3 The Kolvin Service is a specialist Tier 4 community team for young people with mental health difficulties and offending behaviour. The service is based in the North East but offers services on a regional and national level. The service is for children and young people up to the age of 18 years who exhibit serious and/or persistent person-centred antisocial and criminal behaviours and who are suspected of suffering from or have an identified mental or neuro-developmental disorder. The Community Team is part of a wider provision of Children and Young People s Forensic services delivered by Northumberland, Tyne and Wear NHS Foundation Trust that includes an in-reach service working into two secure estates in the North East and an Inpatient Adolescent Medium Secure Service, Alnwood that caters for children and young people with mental disorder with or without neuro-disability. The Community Forensic Team provides: Consultation Assessment Specialist interventions for serious sexual, fire or violent offending behaviour

4 How to refer Please complete our referral form and send with the following information to the address overleaf. Please complete our referral form available on Confirmation that you have advised your client of the referral and that you have obtained their consent. Your client s legal status. Details of your clients local Children and Young People s Service (CYPS). Copies of available assessments carried out by mental health professionals. Copies of your client s full GP medical records. A chronology of the contact your client has had with yours and other Social Services departments. A care chronology if involved with Children s social care. Details of any other agencies involved with your client. Copy of your client s updated record of criminal convictions. ASSET form, if any, and pre-sentence report(s). Minutes of any pertinent multi-agency meetings. Copies of any assessments/risk assessments carried out by your agency or other professionals.

Referral Criteria The young person will be under the age of 18 years. The young person will have or be suspected of suffering from mental disorder.

5 Referral Criteria The young person will be under the age of 18 years. The young person will have or be suspected of suffering from mental disorder. The young person will be deemed to have high person centred forensic need and/or present high person centred forensic risk. The young person must be open to another child and adolescent mental health service who will retain overall clinical responsibility and identify a care coordinator/lead professional who will remain in contact with the case throughout the period of involvement of the Kolvin Service. The Kolvin Service provides a service Monday-Friday between the hours of 9am and 5pm; we do not provide emergency psychiatric cover. If an emergency psychiatric opinion is required outside of these working hours please contact your local CYPS Crisis Team.
