2 01 About Penumbra Penumbra is one of Scotland s largest mental health charities. We support around 1600 adults and young people every week. Founded in 1985, we provide a variety of innovative services in communities across Scotland: 1 st Response and Crisis Support - Short term support for people experiencing a mental health crisis. ARBD - Recovery focused support for people with Alcohol Related Brain Damage (ARBD). Carers Support - Support for people who are caring for someone with a mental health challenge. Nova Projects - Services that promote recovery, social inclusion and selfmanagement. Peer Support Services where staff use their own experience of mental ill-health to help other people recover. Self Harm Projects - Services for both adults and young people who self-harm. Short Breaks - Supportive breaks for people to focus on their recovery. Supported Accommodation - Supported housing and tenancy projects. Supported Living - Practical and emotional support provided in people s own homes. Wellness Centre - Access a range of services in the community which promote positive mental health and support people to recover from mental ill health. Our vision for Scotland We envisage a society where people with mental health problems expect recovery and are accepted, supported and have the resources to fulfil their potential. To achieve this vision our Strategic Plan sets out five aims (RPIII): Recovery - To ensure that all mental health services and activities are hopeful, personalised and recovery focused. People - To ensure a highly valued, skilled and involved workforce. Innovation - To develop, support and provide innovative responses that promote recovery, social inclusion and citizenship for people with mental health problems. Influence - To promote mental health and wellbeing in communities by influencing; policy makers, funders and others. Improvement - To ensure quality and continuous improvement in all business processes to achieve our mission. RPIII is supported by objectives and a results and monitoring framework. As part of this framework, Penumbra services compile an Annual Review which summarises the work undertaken to meet the five key aims, to achieve our mission, and to realise our vision for society. This Annual Review is based around the RPIII structure, with sections for Recovery, People, Innovation, Influence, and Improvement.
3 02 Service overview Contact details Penumbra, Unit 20, Grampian Court, Beveridge Square, Livingston, EH54 8ER. e Click here to enter text. t About the service The West Lothian Supported Living Service was set up in June 2000 to work in partnership with West Lothian Council. We provide support that enables people with mental health difficulties to remain living independently and safely in their own homes. The service is based in Livingston and covers an area of over 165 square miles, reaching from Broxburn in the East to Blackridge in the West. West Lothian is a diverse mix of urban, rural and isolated towns and villages set alongside the growing new town of Livingston. We provide Housing Support, under the flexible Mental Health framework commissioned by the West Lothian Council. The ISA is funded under SDS option 3.We also provide support for (Social Inclusion), commissioned by the Social Work Department and funded through SDS option 2. This is a three way contract between the Supported person, the Council and Penumbra. Under option 2 the Supported person chooses their preferred provider to meet agreed outcomes. Support is primarily provided Monday to Friday during office hours, however the service is adaptable. If support is requested outside these hours it is provided to meet individuals needs. Joint working is part of the ethos of the service and we link with the NHS, Moving into Health, Drug and Alcohol (WELDAS), the Mental health Advocacy project, and others. This approach facilitates recovery, promotes choice, and provides a holistic service that meets the needs of those we support. This helps us to achieve national and local strategies. Types of support provided Support is provided in a variety of ways. We normally visit people in their own homes, on a one to one basis, with support tailored to the needs of the individual. Some people take part in group activities and we hold POWWOWS (Penumbra Workshops On Wellbeing).

