available to the general public. In the past, these clinical trials could only be accessed at large research institutions. Now, patients can participate in these same studies close to their home. Cancer treatment in the 21st century is moving towards individualized medicine. This term means that clinicians are using information from clinical trials, which allows them to take into account traits exhibited by an individual s tumor. The science behind this type of treatment allows for increased drug effi cacy, decreased toxicity and avoidance of ineffective treatment. Participants in clinical trials are leading the way in this new arena. To date, 154 patients have participated in clinical trials at SMML. In 2012, there were 37 patients who took part in treatment, diagnostic and symptom management studies. Some of these patients have participated in more than one study. We are grateful to these patients who are taking an active role in fi ghting cancer, not only for themselves, but also for all cancer patients. Because of these patients, cancer survival rates continue to improve, thus providing an important focus in future research. Clinical Trials Participation Participants Figure 7 8
