4 North Waziristan Agency (NWA) Displacement Sector wise UNICEF Response Updates from 21 st to 28 th July, 2014 Sector Response Wash UNICEF, with the support of local partner has provided 2,376,000 liters of safe drinking water through water trucking to the areas with high concentration of IDPs. WASH implementing partner (IP) SABAWOON installed six hand pumps and nine ventilated improved pit latrines in the host communities of district Bannu, benefitting over 6,000 individuals including IDPs and hosting population. WASH facilities in 136 schools benefitting 10,328 IDPs have been repaired/ rehabilitated so far. In addition, eight waste bins have been installed in eight schools for collection of solid waste and 9,303 individuals residing in schools have been reached with critical sanitation-related information. WASH cluster has provided 5,301,000 liters of clean drinking water, distributed 10,909 bath soaps among 2,775 IDP families and conducted hygiene promotion sessions for 16,239 IDPs residing in schools and host community. 9,000 hygiene kits have also been distributed by OXFAM, NRC and UNICEF. Water quality testing of the water filling points is regularly conducted by WHO and Disease Early Warning System (DEWS) report shared with cluster members to be ready for response to any outbreak in the area. Health EPI services initiated in 20 UC s of district Bannu, with UNICEF support, vaccinated 825 under 2 years children with BCG, 1,287 children with Penta 1 and PCV 10, 59 children with Penta 2, 14 children with Penta 3,1428 children with measles 1 and 143 with measles 2 vaccine. Total 405 women including 34 pregnant women were vaccinated with TT 1 and TT2 respectively. Jointly WHO and UNICEF are supporting the local administration to vaccinate 100 per cent of the displaced population irrespective of their age against polio. 234,885 displaced people including 97,647 children (0-5 years), 62,041 children (5-10 years) and 75,297 children above 10 years of age have been vaccinated for polio at permanent transit points. A mobile health clinic Nonihaal service launched in district Lakki Marwat on 24 July 2014 to facilitate health services in the community as well as EPI services. UNICEF Polio communication team facilitated local radio channel BBC Urdu in KP to air radio programmes on polio vaccination of NWA IDPs. Information is being provided regarding various points set up for vaccination and strong messages to local communities for vaccination of their children to stop virus transmission. Child Protection The two Help Desks in Bannu, set up by Child Protection and Welfare Commission (CPWC) with UNICEF support have so far identified and registered 2,368 vulnerable cases (897 girls) majorly including 1,660 orphan children (673 girls), 23 child labourers (4 girls), 87 separated children (35 girls), 27 out of school children (8 girls), 82 children with disabilities (19 girls) and 208 children with health issues (91 girls). So far needs/issues of 1,133 vulnerable children cases (536 girls) of total 2,193 referred to relevant service providers have been addressed successfully. A female psychologist deployed by CPWC at one help desk in Bannu district has provided 40 children (22 girls) with psychosocial support and counselling services. GCC PDMA, supported by UNICEF, in collaboration with Social Welfare Office in district Lakki Marwat and local NGO identified 60 persons with special needs including adults and children. The identification and registration of vulnerable cases is in process. Nutrition UNICEF in collaboration with WHO and WFP supported nutrition services in 20 health facilities of district Bannu have screened 223 children of 6-59 month age (boys: 106; girls: 117) and 100 pregnant and lactating women (PLW) for assessment of acute malnutrition. In addition 182 mothers and care takers were provided with messages on infant and young child feeding, maternal nutrition and importance of hygiene. Education A contingency partnership agreement with UNICEF partner BEFARE has been activated for three months from 24 July to 23 October 2014 to provide education services to 9,000 children (4,500 girls) in district Bannu. 