1 How to Stay Healthy with IBD KAREN PERLS PA-C Introduction! Vaccinations play an important role in preventing illness! Living with IBD may increase your risk of infection! Very important to be sure you re up to date with vaccinations to reduce your risk of preventable infections How the immune system fights infection! When bacteria and viruses enter body and multiply, they can cause an infection.! Your immune system fights the infection by sending white blood cells to attack the bacteria and virus directly.! The immune system forms antibodies to the specific bacteria/ virus.! After the infection subsides, your body is left with a supply of antibodies that help recognize and fight that disease if exposed again in the future. 1&
2 How do vaccinations work?! Vaccines help you develop immunity to a disease by imitating an infection, without causing illness.! It prompts your immune system to develop antibodies, so in the future you will be able to recognize the bacteria or virus and be protected. Does IBD increase my susceptibility to infections?! IBD can increase your risk to infection, compared to the general population! Your increased risk may be related to:! the underlying disease! your medication! surgery or malnutrition! older age or concurrent illness Can vaccines make my IBD worse?! The answer is yes and no it depends on the type of vaccine.! Vaccines can be inactivated(killed) or live.! If you have IBD, its safe to have inactivated vaccines regardless of your medication type.! No evidence that receiving vaccinations will make IBD worse! If you are taking immunosuppressants such as Prednisone, Azathioprine, Methotrexate, Remicade, Humira, Cimzia you should NOT receive live vaccines. 2&
3 Types of Vaccines Live Vaccines! Measles, Mumps, Rubella(MMR)! Herpes Zoster(Shingles)! Varicella(Chickenpox)! Influenza(inhaled form) Inactivated(Killed) Vaccines! Td/TdaP(Tetanus)! Human Papillomavirus(HPV)! Influenza(Injectable form)! Pneumococcal(pneumonia)! Hepatitis A! Hepatitis B! Meningococcal(meningitis) for adults? for adults? NOT Inhaled Don t Take These Vaccines if on Immunosuppressants 3&
4 What if I have been diagnosed with IBD and am not up to date on my vaccines?! Its best to complete vaccinations soon after initial diagnosis! Live vaccines 2 months before starting immunosuppressive therapy! Or, at least 2 months after stopping therapy are safe! Immunosuppressive therapy may also decrease your response to a vaccine! For example patients using combinations of Azothioprine and Remicade may have a decreased response to influenza and pneumococcal vaccines. What other preventative medical tests can I do to stay healthy with IBD? Certain cancer incidences can be higher with IBD Colon cancer get routine colonoscopies as recommended by your gastroenterologist. Skin cancer - use sun protection and get annual overall body skin checks by dermatologist Cervical cancer - get screened annually and receive Human Papilloma Vaccine(HPV) vaccine in males and females ages Other important tests to keep on top of Baseline Bone Density scan Routine laboratory tests while on immunosuppressants Annual Eye exam Conclusion! Vaccinations play an important role in preventing illness! If you are diagnosed with IBD! Confirm with your primary care provider that you are current with recommended immunizations! Have your primary care provider communicate with your gastroenterologist to determine which vaccines you need! Consider getting any necessary live vaccines when you are initially diagnosed with IBD since you may require immunosuppressive therapy in the future! Don t get live vaccines while on immunosuppressive medications! Don t forget to continue with preventative medical tests 4&
