Jerry Fuss Time: 8:30-9:30 a.m., followed by Maryland Rural Health Association Focus Group Location: Carroll County Health Department Attendance record available Agenda Topic Discussion Summary Follow-up Welcome and Introductions Ed Singer welcomed the group and thanked all for coming. He especially welcomed the Cancer Coalition, who is joining the LHIC. Introductions were made. Minutes Update on SHIP/LHIP Presentation Cancer Coalition The April 19, 2017 minutes were approved with one change. Tammy Black corrected the Access Carroll report to say that detox services are available 7 days a week. Maggie Kunz reported that the state has not yet implemented the new guidelines and assistance planned for LHICs. Barbara White, Director of the Cigarette Restitution Fund at the Health Department, which includes the Cancer Prevention, Education, Screening, and Treatment program (CPEST), gave a brief presentation on cancer services. The Health Department does education and outreach on colorectal, oral, breast, cervical, and skin cancers. The CPEST program provides colon cancer screening and patient navigation for low-income county residents. The Breast and Cervical Cancer program provides similar services for these two cancers. The state is piloting lung cancer screenings and Carroll has applied to be a pilot county. The Maryland Cancer Fund is available to help some county residents with cancer treatment costs. 2 Population Health Update Ed Singer reported on the statewide efforts to reduce opioid use and deaths, the Opioid Operations Command Center. An Opioid Senior Policy group was convened to review community efforts and gaps. Current initiatives include: - Shoemaker (residential substance abuse program) is moving to another part of Springfield Hospital Center so their current building can be renovated to meet JACO accreditation standards. After the renovation they may be able to increase to a 39-bed capacity. - Carroll is also in conversations with Howard County about expanding residential treatment services and opening them to other counties, as Howard has funding but not a location, and a facility that serves multiple counties would be eligible for state funding. - Crisis beds might be part of facility renovation and development. - A Mobile Crisis RFP will be released soon. A team consisting of a mental health professional and a peer will be available evenings and weekends, when the rate of drug incidents is high. - Ed will present to the Board of Commissioners on Thursday to ask for support to use OOCC and STOP grant funding to fund mobile crisis services for all hours that providers are closed. - Ed is meeting with EMS Thursday to facilitate data sharing, so the Health Department can reach out to people with multiple overdoses. Ed also said that he and Tammy Black and others plan to visit a medical respite facility in St. Mary s County. Tammy asked for anyone to send her data or stories to support the need for such a facility, which would provide a place for people without stable housing to recover from medical problems. Maggie Kunz will create and distribute flu messaging to the Flu Planning group. Any other organizations interested in messaging can contact her for more information. Community Health Priority Updates Behavioral Health Amy Baker described the Health Department s (BPWR) new $4 million grant to provide services for children 0-8? 5-8? Who are identified by a parent, doctor, or Head Start provider as having behavioral health issues. Chidren will receive an evaluation from a psychologist at the Health Department, and referrals for services to prevent more serious, long-term problems. Staff will be hired soon, and the grant will officially start in October. Heather Press, a peer support specialist, will now be focusing only on overdoses.

