2 Everyone with Diabetes Counts (EDC) Objectives: Initiative Improve access to diabetes self-management education (DSME) workshops for Medicare beneficiaries identified with diabetes Health disparities focus for minority and rural populations Improve clinical care and outcomes for people with diabetes in medical practices within our region 2

3 EDC goals We aim to improve the quality of life for all persons with diabetes by expanding opportunities for DSME QIN-QIO Goals for Arkansas: 1,724 Medicare beneficiaries with diabetes completing a sixweek DSME workshop series 50 participating practices working on diabetes/cardiac care measures: A1c, lipid testing, comprehensive eye exam, foot exams, blood pressure control Develop a sustainable model in our region 3

4 Data for Arkansas Arkansas went from 5.3% of people with diabetes in 2004 to 11.5% in % of Arkansans age have diabetes 22.1% of Arkansans over 75 have diabetes Number of Arkansans with Medicare who have diabetes - 53,973 Total number of Arkansans with diabetes - 288,781 4

5 How do we plan to improve access? Train educators to provide community, peer-led DSME classes through partnerships Engage Medicare beneficiaries and people living in rural communities with diabetes in DSME classes Technical assistance on PQRS, diabetes objectives and patient education intervention techniques PI CME program on diabetes care and management 5

6 How do we plan to improve access? Encourage recruited providers to distribute diabetes fliers and utilize prescription pads to refer patients to DSME workshops Refer participants that complete the 6 weeks DEEP workshop to an accredited AADE or ADE program site Continuing support plan from accredited program sites 6

7 Benefits of Partnership with EDC Initiative Training and tools to support DSME programs Marketing support and sustainability for DSME programs Improvement in practice clinical metric outcomes (PQRS) Improvement in work flow process and patient satisfaction Technical assistance for the Certified Diabetes Educator exam 7

8 What is required of Peer Educators? Attend 2 ½ days training to become certified in DEEP Provide free DEEP community workshops Have each participant complete paperwork that is provided and send to TMF (FedEx envelopes and packing slips are provided) Have fun! 8

9 Diabetes Empowerment Education Program (DEEP) Evidence-based curriculum developed by the Midwest Latino Health Research, Training and Policy Center at the University of Illinois at Chicago Format includes a training-of-trainers and an educational curriculum Curriculum is designed to engage community residents in self-management practices to help control diabetes 9

10 Objectives of DEEP Provide residents in our communities with tools to better manage their diabetes Reduce diabetes complications and live healthier, longer lives Facilitate short- and long-term behavioral changes Empower individuals to make changes that affect their health 10

11 Implementation Use the curriculum s modules (8 modules) and activities in the workshops DEEP sessions 1 day a week for 6 weeks Session duration is 1 ½ - 2 hours 11

12 The Impact of DEEP workshops in Arkansas to date: 911 participants have enrolled in the last 18 months 572 have graduated from the DSME workshops Conducted over 89 classes 80% retention rate 12

13 Benefits of Partnership with EDC Initiative Free training and tools to teach DEEP classes Visibility as a community supporter of a life-saving program for people with diabetes A service provided under your organization brings added value to your program 13

14 Benefits of Partnership with EDC Initiative We do not bill Medicare, so we will not use the beneficiary's allotted hours per their Medicare benefits A resource to accredited programs for Medicare patients that cannot afford their DSME co-pay at their facility Support from AFMC with classes and sustainability of program 14

Refer patients with diabetes to community DSME workshops Secure venue for facilitating workshops Passionate people to lead DSME workshops Provide a designated office champion to work with EDC consultant Implement improvements to enhance diabetes selfmanagement care Complete requested program documentation 15
