4 Introduction The Government of Assam (GoA) is committed to reducing child mortality and morbidity in the state by improving full immunization coverage through universal immunization programme (UIP). Assam has a birth cohort of 6.68 lakhs children annually. The UIP programme provides vaccination against nine life-threatening diseases (diphtheria, whooping cough, tetanus, polio, tuberculosis, measles, Haemophilus influenza, Japanese Encephalitis and hepatitis B). Despite this programme being operational for the past more than 30 years, only 47% children in Assam (Source: NFHS-4) receive all vaccines during their first year of life. It is estimated that annually, more than 3.54 lakh children in the state do not receive all vaccines that are available under the UIP. As a strategic endeavour, GoA is launching Special Immunization Drive in September-December 2016 to achieve more than 90% full immunization coverage in the state. This initiative will eventually close immunity gaps and strengthen immunization coverage. A total of three/four rounds will be conducted under the drive as per GoI guidelines. Each round will be conducted for 5 working days (other than Sundays, Holidays and regular RI days). Target beneficiaries for this drive would include all children under two years of age and pregnant women. However, children above two years of age seeking vaccination will not be denied due vaccines. This special drive will be conducted in all the districts of the State and will focus intensively in vacant subcenters area, sub centers who have not conducted RI in the last three months and poor performing SCs (SCs with high drop out and left out children) through additional sessions. However, left out and drop out children in remaining areas would be covered through updated microplans and community mobilization activities under routine immunization sessions. The entire drive would be supported through intensified monitoring and supportive supervision through medical college faculties and development partners. 3

5 Supportive Supervision Supportive supervision (SS) has been envisaged as an integral component of the Special Immunization Drive. It involves periodic visits to all the districts at different level including district, block, sector, cold chain point, sub-centre and village level, by a team comprising of representatives of government health department and development partners. The objective of supportive supervision is to identify gaps in real time and to address them locally, at sub-district, district or state level. It implies a regular and dependable interaction with health functionaries in the field to help identify and solve problems, improve preparedness, planning and implementation of the Special Immunization Drive. Key functions of Supportive Supervision includes: - Supportive Supervision (SS) visits to all districts before, during and after the campaign to monitor preparedness, planning and implementation of Special Immunization Drive - Assess the progress of preparedness, planning and implementation of Special Immunization Drive - Feedback to all stakeholders regarding progress and solve problems and improvement in quality implementation. - Ensure timely support to the districts from the state to implement the most critical interventions. - Facilitate sharing with the state and cross learning within districts 4

6 Supportive Supervision Checklists There are two sets of checklists: For Preparedness and Planning phase For Implementation phase- During Campaign Preparedness and planning phase checklists There are four checklists for Preparedness and Planning phase: District level preparedness checklist Block level preparedness checklist Sector/ Cold chain point level preparedness checklist Sub centre/ Village level preparedness checklist The objective of conducting Supportive Supervision before the campaign phase is to assess the preparedness and planning of Special Immunization Drive in the districts at various levels. These checklist focuses on progress of planning meetings, district/block/sector level orientations of health officials/ ANMs/ ASHAs/ mobilizers, preparations of linelists/ due lists, identification of left out areas/ left out beneficiaries / drop out beneficiaries, display of linelists/ My village My Home tool, vaccine and logistics preparedness, printing and distribution of planning/ reporting format and IEC materials, preparedness for IEC activities, etc. The Supervisors are expected to assess the preparedness and planning of SID in their respective districts and provide timely support and feedback to the district to ensure quality implementation of SID in the district. Visit Schedule The supervisor team for every district need to cover district level monitoring, each supervisor is to visit at least one blocks, two sectors/ccps, two Sub-centres and two villages. In practical terms, if the supervisory team has two members then number of checklists which are to be completed and submitted would be: 5

