7 Continuity of Care Model Mobile Dentistry OHI s Mobile Dental Teams will travel to Broward County Public Schools to deliver preventative dental services indoors and during school hours Oral Examinations Prophylaxis (Dental Cleaning) Dental Sealant(s) Placement for Permanent Molars Fluoride Varnish Treatment Oral Hygiene Instructions/Education Dental Home Referral (Diagnosed Decay) Participating Students Will Receive Free Toothbrush Free Toothpaste Oral Hygiene Pamphlets/Brochures Dental Home Referral Information

8 Mobile Dentistry OHI Mobile Team 1. Site Coordinator 2. Florida Licensed Dentist 3. Registered Dental Hygienist 4. Dental Assistants School Liaison Each participating school will select an individual(s) to act as the liaison for OHI. The selected individual(s) will volunteer his/her time during/or after school hours, if necessary. Qualified liaisons include: teacher, nurse, office administration, Principal etc. Responsibilities include, but not limited to: Oversee permission forms Coordinating with teachers/staff to ensure each student receives registration forms Collection of submitted consent forms (forms will be organized by classroom with a classroom roster) Collaborate with OHI s Site Coordinator to ensure scheduling of dental service date(s). Work alongside Site Coordinators by directing participating students to and from designated dental area.

9 School Based Preventative Dental Consent Forms To be distributed during the beginning of the school year or sent home with students on the 1st day of class Permission slips must be returned to the classroom teacher or main office, and delivered to the School Liaison Site Coordinator will communicate with School Liaison for consent form totals and pick up Preventative Dental Service Day OHI Dental Teams (Dentist, 4-5 team members) arrive at 7 am for setup in assigned space (empty classroom, gymnasium, auditorium, library, etc.) Students will be excused from class for up to 30 minutes to receive preventative dental treatment Dentist will perform examination, as hygienist & assistants deliver cleaning, fluoride, and sealants. Take Home Forms After dental treatment is completed, each child will receive a take home letter. Parents will be informed of the dental service their child received and whether any diagnosis was made. Students with diagnosed decay will receive an office referral forms, which lists dental practices in the surrounding area that accept all Florida Medicaid Plans & PPO insurance Take home forms and/or dental information will be available in Spanish, Haitian Creole, or other(s)

14 Outcome-Based Dentistry The Dentist or Dental Hygienist will evaluate the status of each child s oral health. Evaluations will be broken down into three categories: Good oral health. No caries were charted/no additional treatment is needed/routine six month check-up One to four caries are present. Restorative treatment needed/dental referral Five or more caries/abscess is present/major treatment is required/child requires urgent care Students diagnosed in categories 6002 and 6003 will be supplied with a dental home referral. If families request/require specialty offices, OHI s administrative team will assign families to the appropriate dental home.
