1 BadgerCare Plus Healthcheck Requirements With Periodicity

2 Program The HealthCheck Programam The Healthcheck Program The Healthcheck Program is a required service under Wisconsin s BadgerCare Plus program for children under 21 years of age. This pamphlet outlines the periodicity of the required healthchecks and the components that must be completed during the visit. The required components are the following: 1) Comprehensive Health and Developmental History 2) Comprehensive Unclothed Physical Exam 3) Age Appropriate Vision Screening 4) Age Appropriate Hearing Screening 5) Appropriate Lab Tests 6) Immunizations 7) Anticipatory Guidance In addition to outlining these components this pamphlet will highlight when a Blood Lead Test is required. It is federally required that a Blood Lead test be completed at one and two years of age. A verbal assessment will not satisfy this component. 1
