2 Overview: Peronia is a hillside community on the outskirts of Guatemala City. The average person in Peronia earns about $400 per year and families struggle to fulfill basic needs. As you can imagine, having you traveling to their town and bringing a service they would not have access to otherwise is a big deal to the patients you will see, they are genuinely grateful (expect hugs) and will wear the best clothes they own to their appointment with you. 4 Patient Services Reporting by Operatory Prescriptions Dental Visit Example Protocol Overview: Ideally the end goal is to have every patient have a comprehensive exam as soon as is reasonable. The reason behind this is so every patient is informed and can plan to take care of their oral health longterm. FIRST VISIT: 1. Chart all existing fillings and missing teeth with a blue colored pencil Visual exam best if done with an assistant i.e. #30 MO filling, #3 extraction 2. Chart all the teeth that have decay or needs a restoration with RED colored pencil. If there is an existing restoration on a tooth that is failing place a red line under the blue line that marked the existing restoration i.e. #30 existing MO filling is failing: chart a blue line and then a red line below the blue line, this marks what needs to be replaced. 3. Take the necessary x-rays. (SEE X-ray section for GUIDE) Example 1. Patient has a toothache 1 BW and 1 PA may be necessary for diagnosis Example 2. Patient comes in for basic exam 2 BWs or 4 BWs may be appropriate Example 3 Patient comes in for a comprehensive exam
5 Take full set of x rays Take a full set of photographs of the patient s mouth Evaluate the periodontal condition Oral Cancer exam 4. Review your findings with your patient from the exam, x-rays and photos (if taken). SEE Patient Exam, Photos GUIDE and X-rays GUIDE sections. Help patient to be aware of what of the condition of their mouth Discuss with the patient the possibilities of treatment Discuss the benefits of treatment Formulate a Treatment Plan with the patient SECOND VISIT 1. When treatment is completed, blacken with pen over the red line on the chart. This ensures that every chart is completed and accurate. i.e. #3 MO was charted in red. Now it is black signifying treatment is completed. 2. Always write down the patients next visit so that there is no confusion as to what the next procedure is, especially if there is more than one dentist treating the patient which is typical. Patient Treatment Planning Patient Treatment Planning Example Patient Flow Description Typically, you can expect to see a patient every 45 mins. or so averaging 1-2 procedures per patient (8-10 per day). Patient arrives and checks in with front desk, reviews their medical history, gets triage and is charted if needed by local team. As a dentist, you will treatment plan the patient Chart all missing teeth and decay - Determine needed x-rays. X-rays would be reviewed and diagnosis and appropriate treatment made by you. For example: filling, cleaning and or extraction. Treatment is completed by next available dentist on site. Post-op instructions are given through anyone who can translate, the front desk will go over any prescriptions and dispense nonprescription medication as needed. * Feel free to adapt to whatever scenario works best and coordinate with the local staff and students as you feel is needed. When volunteer dentists join in there are a couple of setups that have worked well:
6 Scenario 1: Triage System Example (Multiple Dentists Group): 3 dentists are on a week of service, 1 person diagnosis, charting, treatment planning and the patient is turned over to restorative chair dentist who performs the treatment. This can be a great rotation if desired. Scenario 2: Full Service System Example: Single Dentist 1 dentist with local staff students. Take patient all the way through beginning to end with the assistance of a dental student using mentoring opportunities and role switching. Variable Scenarios - Dentist plus staff or dentist + family - Specialty Week or Specialist - Mentoring from U.S Dentists to local dentists and students Infection Control Infection control is an important part of every week, consistency when there, is always staff variations is accomplished by having a local staff member doing sterilization throughout volunteer weeks. If you have any input, questions or concerns regarding infection control, please speak with dentists of the local staff to explain or address. There is an infection control and biohazards protocol available in the clinic for your review. X-Rays
