3 HMH Cancer Program Hardin Memorial Health is pleased to present its 2016 Cancer Program Annual Report. This report outlines the activities of the Cancer Program and reviews of newly diagnosed analytic cancer cases for Analytic cases are those that were either diagnosed and/or treated at our facility. The purpose of Hardin Memorial Health s multidisciplinary cancer program is to decrease cancer morbidity and mortality and improve the quality of care of patients diagnosed with cancer. 4 Comments from Karen Johnson, M.D. Cancer Committee Chair At Hardin Memorial Health, cancer care is a top priority. We utilize advanced technology and have access to the most current treatment regimens available. Hardin Memorial Health s cancer program is a Commission on Cancer accredited program which is a special designation for centers meeting strict criteria for cancer care. In addition, we are accredited by the American College of Radiology as a Breast Imaging Center of Excellence. Hardin Memorial Health s cancer program has a long history of excellence with tumor conferences allowing physicians from multiple specialties an opportunity to discuss complex cancer patients with one another and coordinate care. We also are a part of the University of Kentucky s Markey Cancer Center s clinical trial program which allows us to offer clinical trials to our cancer patients and keeps patients close to home while receiving comprehensive care. Our physicians are committed in the fight against cancer including utilization of cutting edge diagnostic technology and treatment. We are proud to be an active affiliate of the University of Kentucky Markey Cancer Center, which is the only NCI designated cancer center in the Commonwealth of Kentucky. 9 Clinical Trials Cancer treatment clinical trials are studies involving research to find more durable and successful ways to treat cancer. It is important that people of all ages, race, ethnicity, and geographical background take part in research so new information will help guide future treatment of cancer patients. The National Comprehensive Cancer Network (NCCN) Believes the best management for any cancer patient is in a clinical trial. HMH oncologists follow NCCN guidelines in the treatment of their cancer patients. HMH started opening clinical trials in Our oncologists participate in clinical studies for adults 18 and over that include breast, lung, head and neck, among others. An important benefit, HMH offers comprehensive cancer care close to home. Cancer treatment clinical trials are studies involving research to find more durable and successful ways to treat cancer. HMH s Cancer Center has accreditation from the American College of Surgeons Commission on Cancer, and is affiliated with Baptist Health Kentucky, University of Kentucky, and Kentucky Clinical Trial Network (KCTN). Trials are offered from the National Cancer Institute (NCI), National Clinical Trial Support Unit (CTSU), and investigator initiated trials from University of Kentucky. 10 HMH offers numerous trials that fit our patient population. Of note is one of our current trials, Optimization of Smoking Cessation Strategies Concurrent With Treatment of Tobacco Related Malignancies (MOST). This Kentucky-based trial focuses on tobacco related cancer for smokers wanting to stop smoking. This study analyzes 12 different modalities used to stop smoking using nicotine replacement therapy (NRT), or current FDA approved medications designed for that purpose, along with either high or low intensity counseling. HMH has consistently remained the top enroller for this trial for 24 months among nine other sites in the state. HMH looks forward to opening an additional trial, the Kentucky Lung Education Awareness Detection and Survivorship (LEADS) Trial, in collaboration with University of Kentucky, University of Louisville, Kentucky Cancer Consortium, and Lung Cancer Alliance and funded by a grant from Bristol-Myers Squibb Foundation. This trial is looking to promote provider education, survivorship care, prevention and early detection regarding lung cancer. We look forward to increasing our clinical trial portfolio with the exciting future and promise of clinical trials, and in the improved survival and quality of life of our patients. 