1 Community Health Needs Implementation Strategy FY FY2015

2 FY2013-FY2015 Community Health Needs Implementation Strategy Overview Located in Jasper, Alabama, Walker Baptist Medical Center is part of Baptist Health System, a faithbased, not-for-profit health care system consisting of four hospitals and a network health centers serving north and central Alabama. With 267 beds, Walker Baptist Medical Center is a major provider of health care services in Walker County and the surrounding area and is dedicated to the Baptist Health System s faith-based ministry, focusing on whole-body wellness for all patients. This report includes the FY2013-FY2015 plans for Walker Baptist Medical Center to address the prioritized health needs identified in the Community Health Needs Assessment report. The full Community Health Needs Assessment report is provided in a separate companion document. Target Areas and Population As described in the Community Health Needs Assessment report, Walker Baptist Medical Center serves the broader region, which includes a population of over 77,000. The majority of the population is White (90%), followed by African American (4.5%), and Hispanic (2.7%). Current estimates project an increase in number of older adults, with those aged 65+ years expected to grow 10% over the next five years. Health Priorities The FY 2013 Community Health Needs Assessment for Walker Baptist Medical Center was conducted using both statistical data on the current health status of the as well as interviews from individuals who provided key insights into the broader health interests of those served by Walker Baptist Medical Center. The resulting health needs were complied and ranked using a system that incorporated comparative methods with those health needs meeting high or medium criteria deemed as of greatest priority. Subsequently, 8 health priorities were identified as meeting high or medium criteria and are listed with a brief description below: 1. Cardiovascular Disease: The heart disease death rate is higher in than in the State and health education relating to chronic diseases (including cardiovascular disease) was mentioned as a topic of interest by members of the. 2. Health Care: Members of the expressed concerns over lack of knowledge regarding when to seek medical care. WBMC Version 1 - June 2013 Page 2 of 7

3 FY2013-FY2015 Community Health Needs Implementation Strategy 3. Maternal and Child Health: The infant mortality and low birth rates in are higher than Alabama. 4. Nutrition: Members of the mentioned nutrition and other nutrition related issues as a concern. 5. Overweight/Obesity: More than one-third of adults in reported a BMI greater than 30 (higher than Alabama) and reducing obesity was mentioned as important to members of the. 6. Smoking: Members of the mentioned smoking as a health concern. 7. Sexually Transmitted Infections (STIs): The rates for select STIs are higher in than Alabama and members of the expressed the need for education on reducing risky behaviors in teens. 8. Substance Abuse: Several members of the discussed substance abuse as a health concern. Development of Implementation Strategy Following a comprehensive review of the Community Health Needs Assessment report, the Walker Baptist Medical Center leadership team was responsible for determining the best approach to address the identified health priorities. The resulting Implementation Strategy links specific activities to achieving targeted outcomes for each priority s specified goals and objectives. The activities in the Implementation Strategy including existing s and partnerships as well as new activities aimed at enhancing the benefits Walker Baptist Medical Center provides to the. Additionally, rationale was provided for any health priorities that will not be address at this time (if applicable). Implementation Strategy FY FY2015 The following Implementation Strategy details the plans for Walker Baptist Medical Center to address the health priorities over the next 3 years: WBMC Version 1 - June 2013 Page 3 of 7