Referrals for Social Inclusion come to us via Social Work Department. However this year we have supported one person who self referred and is funding their own support. Referrals / /2018 Referred Exited Referrals has remained consistent, 15 people exited the service, 3 moving to a more supportive enviroment,10 people successfully met the outcomes they wished to achieve. People who use our services complete an I.ROC by scoring 12 wellbeing indicators every three months. These scores allow us to build up a picture of their wellbeing over time, to assess and measure personal recovery journeys. Scores for everyone using a service can be averaged to show general trends. 6 05 Recovery story We provide a service focused on working towards Recovery. We encourage people to be involved in creating their support plans and enabling them to have more control in their lives. We work towards developing a person's self management strategies, valuing themselves, and building links with the wider community. We promote the use of local community resources and support people to source activities which will help them achieve their outcomes. We use tools in the HOPE toolkit to promote Self-Management. Donald's Recovery Story I have been depressed for the past 3 years without knowing what caused it. My life was hiding away - no TV, no music, no reading papers, spent days just lying in my bed. Then one day, my Penumbra worker was trying to see what activity I would like to do. She mentioned Indoor Bowling and when she did, it brought memories back when I used to go to Indoor Bowling with my grandpa and watch him play. I found out there was Indoor Bowling at Lantern Community Centre where I met Jack who runs it and found there were about 14 people there, men and women from age of 45 to the golden oldies. The idea of the game is to roll a bowl at a small yellow ball and play your bowl to as near as you can. Since taking a new medication and playing bowls it has given me a new lease of life. I am driving again, watching television and taking an interest in news and current affairs, I now have two holidays planned for the next year. I now have something to look forward too. I no longer need as much support, I have made new friends and turned my life around, and I look forward to a better life and a new start.
7 06 Recovery story Jim's Recovery Story When I was referred to Penumbra I did not go out on my own due to my anxieties about travelling and lack of finances. I did not feel comfortable meeting new people, I had no bus pass and was unsure of what benefits I was entitled to, and I had no health care service involvement. Now I regularly attend Social groups, have a fantastic social care package, a bus pass that I use to travel to college and several agencies provide healthcare. It has been a successful outcome thanks to agencies working together to get the best outcomes for me. My symptoms have not magically disappeared but I am looking forward now. Recovery Feedback and the I.ROC The I.ROC data and feedback from Supported people provides helpful information which shows the service outcomes as well as the individual Supported people's outcomes. Part of our aim last year was to encourage people to attend groups and activities thus reducing social isolation as people identified at assessment stage that being lonely and isolated was one of the factors causing low mood. The service has made a positive difference in the area of social networking. In the last year people have got together for the walking group, a visit to Polkempt Park and the owl sanctuary, a day at the beach in North Berwick, the annual Christmas party and a few people attended the West Area Road show. This year s results are disappointing for Hope For The Future which shows the biggest difference. This is an area that we can work towards improving in the next year. The support gave me a reason to get out of bed and out of the house. It has encouraged me to go out walking and increased my motivation.
8 07 People Staff West Lothian has a team of 11 people -a Manager, three Recovery Practitioners, 4 full time Recovery workers and 3 part time Recovery workers. Over the past year 2 people have left the team one member of staff retired and another moved on to work in a hospital setting. Over the past year we have provided placements for 3 students, one person training to become a CBT therapist and two students from West Lothian college completing their Health and Social care higher. Training Training for staff can be sourced through the Penumbra training programme, e-learning and external agencies and other organisational bodies. We have an established team in West Lothian who have all made use of the e-learning provided by RBS mentor and used the SSSC and the Hub to refresh their knowledge in areas such as Safeguarding, Data protection. We had a full day of training on I.ROC and how to enter data on to Carista. The majority of the team completed 2 day medication training with the remaining members of the team booked to complete in June members of staff completed SVQ level 3. The majority of the team are trained to this level with the exception of 2 workers. As a team we looked at guidance provided on Chronology reporting and in preparation of the principal of care changes completed an exercise looking at the Health and wellbeing and Social Care standards assessing and reflecting on what contributes to good quality care. The team gave examples of the ways that they meet these standards. They went on to explore what we can do to improve the way we work. The team spent part of the day looking at our inspection report and the QAF report findings to begin the implementation of a development plan for the following year and again looking closely and offering suggestions on ways to improve our existing service. SSSC registration The Manager and three Recovery Practitioners and 5 of the Recovery workers are all registered with SSSC under the criteria of Housing support services. The 2 remaining staff submitted their registration applications and are waiting for clarification from SSSC. One of the Recovery practitioners, who is in a supervisory, role has the necessary qualifications required as part of the registration. The other two Practitioners are hoping to start their accredited training in 2018.
