FIRST STAGE: It is characterised by a hypodermis which is swollen due to water retention induced by the permeability of blood vessel walls. The adipocytes become full of lipids and grow bigger. They form fatty nodules which disturb the microcirculation, which accentuates the oedema: appearance of orange peel skin visible only under pressure between two fingers. Then there is aggravation with stages II, III and IV. STAGE II : The fatty nodules swell. The collagen fibres cross each other and become welded together. Fibronectin acts an adipocyte support with their integrins attached to the cell s internal skeleton, enabling the cell to preserve its expanded form. It has been shown that: - Preadipocytes only form adipocytes with difficulty when they are cultured with fibronectin. - The fibronectin produced by the TGF-β, (Transforming Growth Factor), a powerful growth factor, inhibits adipogenesis and maintains the expanded form of the cells.