5 International Committee of the Red Cross (ICRC) Assistance Updates as of As a result of a military operation in North Waziristan, in the Federally Administered Tribal Areas, hundreds of thousands of people have been displaced to various districts in neighboring Khyber Pakhtunkhwa province. From the outset, the Pakistan Red Crescent has delivered emergency aid, medical services, food, water and sanitation. On 25 July, the number of internally displaced people registered by the Pakistani authorities stood at almost one million. However, the verification process still under way is expected to significantly bring this number down. With the increased number of displaced people, the Pakistan Red Crescent has recently doubled its caseload to 8,000 families (around 100,000 people in all). The ICRC allocated one million Swiss francs to support the Pakistan Red Crescent for the first three months in this relief operation, which is being carried out in close coordination with the local authorities and the other humanitarian organizations providing aid for displaced people. Between 19 June and 28 July, with ICRC support, the Pakistan Red Crescent provided: Health services Deployed six mobile health units that have already treated over 25,000 men, women and children. Two of the units are permanently deployed in Milad Park, in Bannu, while the other four move to different union councils according to need; Administered first aid to approximately 660 people at the beginning of the operation; Formed six two-person (one male and one female) volunteer teams to provide psycho-social support. The teams will be operational in August. Water and sanitation Installed a water filtration plant at Milad Park to provide 32,000 litres per day of clean drinking water; Installed 59 pit latrines and repaired 14 latrines; installed 17 water tanks. Distribution of food and other items Provided cooked food for 23,000 people in the initial week of displacement; Distributed 700 food rations donated by the Punjab branch of the Pakistan Red Crescent; Provided an Iftar dinner for 1,000 displaced people; Distributed 500 food packs donated by the Turkish Red Crescent Society; Signed an agreement with the World Food Programme to distribute food packages to 8,000 families in August; Set up three distribution points in two union councils of Bannu to facilitate delivery of essential aid; distributed essential supplies to 8,000 families. Restoring contact between family members: formed a team to assess the needs of family members dispersed by displacement. The team will begin its activities in August. 6 Provided one million Swiss francs in funding; Provided technical support to help draw up a comprehensive plan of action; Donated two kits to facilitate distributions; Sent several technical staff to join Pakistan Red Crescent personnel in the field; Made available and installed an 80 kilovolt-ampere generator for the Pakistan Red Crescent operational hub in Bannu; Sent a photographer to the field in agreement with the Pakistan Red Crescent Society to en hance the society's visibility. The ICRC will continue to support the Pakistan Red Crescent and monitor the needs of displaced people. It will base its further assistance on settlement patterns of the displaced and on the needs of those who might choose to return to their home areas. Source: ICRC Updates/ published on Médecins Sans Frontières (MSF) Response Updates as of Tens of thousands of people fleeing major military operations in the Pakistani region of North Waziristan have crossed the border into Afghanistan over the past months, seeking refuge in Khost, Paktia, and Paktika provinces. In response, teams from Doctors without Borders/Médecins Sans Frontières (MSF) are providing medical and humanitarian aid in Gulan refugee camp, Khost province. Most of the refugees in Khost province are being hosted by the local community, but an estimated 1,770 families have settled in Gulan refugee camp, a vast stretch of land some 12 miles (19 kilometers) from the Pakistani border. Since July 5, MSF teams have been running a clinic set up just outside the camp and have been working to improve poor sanitation within the camp. North Waziristan, from where the refugees fled, has very low vaccination coverage, so MSF teams have prioritized vaccinating children between six months and 15 years old against measles, a deadly disease that can spread rapidly within camps. So far, MSF teams have vaccinated 2,965 children an estimated 87 percent of all children in the camp against measles, at vaccination points set up in the camp. They will continue to provide vaccinations in the clinic for new arrivals and for children who have slipped through the net. 7 Food Security Cluster (FSC) Assistance for NWA Displacement as of WFP distributions are currently ongoing at nine sites. Ever since WFP began distributions on June 22, a total of 54,910 households i.e. over 686,375 individuals have received assistance and a total of 4,995 MTs food has been distributed by Jul 24, WFP continues to implement through cooperating partners, PRCS, BEST and CERD, who operate in Bannu, Lakki Marwat, Karak and FR Bannu; and LHO which operates in D.I. Khan and Tank. The FAO has provided 100,000 doses of Peste des Petits ruminants (PPR) vaccines to Directorate General Livestock and Dairy Development (L&DD), sufficient for 100,000 small animals. PPR is an acute contagious