Diet (B12 in meat & dairy products): Vegans/vegetarians Malabsorption: Terminal Ileum: IBD (Crohn s disease). Metformin. Prolonged treatment with PPIs. Gastrectomy; bariatric & gastric bypass surgery. Low levels can be seen with: Pregnancy COCP