11 Community Outreach Standard 4.1 Prevention Programs Social Media: Though various venues of social media our regional community can learn more about cancer prevention, education and various cancer support group opportunities. Additionally, Hardin Memorial Health hosts Health in the Heartland a monthly informational educational program airing on the local television cable channel, our organizational Internet, and YouTube focused on cancer topics from breast, skin, colorectal and prostate are featured with our healthcare providers discussing prevention, services and current treatment options. The Incredible Colon: March 12, 2016 Partnering with the Kentucky Cancer Program, HMH offered the community the opportunity to explore the Incredible Colon a 20 ft. inflatable, interactive medical model to learn about colorectal cancer and the importance of early detection screenings. Tiffany Richardson APRN, with HMH Digestive Disease Center, was present to engage participants in a discussion regarding risk factors, prevention tips and screening guidelines for colorectal cancer. Fifty-nine community members visited the display in the Towne Mall center court and completed a colon cancer screening survey. Hardin Memorial Health Cancer Care Center 2016 Annual Report Page: 11

12 Purses, Pumps and Prevention: A Cancer Awareness Social April 14, 2016 Recognizing April is National Cancer Control Month, Hardin Memorial Health held a community cancer prevention awareness program featuring a fashion show, dinner and educational program for participants to learn about breast, colorectal, cervical, skin and lung cancer prevention to the 178 attendees. Cancer prevention educational handouts were provided in addition to local support and resources. Collaborating partners: the American Cancer Society. A pre and-post cancer survey was administered to participants to encourage adult individuals to recognize cancer prevention screening recommended guidelines and seek appropriate medical assistance. 13 Standard 4.2 Prevention Programs Screening Program: Skin Cancer Screening May 2, 2016 In recognition of National Skin Cancer Detection and Prevention Month, a free skin cancer screening was provided: 57 community members screened following the American Academy of Dermatology (AAD) screening guidelines with the assistance of 3 local dermatologists. Educational materials from the American Academy of Dermatology and National Cancer Institute were distributed to promote skin cancer prevention. Demographics below: Average Age: refrred for follow-up (10 male and 10 female) 9 biopsies recommended Participants completed a screening evaluation noting: 86% of the participants stated they regularly use sunscreen, 56% perform monthly self-preventive skin check examinations and 40% indicated they will start checking their skin monthly for changes. Follow up phone calls were conducted by a registered nurse to the 20 referring participants reaching 10 clients. Skin Cancer Prevention Education Two worksite sun safety programs were conducted for 30 participants of a local water district. During May- National Skin Cancer Awareness Month and July- UV Safety Month skin cancer prevention educational awareness materials were distributed to 33 healthcare providers and service locations throughout our healthcare system for staff, patients, families and visitors. 14 Community Health Initiatives: Cancer Prevention and Screening Awareness WOW Wellness on Wheels SM mobile health unit provides regional monthly preventive health and wellness screenings in Hardin, Meade, LaRue, Nelson and Grayson counties. From January November, 486 community clients visited the WOW, and 284 clients voluntarily shared they are up to date regarding preventive cancer screening for breast, colon, skin and lung. Noting, 58.4% of the clients are following the American Cancer Society recommended guidelines for cancer preventive screenings for breast, colon, skin and lung. Demographics:? 15 CANCER PREVENTION % SCREENED COUNSELED INFORMATION FOLLOW UP INITIATIVES Regarding Preventive Screenings BREAST HEALTH 96% COLON HEALTH 84% SKIN HEALTH 93% LUNG HEALTH 49% Demographics: Male: 122 Female: 162 African Amer.:16 Hispanic: 0 Other: 268 Tobacco/Smoking Cessation Hardin Memorial Health offers a variety of assistance to help individuals become tobacco free: Cooper/Clayton Method of Stop Smoking, and Live Smoke Free! smoking cessation support group. However, no courses were implemented due to lack of registration. Tobacco cessation resource packets were mailed in conjunction with telephonic counseling for 73 community participants who inquired about assistance along with referrals to the Quit Line. Wellness coaching through a Well@Work worksite wellness provided assistance to a local employer working with employees to earn health insurance incentives for becoming tobacco free. Ninety clients indicated they were tobacco users. Clients were referred to the QUIT NOW Kentucky tobacco cessation assistance phone line. Eight employees completed the Quit Line earning a certificate for telephonic tobacco cessation. Of the 8 employees, 3 employees became tobacco free and the 5 other clients indicated a significant decrease in tobacco usage continuing on the path to becoming tobacco free.
