1 Rotosonic Scaler bur Rotosonic scaler bur are manufactured in two shape. One is perio type and the other is flame type. Perio type is made of stainless steel but flame type is made of carbide. Characteristics & Cautions in usuage 1. Rotosonic burs have 6 sides with non-cutting edge and will not injure soft and hard tissue. 2. When used in high speed air driven handpiece( 200,000~ 400,000 rpm) with light hand pressure, they will produce rotary ultrasonics of about 20,000 vibrations per second. The six flat sides of the bur strike the calculus and fracturing it; the water mist from the highspeed handpiece cools the bur and flushes away the debirs. 3. They should be used 1 or 2 mm above epithelial attachement when used for subgingival root planning. 4. Final hand curette root planning is recommended for more smooth root surface. 5. Perio type is made of stainless steel but the head of flame type is made of carbide. The small perio type is used to remove small pieces of calculus and clean between roots. It can also be used within the gingival sulcus to remove subgingival calculus. The large flame type bur is used on the tooth crown or exposed roots where large amounts of calculus have accumulated or in case of advanced periodontitis. 6. Stainless steel head will not injure soft an hard tissue with high rpm but wears down easily. On the other hand, carbide head is more sharp and keeps its sharpness for a long time. So when carbide is used for root planning, reduce rpm and pressure applied to the

2 tooth. Purpose 1. Originally they were developed for supragingival, subgingival calculus removal. 2. They can also be used for gingival soft tissue trimmer ( gingival troughing ) before impression making procedure. The following Soft tissue trimmer ( Meisinger ) or Soft tissue trimmer ( Axis dental ) are used for gingival troughing but very expensive. They are more than 10 times expensive than ours. When this bur is used for gingival troughing, blood can oozing out from gingival sulcus in case the gum is not in healthy state. In this case we recommend paste type such as Gingimaster or Expasyl instead of gingival cord and liquid type hemostatic agent such as Hemodent for bleeding control. Although paste type is less time consuming, it s up to practioner s preference.

3 Most of times, small perio type is used for gingival troughing but flame type can be used when the gingival sulcus is deep and thick such as distal to second molar. 3. Rotosonic burs can be used as Periotome before tooth extraction and extraction can be extremely easy with this procudure. Small perio type can be usually used but some practioners prefer to use flame type for this purpose because flame type is more aggressive and cut the bone more easily. For this purpose, push the bur in several points around tooth and connect each points together. Then practioners can get widened alveolar sulcus just like when Periotome is used. As stated on the above, stainless steel head perio type wears down easily but it can be used for this purpose after it becomes dull. The following pictures are how to use for periotome substitute and the apperance of tooth surface after rotosonic bur is applied for this purpose. 4. Large flame type rotosonic bur can be used to treat peri-implantitis. Many companies developed tools ( laser detoxification, air abrasion, brush) to treat peri-implantitis but we believe shaving down the fixture thread is the best way. Detoxification of fixture surface after smoothing will be helpful though. Practioner can also use diamond bur for this purpose but 6-sides flame type carbide bur is less aggressive. a.

4 # 37 fixture was smoothened using flame shape rotosonic bur due to peri-implantitis. b. # 26 fixture was smoothened using flame shape rotosonic bur due to peri-implantitis. In this case, we could get bone fill around # 26 fixture after 6 months but further research will be recommended. 5. Flame type rotosonic bur can be used to cut attached gingiva. For example, the attached gingiva around partially impacted wisdom tooth can be easily cut without using knife or laser. But do not use to cut non-attached gingiva.

5 6. Flame type rotosonci bur can be used to contour bone around tooth in gingival flap procedure. 7. Flame type rotosonic bur can be used to shave down glass ionomer filling or overhaning amalgam filling. It also works very well to remove glue on teeth from removed braces since 6 sides non-cutting edge will not shave down enamel. But we do not recommend this bur for the contouring of composite resin filling. Sharp perio type bur which has stainless steel head can also be used for this purpose but the edges wil become dull soon. 8. Both flame and perio type bur works very well to remove stain on teeth.

6 Comparison with other products A. Roto-pro scaler ( Ellman ) Ellman produce 4 different shapes of rotosonic bur with six sides non-cutting edges but they are all made of stainless steel. They wear down very easily and expensive than our burs. B. Desmoclean bur ( Hager )

8 The application stated above is just a bullshit Low speed cannot generate ultrasonic vibration. They easily become dull and very expensive. C. Root planning bur RA Same as desmoclean bur, low speed cannot generate ultrasonic vibration. The diamond wear down very rapidly and very expensive. It s good to say that they are for root shaping instead of root planning.
