2 Goal of the Project The goal of Diabetes Connections is to reduce the prevalence and morbidity of type 2 diabetes among individuals in high morbidity zip codes.
3 Partners This project was funded by subcontract from Grant/Cooperative ti Agreement 1U58DP from the Centers for Disease Control and Prevention (CDC) and REACH U.S. SEA-CEED, Medical University of South Carolina College of Nursing.
4 Participants Five churches were selected from the Community Health Leadership Academy (CHLA). The purpose of the CHLA is to engage faith-based organizations in educating and training i community health ambassadors. Four lead health ambassadors participated from each church health ministry. Churches selected for this project must have completed the Black Women s Health Imperative Health-Wise Woman Diabetes Prevention Education Project training.
5 Activities Developed standardized screening protocol for type 2 diabetes risk factors in church congregations. Conducted intensive screening training for health ambassadors. Conducted concept mapping activity to understand how members of the community conceptualized diabetes.

7 Our Concept Mapping Process Brainstormed with group during comprehensive screening training resulting in 68 different statements in response to the focus prompt. In a special session, sixteen (16) selected project members, including health ambassadors and project health professionals, sorted and rated these statements as to their importance in relationship to the focus prompt. Using multi-dimensional scaling and cluster analysis, maps of the underlying community perspectives were developed.

14 Using the Results Maps can be used to develop components of the curriculum for the Community Health Leadership Academy. Maps can be used to identify differences to improve the collaboration between Health Ambassadors and the Health Professionals Team. Specific c cluster components identified ed can help frame the concept of health disparities for a specific community. Broad-based and specific health ministry recommendations can be provided to support the health improvement efforts of participants and their community. Final participant analysis will provide a context for developing a consensus for Next Steps.