8 Standard Operating Procedure (SOP) General instructions for filling the tool Please select the most appropriate respondent at various levels for Special Immunization drive. You may ask for the nodal point/ lead for SID, it may be ACMO, DIO, ADC (Health) or any other district level official at district level. Estimated time to fill every checklist is 1-2 hours. Please inform the importance and estimated time to the district level respondent before the interview. To mark the correct/ appropriate response, please check the box. In some questions, some numbers are expected to be filled. Please ensure filling numerator and denominator both. In some of the questions marked as *, more than one response may be correct, please check all the appropriate/ relevant responses. To check the availability of different formats or IEC materials or vaccines/ logistics, supervisors should verify with records/ registers/ physical presence/ other means of verification Supervisor need to send the completely filled checklist tool, incompletely filled tool will not be acceptable. While answering the questions, keep track of negative responses, try to assess the bottlenecks of the problem, accordingly suggest the best solution. 24 Supportive Supervision during Campaign Monitoring checklists during campaign There are four checklists for Implementation or campaign phase: District level monitoring tool Sector/ Cold chain point level monitoring tool Session site monitoring tool House to house monitoring format The objective of conducting Supportive Supervision during the campaign phase is to see if Special Immunization Drive activities are being conducted as per conceptualization and plan, in the districts at various levels. These checklists would be useful in specifically monitoring communication, cold chain and SID campaign activities. In case, there is any deviation observed, Supervisors are expected to facilitate on-site corrections. The supervisors are also expected to provide the feedback at block and district level through participation in daily evening meetings. Visit Schedule The supervisors need to cover maximum number of session sites possible (3-5 sessions/ day) every day during the campaign. Supervisors are also expected to do house to house validations from 2 nd day onwards in the villages/ areas where sessions have already been done. After the session monitoring and house to house validations, supervisors should also do sector/ CCP and district level monitoring through the given checklists. Each supervisor is to visit at least 1. One cold chain point per day 2. SID session- 3-5 per day 3. House to House format- 5 per day (2 nd day onwards) 4. District level monitoring tool- once per round

25 Standard Operating Procedure (SOP) General instructions for filling the tool Please select the most appropriate respondent at various levels for Special Immunization drive. Yes otherwise even if one of the all the available boxes is not satisfying in terms of dry and cleanliness, then select No. Considering the recall bias and variation in power availability, it was considered optimum to collect the average power availability in 24 hours for the last week. So select the power availability as per the following bracket, less than 8 hour, 8-20 hours and>20 hours. There has to be some back up for power supply. If it is available, then select Yes and if not available, select No. Usually three types of options are available (Generator, Inverter, Solar) and select as per availability At district level, the requirement of dedicated space for cold chain equipment (CCE) is always there. If the space is provided in an exclusive room for the CCEs it is always welcome. Even if there is dedicated space made available is a large room, even that would also serve the purpose. So enter Yes if available, otherwise No is to be selected. Enquire from the Cold Chain Technician to show the space used for repair and maintenance of CCE brought from the PHCs. Take his feedback about the adequacy of space for this purpose and also use your own judgement to assess the adequacy. If it is adequate, select Ask the CCT to show the toolkits and spare parts and select Yes or No only after physical verification The diluents and syringes are to be kept in a dedicated area of the DVS facility for stock management. If it is available, select Yes and if not available, select No. The vaccines should be transported from DVS to the block vaccine stores or CCPs using appropriate cold chain system. Usually dedicated Vaccine Van is available for this purpose. So if it is available, select Yes, otherwise select No. It is recommended to place the icepacks inside the Deep Freezers in crisscross pattern, so that all the sides of the icepacks get equal exposure to the cold air. This results in faster formation of ice inside the Icepacks. If it is arranged as per guidelines, select Yes and if recommended practice is not followed, select No. Check atleast 5 icepacks (10% of icepacks) and enter Yes if filled up to mark and capped, select Yes, otherwise select No. Conditioning of icepacks to be done on a clean surface. Any table / elevated platform could be considered as an option. However floor of any room should not be considered as an option at any cost. So select Yes, if satisfactory option is available, otherwise select No Clean cloth available for wiping of Icepacks after conditioning? After conditioning of icepacks, the water on the outer surface of the icepacks is to be cleaned using a piece of clean cloth, so that when it is kept inside the cold boxes, it is in dry condition. So the availability of a clean cloth in this regard is highly crucial. Observe the practice. If the supervisor/s could not witness, then ask for the cloth used for this purpose. If the cloth is shown and it is clean, then select Yes and if the cloth is not shown or the clean is shown and not clean, then select No. As per the direction of GoI (MoHFW), a committee is to be formed for condemnation of equipment at the district level. Evidence to this effect is to be looked for in the form of a letter mentioning the formation of committee and about its members. If formed, then select Yes and if not formed, then select No. All the electrical Cold Chain equipment (functional or non-functional, but repairable) should have a dedicated functional thermometer. If available, then select Yes and if not, select No. As per the guidelines, Temp. log book has to be maintained for each CCE at DVS.