3 The CHES (Collaboration for Homeless Enhancement Services) program at the Health Department has been very successful. The goal was to serve at least 60 homeless individuals, and they are serving over 100, 25 of whom have been housed. They are hiring additional staff, but also looking for ways to sustain the program after the grant ends in 1 ½ years. The demand for Mental Health First Aid has increased dramatically. Trainings are available through Carroll Community College. Many groups have already been trained, including Health Department staff, the Westminster Police Department, and the Sheriff s Office. Chronic Disease Prevention (diabetes, heart disease, obesity) Carol Ann Bauman reported that the evidence-based National Diabetes Prevention Program (NDPP) is being offered to Health Department and Access Carroll staff, and that hospital staff from the Diabetes Center have been trained and will offer it to their staff starting in August. The Health Department plans to offer an NDPP class to the public this fall, following a prediabetes awareness campaign. Lisa Wack reported that The Partnership s Advancing Health and Wellness Leadership Team has been focusing on specific diseases such as diabetes and cancer, to examine resources and gaps that could be filled. The Healthy Aging Leadership Team is planning a skin cancer awareness campaign, with flyers in public bathroom stalls. Tobacco Use Leslie Wagner reported that the Bureau of Aging & Disabilities is providing an evidence-based self-management program to the community free of charge. It will cover both Chronic Disease Self-Management and Diabetes Self-Management. The program is titled Living Healthy, Living Well and they will soon be added to the Bureau s web page. Workshops are 2 ½ hours long for 6 weeks and led by 2 facilitators. Originally developed at Stanford University, the sessions are very scripted. Chronic Disease Self- Management workshops are scheduled for the fall at North Carroll and South Carroll Senior Centers. Barbara White reported that the Health Department s Cigarette Restitution Fund offers two walk-in clinics a month with free patches and gum. Program staff will be working with the school system to create an e- cigarette curriculum for elementary and high schools; a middle school curriculum was created last year. 4 Oral Health Carol Ann Bauman reported that the Oral Health Program at the Health Department has decided to expand the fluoride rinse program to all elementary schools, because even when the school s water is fluoridated, the students may not have fluoridated water at home. To reduce cost, the middle school rinse program is being eliminated; data showed that younger children benefit more and participate more. Tammy Black reported that Access Carroll has hired a new dentist, Dr. Boodhoo, who is excited about community partnerships and will attend ED team and other meetings after she gets settled. Also, Access Carroll has served 2200 individuals, provided 700 dentures, and done 1600 restorative procedures in FY17. Other LHIC Priority Health Concerns Flu Maggie Kunz reported that the flu messaging plan for , created through the guidance of the multi-organizational flu vaccination planning committee, is available. There is also a flyer highlighting the group s two main messages wait until after October 1 to get your flu shot, and get the quadrivalent if possible. The Health Department s web page has been updated, and the resource page with pharmacy information and local vaccination clinic information will be available this fall. Flu shot clinics are scheduled and high schools will be included this year; better participation is expected in the second year of flu shots instead of FluMist. The School Health Council is working on mental health initiatives over the summer. Access to Health Care/ Social Determinants of Health Access Carroll Tammy Black let the group know that Access Carroll is hiring 3 positions to help them expand services. She will also be sending out information soon about the next Cross-Agency Training, on November 17 th, which will focus on Behavioral Health. Look for an update about the next Cross-Agency Training. Health Benefit Exchange Door to Healthcare will no longer be HBE after June 30 th ; but the new organization, HealthCare Access Maryland (HCAM), is keeping local navigators in place at DSS and Access Carroll. However, calls should go through them to be directed to local navigators.

5 Open enrollment will be held from Nov 1 st -Dec 15th only this year. There will be events at the beginning and end of this period, Beat the Rush on Nov 4 th and 5 th, and Last Chance on Dec 9 th and 10 th. Maryland Children s Health Program MCHP Transportation: - MA Transportation - Transportation Advisory Committee (TAC) New ACCU and MCHP Supervisor Taina Metsa-Mahka reported that program staff are still going to the Detention Center, usually first Wednesday of the month and then toward the end of the month. Parole and Probation, every week in the afternoon and no specific days. Staff has also met with Triangle recovery, Mountain Manor and Shoemaker Center. People in the parole and probation programs receive assistance with Medical Assistance application process. She also mentioned that clients with AmeriGroup may experience some inconvenience as they no longer work with Rite Aid pharmacy. Amanda Haugh reported that MA Transportation is hiring new staff. David Reese, Deputy Director of Public Works, recently acquired the county s transportation program. He reported that the county has received new vehicles and now as 16 buses, after not receiving promised buses for several years. The blue buses are Trailblazers that provide shuttle service, and there is now an extra one in case any bus breaks down. White buses provide door-to-door service. The office is developing new routes based on the Transportation Advisory Council recommendations. The process will take several months to complete, as MTA needs to ride the routes to determine the schedule. Then the county will train drivers, and invite representatives from special interest groups to ride the routes to see if they are accessible and meet their needs. Then there will be a public hearing process and the Board of Commissioners must approve the routes. Dave hopes to have the new routes in place by January 1, 2018, and to offer free trial rides to attract new users. Carroll County has one of the best veteran s shuttle programs to transport vets to area VA facilities. David Reese can be contacted at dreese@ccg.carr.org. He will attend the October meeting and provide updates. Other updates/announcements Community Health The Partnership is in the middle of the Community Health Needs Needs Assessment Assessments. Key informant sessions, to elicit feedback from community leaders, and focus groups to explore the needs of underrepresented Carroll County residents are encouraged to take the community health survey at

6 populations, are scheduled. Over 900 people have already taken the community health survey. Other news Next Meeting Adjournment The Nursing Bureau at the Health Department is being reorganized, but this will not impact the services offered.
