1 West Nile Virus Surveillance and Management in Québec: a successful approach (so far ) Pierre Gosselin MD MPH Institut national de santé publique du Québec Objectives of Surveillance Main objective is to prevent complications and human deaths linked to WNV Ensure early detection of presence of WNV in a geographic area Identify areas of transmission to humans for preventive action and monitoring Qualify the level of transmission to humans Hopefully, support following year s planning Components of the Surveillance System started in Implementation and continuous update of an extranet site dedicated to: online entry, data warehousing, document exchange, scientific news, and also tables or graphical representations of all relevant data from surveillance program 2. Implementation of an online geocoding tool ensuring the validation and geographic location of relevant events (reported birds and others) 3. Implementation of an online mapping tool for all data relevant to WNV in real time 4. Creation of a group of experts to interpret all results and surveillance data Components of the Integrated Surveillance System Partners included MSSS, INSPQ, regional health authorities, MAPAQ, MENV, SOPFIM, Labs, GDG Environment Inc. Group of experts included INSPQ, academics, GDG entomologist, MSSS. Met once a week. 1
2 Components of the Integrated Surveillance System Criteria for action were developed by the expert group over the summer, and implemented/modified in real-life context in 2003 and Later adapted to low-level activity of WNV in late 2006: no more active surveillance. System described in International Journal of Health Geographics (Gosselin et al., 2005) at: Components of WNV Surveillance Human: Information on Human Cases. Animal: Avian (crows) (sightings and analysis for WNV), Horses Mosquitoes: Zones of potential transmission to humans Intervention spraying of larvicides: Zones treated (preventively or for monitoring purposes) Locations of Human WNV cases in 2003 Human cases Probable Confirmed Insecticide spreading Treated Zone Untreated Zone Regions Municipalities 2
3 Some functions: automatic risk analysis for the expert group Result Visualization: number of positive crows by RSS, CLSC, municipalities in 2003 After 3 years of operation, evidence of decreasing threat to public health (fall 2006) Analysis of climate role in 2002 outbreak in Northeastern America Comparison of winter and summer of 2002 climate statistics for Montreal, Toronto, Detroit, Chicago, Cleveland and New York where the outbreaks had been severe, over last 60 years Conclusion: a rather rare event (about once in 40 years) 3
4 Published in IJHG El Adlouni et al. (2007) at Used as input for Health Ministry to suspend larviciding and active surveillance Investment in WNV Geosimulation instead, in order to improve predictions of critical situations Geosimulation project with our friends Bouden and Moulin, in close cooperation with INSPQ, GDG Environment, others. Published recently (Bouden et al., 2008) at: Next step is to validate this geosimulation tool in other contexts and, eventually, use it confidently for public health decisions. Lyme disease is ournexttargetwithn.ogden Done within the framework of the Quebec Action Plan on Climate Change (Health component) where improving surveillance tools is a major objective. 4
