1 Administrative Policies and Procedures Originating Venue: Medication Management Policy No. MM 2704 Title: Look-Alike/Sound-Alike Medications Cross Reference: Date Issued: 11/14 Date Reviewed: Date: Revised: Attachment: LASA Medication List Page 1 of 1 Purpose: Look Alike Sound Alike (LASA) medications involve medications that are visually similar in physical appearance or packaging and names of medications that have spelling similarities and/or similar phonetics. PMG has developed guidelines and practices in order to identify Look-Alike/Sound-Alike medications and prevent the potential for medication errors that may result from confusion of these medications. Policy: Using the LASA medication list from ISMP, along with any other medication deemed a candidate for LASA status, PMG will identify these pairs and take steps to ensure their proper storage and labeling that calls attention to the potential for medication error. Staff will be educated regarding LASA medications, their storage and the importance of safe medication administration. Please see attached list for a list of LASA medications used by Premier Medical Group. Procedure: 1) All LASA medications will be stored in a storage bin within in the locked medication area and will have a colored sticker applied to the medication in order to call attention to the potential for medication error. The LASA medications will also be separated in the storage area in order to enhance the awareness for the appropriate precautions to be taken with that particular medication. 2) The LASA medications will be updated at least annually per Joint Commission standards and as new medications are added to the formulary. Staff will be educated on the Look-Alike/Sound Alike Policy of PMG upon hire, upon additions to the formulary and upon annual competencies.