disease caused by a Morbillivirus which affects mainly sheep and goats. The immunity induced by the vaccines lasts for lifelong and this will greatly help to protect the small animals of IDPs. The FSC cluster s fortnightly meetings are routinely held in Peshawar and Islamabad. For better coordination, Asst Cluster Coordinator is being deployed in Bannu/DIK. The gov t has requested WFP to help it in distribution of food the gov t keep on receiving from charities, companies and philanthropists. To undertake this initiative, WFP will set up a Food Bank. WFP has shared about this initiative in the last FSC cluster members in Islamabad and KPK and invited them to become a partner in this. Led by OCHA & PDMA, the FSC has supported a multi-cluster initial rapid assessment is in the final stages of collecting data from IDPs areas. Its results will be available by shortly, which will give the cluster a better pictures vis-à-vis livelihood impacts and needs of IDPs to inform the humanitarian response. 10 Nationwide Updates Polio Updates Current Epidemiology of Polio in Pakistan As of 25th July 2014, 102 type-1 wild poliovirus (WPV) cases have been reported from 15 districts/ towns/ tribal agencies/ FR areas (compared to 24 in 2013 from 12 districts/ towns/ tribal agencies/ FR areas). Moreover, WPV-1 has also been isolated from 54 envi-ronmental samples in 2014 (out of 172 samples for which results are available) from Karachi (Gaddap, Baldia and Gulshan-e-Iqbal towns), Hyderabad and Sukkur in Sindh, Peshawar in Khyber Pakhtunkhwa, Lahore and Rawalpindi in Punjab and Quetta in Balochistan. Nineteen sabin-2 like poliovirus cases have been reported so far in 2014 including 17 from FATA (nine from North Waziristan agency and eight from FR Bannu) and two from Khyber Pakhtunkhwa (one each from Bannu and Lakki Marwat). Following are the key epidemiological characteristics of polio cas-es reported in 2014 so far: 85 (83.3%) polio cases are younger than two years (median age: 16.5 months). Based on recall of parents, 80 (78.4%) cases did not re-ceive any OPV dose (through routine vaccination and/or campaigns). Province Polio Situation Khyber Pakhtunkhwa/FATgether from Khyber Pakhtunkhwa 16.6% (17/102) and FATA 73.5% (75/102) indicating that It is worth mentioning that, 90.2% (92/102) polio cases so far in 2014 have been reported to- these regions remain the key polio reservoirs in the country. Sindh Situation in Karachi has also been very concerning since all the three high risk towns (Gadap, Gulshan-e-Iqbal and Baldia) had the continuous presence of wild poliovirus in environmental samples collected from Jan to May Moreover, eight out of nine wild polio cases from Sindh province this year has been reported from Karachi. This situation in Karachi correlates with deteriorating quality of supplementary immunization activities (SIAs) in 2013 and in the first half of Recently, the wild polio case reported from Sanghar district in Sindh is also genet-ically linked to Karachi. Hyderabad district in Sindh also detect-ed WPV-1 in the last three consecutive environmental samples since April Genetic analysis of the isolated virus also indicates its linkage with the virus found in the environmental samples from Karachi. Sukkur district in Sindh has reported WPV-1 in two of last four environmental samples including an Orphan virus which in turn indicate the substantial immunity and surveillance gaps in Northern Sindh. Punjab No polio case has been detected so far in 2014 from Punjab. However, detection of WPV-1 in nine out of twelve environmen-tal samples collected since April this year from four sites of Lahore indicates a very concerning situation. Moreover, WPV-1 has also been detected in the environmental sample collected on 21st May from Rawalpindi. The initial four environmental samples collected from the capital territory of Islamabad since 21st March turned negative for wild poliovirus. No positive envi-ronmental sample has been reported from Multan this year while environmental samples from Faisalabad have been nega-tive for over 18 months now. Balochistan Balochistan province did report a wild poliovirus case after the interval of 23 months from district Killa Abdullah. Furthermore, detection of WPV-1 in the last three of four environmental sam-ples collected from the two sites in Quetta already turned the situation to utmost concern in Quetta Block. The genetic analy-sis of the isolated virus indicates missed viral circulation for more than a year; which in turn indicates missed populations by the Supplementary Immunization Activities (SIAs) and AFP surveillance. The ongoing sub-optimal quality of SIAs in Quetta Block complements the findings of the genetic analysis. An impending polio outbreak cannot be ruled out in the region.
