1 Child and Teen Checkups (C&TC) in Dakota County Child and Teen Checkups Narrative Report
2 INTRODUCTION The Early and Periodic Screening, Diagnostic and Treatment (EPSDT) benefit is a federal mandate that provides comprehensive and preventive health care services for children under age 21 who are enrolled in Medicaid. States are required to provide comprehensive services and furnish all Medicaid coverable, appropriate, and medically necessary services needed to correct and improve health conditions, based on certain federal guidelines. In Minnesota this program is known as the Child and Teen Checkups (C&TC) program. EPSDT is key to ensuring that children and adolescents receive appropriate preventive, dental, mental health, and developmental, and specialty services. Medical providers deliver the direct screening and diagnostic services for eligible children. Local public health agencies under contract with the Department of Human Services provide administrative services that include outreach and follow-up activities to promote participation in screening and follow-up. HELPING FAMILIES 12,206 Introduction letters to notify families of the importance of well child checkups - Federal Requirement 4,695 Contacts during WIC appointments to educate and assist families eligible for Child and Teen Checkups 27,966 15,361 Reminder letters sent to remind families to schedule a Child and Teen Checkup - Federal Requirement Phone contacts/attempts to provide assistance with scheduling appointments, transportation, interpreters and community resources - Federal Requirement 62% 74% of WIC clients received a Child and Teen Checkup within 6 months of outreach of families receiving Public Health Nurse home visits had a Child and Teen Checkups within 6 months of outreach 1
3 HELPING COMMUNITIES Education Outreach Activities ELL: ISD 191, 196, 197 & Special District 6: Total 9 classes ECFE: ISD 192, 196, 197, 199 & 200: Total 27 classes Dakota County Technical College: Child Growth and Development class: 2 classes ShakoCAPS student project Represented C&TC Metro Action Group in year 2 of a project with Shakopee High School Students. Students used student survey data from year 1 to develop a short pre and post intervention survey. There was an 11% increase in students who reported having a checkup on the post-intervention survey, however the survey was a blind survey, therefore it may not have been the same set of students answering the survey questions. Health and Safety Fairs (sponsored by schools): ISD 194: Ready, Set, Achieve Backpack Event participants ISD 197: Backpack Event at Garlough participants; Henry Sibley High School Health Fair participants SSD 6: ECFE Baby Shower ISD 199: Health and Safety Fair participants DCTC Health Fair - 45 participants C&TC Message in Print, Website and Social Media Friday Take Home Folders: 16 Elementary Schools District 191 Community Education Fall Catalog Zerkalo Russian Newspaper and Directory with MAG Partners Developed by Anoka County 2017 Pocket Calendar: Distributed by WIC, Family Home Visiting Staff and at Community Events Community Resource Guide: Hard Copies and Online Utilized by: Dakota County Public Health Programs, Medical Clinics, Dakota County Libraries, Regina Hospital in Hastings, Fairview Ridges Hospital, Faith Communities and other community partners who request copies. Facebook Posts: 9 posts with post total reach of 6853 Dakota County Website: 1,363 hits to the main C&TC web page; 679 hits to the Provider Page that houses MAG documents Meeting Families Where They Live Outreach to children and families was provided in collaboration with the Open Door Mobile Lunch Box, Dakota County Libraries and School District 194 for 16 events at residential sites throughout the county. Metro Islamic Center Trinity Evangelical Free Church Medical Clinics Annual visits to the 37 clinics that provide care to children eligible for Medical Assistance Utilized a Pre-Clinic Checklist to gather data and analyze clinic adherence to the C&TC Schedule of Age-Related Screening Standards. The checklist guides the plan for a particular clinic and for county clinics as a whole. C&TC component training at individual clinics: 13 Vision Screening Evaluation developed by MDH. Results sample: 1. How would you rate your ability to identify required C&TC vision screening components? 1.55% increase in knowledge based on a 5 point scale following training 2. Monthly team meetings are a time to plan upcoming outreach activities and evaluate effectiveness of activities in the past month. All community outreach and clinic activities are documented in PH-Doc. Reports developed in 2017 to improve ability to do ongoing monitoring. Shared companion outreach activity for WIC with WIC coordinators and supervisor each month. Implemented new process for documentation of Child and Teen Checkups for family home visiting nurses as part of a Continuous Quality Improvement project.