4 FY FY2015 Community Health Needs Implementation Strategy Health Priority Goals, Objectives, Strategies, and Partnerships Activities Performance Outcomes Cardiovascular Disease To support activities aimed at increasing education, awareness, and the reduction of cardiovascular disease and related health conditions ; RSVP Senior s ; Potential opportunity with the University of Alabama Department of Health Science Conduct cardiovascular screenings at events and provide educational material on cardiovascular disease and risk factors associated with cardiovascular disease Initiate and promote an educational on the signs and symptoms associated with having a stroke (including when to seek medical attention) Support the initiatives aimed at identifying and reducing potential cardiovascular disease risk factors including - Blood pressure screenings, smoking cessation classes, discounted memberships to Weight Watchers, enhanced access to gym/physical fitness activities, and health eating/nutritional education Partner with the RSVP Senior s to provide education sessions specific cardiovascular health in older adults (65+ yrs old) Partnered with the University of Alabama Department of Health Science on the preparation and submission of a grant to fund a heart health coaches in In funded the will track effectiveness of health coaches in reducing cardiovascular disease in Number of cardiovascular screening events Number of participants screened for cardiovascular disease Specific resources for stroke awareness campaign Number of RSVP educational sessions Number of participants attending RSVP educational sessions Number of individuals participating in cardiovascular health related screenings and activities through the Baptist Health System Workforce Wellness Increased awareness of efforts to reduce cardiovascular disease in the Health Care To improve and effectively promote access to health care resources and increase information on when to seek medical attention Baptist Health System; Health Action Partners and HOPE Clinic; Potential opportunity with the Capstone Rural Health Center Utilize the Baptist Health Minute (TV segments) to provide educational information on when to seek medical attention Collaborate with the Health Action Partners to support available transportation services and awareness of services (Clastran, etc) Continue to provide access to health care services for underinsured and uninsured patients through partnership with the HOPE Clinic Number of broadcasts (Baptist Health Minute) with information on when to seek medical attention Number of patients utilizing transportation services Improved perception of access to health care the services in Partnered with the Capstone Rural Health Center to develop promotional campaign focused improving awareness of health care services in the rural communities of Number of patients utilizing HOPE Clinic WBMC Version 1 - June 2013 Page 4 of 7

6 Health Priority Goals, Objectives, Strategies, and Partnerships Activities Performance Outcomes Smoking To improve the awareness of smoking cessation resources provided and to continue to support effort to reduce smoking in Walker County Baptist Health System; ; Potential partnership with the Jefferson Health Action Partners ; Investigate potential opportunity to expand Faithfully Fit into Support the to provide smoking testing and cessation resources Utilize the Baptist Health Minute (TV segments) to provide information on the importance of smoking cessation including reducing smoking in adolescents Investigate potential partnership with the Jefferson Health Action Partners and opportunity to utilize anti-smoking initiatives in Support the Baptist Health System non-smoking hiring policy by screening potential employees for smoking and only hiring nonsmoking employees Investigate potential opportunities to expand the Faithfully Fit (in partnership with local churches and the United Way of Central Alabama) in Successful pilot of coordinated health conference for residents of Jefferson County (2012) with emphasis on promoting healthy living, smoking cessation, and tobacco prevention through targeted educational and ministry focused activities Number of employees provided with smoking cessation resources Number of broadcasts (Baptist Health Minute) with information on smoking cessation Improved awareness and utilization of smoking cessation resources in the Sexually Transmitting Infections (STI) To support an increased awareness of sexual abstinence s in efforts to reduce sexually transmitted infections Local school systems; Department of Health Pregnancy Testing Resource Center (PTRC) Support the Health Department and PTRC abstinence by providing educational speakers at local schools and events Number collaborative events/speaking engagements Number of participants at educational secessions Increased efforts to reduce the burden of sexually transmitted infections in Substance Abuse WBMC will not directly address the expressed needs relating to Substance Abuse at this time Limited resources will prevent Walker Baptist Medical Center from addressing this health priority; however, we will continue to support area rehabilitation centers and local school system efforts to reduce the prevalence of substance abuse throughout the Not directly addressed Continued efforts to support the decrease of substance abuse and effective treatment in WBMC Version 1 - June 2013 Page 6 of 7

7 FY2013-FY2015 Community Health Needs Implementation Strategy Ongoing Monitoring To ensure the health priorities are being addressed, the Walker Baptist Medical Center leadership team will review and update the Implementation Strategy in an ongoing manner beginning with its formal adoption through FY2015. Updates to the plan may include revisions to existing activities as well as additional s and partnerships that further the goals to address each health priority. The next scheduled Community Health Needs Assessment will be conducted for FY2016. Approval The Community Health Needs Assessment report and FY2013-FY2015 Implementation Strategy were reviewed and approved by the Baptist Health System Quality and Missions Committee. Any changes to the Implementation Strategy will be provided for review to the Quality and Missions Committee and incorporated into this report annually.
