1 Roche CoaguChek XS Plus Error Codes For Firmware Version onwards Description of Error The instrument will not turn on -The instrument has 'frozen' or not receiving power -Remove the power to the meter AND remove the battery pack. Leave out for 10 seconds and replace battery and then plug back into the power -If instrument is still not turning on, contact iccnet CHSA technical support Description of Error Asking for a QC code chip when trying to test quality control material -Selecting Control Test instead of Patient Test -All liquid quality control testing should be performed as a Patient Test rather than a Control Test and the QC barcode scanned as the patient ID -Patient ID has not been entered -Enter the Patient s UR number as the Patient ID. If a UR number is not available, enter the Patient s Surname followed by their date of birth (i.e Smith ) -Test strip past its expiry date -Check that the date setting in the setup menu of the instrument is correct -Dispose expired test strips. Insert a new test strip from a new test strip lot which ha not expired. Insert the corresponding code chip from the new lot.

2 -The barcode on the test strips cannot be read -Power off the meter. Please check the test strip and restart the test with the same test strip. -If error continues to occur, try using a new test strip from a different vial and/or different lot. If problem continues further contact iccnet CHSA technical support -The test strip is unusable or the test strip was not inserted correctly. -Power off the meter. Please check the test strip and restart the test with the same test strip. -If error continues to occur, try using a new test strip from a different vial and/or different lot. If problem continues further contact iccnet CHSA technical support -Code chip for current lot of strips has not been inserted into the instrument -Insert the missing code chip (the code number on the chip should coincide with the number on the box of strips) -If correct code chip cannot be located contact iccnet CHSA technical support PLEASE NOTE: Instrument has a memory and will remember previous code chips -The battery is running low -Plug the instrument into the power source, the instrument should be plugged in at all times.

3 -Instrument too cold or warm -Unplug the instrument from the power source. Ensure the ambient temperature in the room is C and allow the instrument a few minutes to come to room temperature -Power off the meter and place the instrument in a warm blanket or hold it in your hands to help raise the temperature -Sample detection has failed -Power off the meter. Repeat measurement with a new strip and blood taken from a new puncture of a different finger -A dosing error was detected (Not enough sample, sample applied to early, blood volume too low or blood too thick, scratches on the electrode, test strip wet) -Power off the meter. Repeat measurement with a new strip and blood taken from a new puncture of a different finger -Measurement error (Test strip was moved during testing or faulty, haematocrit is too high) -Repeat measurement with a new strip and blood taken from a new puncture of a different finger -Do not touch or remove the test strip during testing

4 -Sample error -Repeat measurement with a new strip and blood taken from a new puncture of a different finger. Ensure samples is applied within 15 seconds of lancing the finger. -In rare cases, E-406 may be received by patients with abornal or unusually long clotting times (>8 INR), or anticoagulated samples (INR>10) e.g. Heparin. - If this error message appears again when repeating the test, results must be checked with another method. Notify your medical officer. -Hardware self-check error -Switch the meter off and wait for at least 15 minutes. Switch on the meter. -Self test barcode error detected -Reset the instrument by removing the battery pack and unplugging the power for 10 seconds -Repeat measurement with a new strip and blood taken from a new puncture of a different finger -If error reoccurs again, contact iccnet CHSA technical support -Code chip error (code chip is not properly inserted or code chip is damaged; code chip slot is broken) -Insert the missing code chip (the code number on the chip should coincide with the number on the box of strips) -Remove code chip and reinsert it, making sure that the code chip clicks into place -Check code chip slot for damages -If error continues to occur, contact iccnet CHSA technical support

5 -Hardware or electronic error -Communication error with the dock -Remove the meter from the dock and re-dock it -Turn the meter off and on. Restart the dock and check the power and network cables are connected -Internal communication error within the meter -Internal communication error with the barcode reader