9 08 Innovation When looking for ways to improve the service, part of this process was to ask staff to look at the principals of care and our development plan. We sent out a participation questionnaire to those we support. We asked people if they would like to be involved in the shaping of the service, something different to sending out questionnaires. As a result we are moving forward in the next year to hold meetings with Supported people to gather their views on whether we meet the Health and Social Care standards and how we can build improvements into our development plan. We have worked hard to keep the teams in area's providing people with a core group of workers who can support them and we hope that these changes will reduce the amount of travelling time and the costs involved. We work closely with West Lothian college to provide student placements for NC and HNC Social Care students and will continue to offer this in the future. Students often return to us to work on our Relief register and in some cases have joined the team as Recovery workers. During the time that students are on placement they shadow and are mentored by Recovery workers, this helps in the development of leadership skills for members of the team. POWWOWs and outreach This year our programme for POWWOWS included Snooze how to get a better night's sleep, Dealing with Addictions, Emotions and how to cope with these and to conclude the years programme a workshop on anxiety and stress and exploring coping mechanisms. These groups were co lead and planned with the help of our CBT student and one member of staff assisted by starting each group with a mindfulness activity which helped us to relax before beginning the Pow-wow. Each group was attended by a small number of people and feedback was positive. People took away self help techniques that they could use at home. The general consensus was that the groups were a good opportunity to meet others and share ideas with others on self management. "I enjoyed the company and through speaking to others who get support I have got some tips to understand things I could do that will help me, these are things that I had not considered"
10 09 Influence Throughout the year we have attended different forums and meetings within West Lothian. We attend meetings that are held by the VAWL covering such topics as Local out comes and Improvement plans, Safer communities and tenant participation these meetings are very informative helping us to understand what is happening in the area and meeting with other third sector workers. We attend regular Care at Home provider forum's to liaise with other organisations that provide personal care and joint support to people in the service; this enables us to plan co-ordinated support and promotes partnership working. Open days at West Lothian College West Lothian staff were invited in October to host a stall at West Lothian College during Mental health week. A member of staff and one of the student went along to this day and answered questions from the general public about the service and how to get help in relation to Mental Health difficulties. The day was a successful and we have recently been invited back to attend a Spring fair. Because of the success and the interest in Self help techniques, two of the Recovery workers and one of the students on placement attended and in addition to promoting our model of support gave people tips on Self management and resources they could use to improve their wellbeing.
11 10 Improvement Care Inspectorate The service had an announced inspection on the 11th December In this inspection the quality of Care and support and Quality of management and leadership were assessed with grade 5 (very good) this grade has remained the same as our previous grading history in The Care Inspector spoke to eight supported people plus three carers and evaluated responses from pre-inspection questionnaires. The feedback they received from people about the service and the support staff was very positive. People felt they had achieved positive outcomes including self travel, building confidence and being involved in the community. The report stated: People told us they had formed a very positive, trusting relationship with their key worker, people told us it did not really matter who they contacted in the office; everyone was very supportive and knowledgeable of their support needs. Quality Assessment Framework (QAF) Penumbra's Quality Assessment and Improvement Framework (QAF) has shown that the service has made improvements over the last three years. In year one the overall score was 4.06 and in this third year of assessment we scored The service has improved in three areas: Recovery, Influence and Improvement. This has been achieved by a move from paper files to storage on Carista which enables staff more immediate access to files held electronically. Comments made in the report: the team have made improvements in recording outcomes and goals, improved risk assessment process and using feedback from Supported people to make improvements and changes to the service activities. Feedback We are constantly monitoring and checking the quality of the service throughout a person's support. We do this in a variety of ways: at review and by sending out feedback questionnaires and at the end of service a questionnaire. In 2017 we sent out 67 Questionnaires and received 52 replies, 49 from Supported people and 3 from carers. This gave us a overall picture of who would like to be involved in area's such as recruitment, Care Inspection interviews and questionnaires, meeting with us to review aims and objective, policies and plans for the future. There were people who stated they did not like the constant evaluation and did not want to be part of this process or any group work and this will be respected. Overall people are satisfied with the commitment of the staff who support them, finding that the service has been of use in helping them to achieved their desired outcomes.
