This is accomplished through a combination of county-operated services and contracted services with private not-for-profit corporations. County-operated services include Outpatient Clinics for adults and children, a Continuing Day Treatment program for adults and Case Management for adults and children. A Public School Outreach Program to all local schools providing on-site consultation and early intervention is operated through our children s clinic. Genesee County Mental Health also provides coordinated community mental health services through its Single Point of Access (SPOA) program. The contracted services are the Genesee Council on Alcohol and Substance Abuse clinic and prevention programs, Mental Health Association of Genesee & Orleans Counties (Social Club), NYSARC-Genesee Co. Chapter Vocational Services, the Restoration Society s ACE Employment Service, Care & Crisis Helpline, Cazenovia Recovery Services and Living Opportunities of DePaul.

2 DEPARTMENT: MENTAL HEALTH - COUNTY SERVICES STRATEGIC FRAMEWORK Mission Community Mental Health Services Board "It shall be the Mission of the Community Mental Health Services Board to promote the development and insure the delivery of quality, cost effective mental hygiene programs of prevention, education, treatment, rehabilitation, and community support for individuals and families in Genesee County coping with mental illness, developmental disabilities, and alcohol and substance abuse." Mission County Clinical Services "Our Mission is to assist county residents with symptoms of mental illness & substance abuse in achieving and maintaining their highest level of functioning." To accomplish this Mission we will: Respect the dignity and worth of all individuals. Provide quality services in a caring, nonjudgmental manner. Encourage the development of full potential and individual responsibility in the areas of living, learning, working, parenting, and socializing. Work cooperatively with other community resources. Promote a positive working environment for all staff. Maintain compliance with NYS Mental Hygiene Laws and Office of Mental Health Regulations. 4 CONTRACT AGENCY: RESTORATION SOCIETY - Co. Acct. # 4323 DESCRIPTION: The Genesee ACE Employment Program is designed to prepare mental health consumers for employment through psychiatric rehabilitation readiness assessment, skills assessment and development, and individualized planning services. ACE develops supported employment positions in local businesses and assists in filling the positions with the ultimate goal of increased independent functioning, satisfaction and success. Participants are individuals diagnosed with serious and persistent mental illness who have not been successful in utilizing other employment services, have had difficulty in maintaining independent, competitive employment, and demonstrate a desire and/or need for support in choosing, getting, and or keeping employment. This program is supported strongly by State Reinvestment Funding and involves no county tax dollars. The Peer Specialist Program at LeRoy Manor is designed to provide outreach and peer support to individuals with mental health conditions in a residential setting. The Peer Specialists assist in the development of self-advocacy skills, development of peer networks, educational and life skills groups and support groups. They serve as a role model and advocate in the areas of entitlements, empowerment, and daily living activities and accompany residents to medical appointments, shopping and other activities in the community. Peer Specialists work with residents both individually and in group settings to identify and achieve goals that they have personally identified. The Peer Bridger Program, through collaboration with Genesee and Orleans County Mental Health systems has implemented services to assist with helping to ease the transition into community living for individuals being discharged from Rochester Psychiatric Center (RPC) and other Article 28 facilities. The Peer Bridger is a trained individual with successful management of their mental health recovery and offers candidates ready for discharge from aforementioned facilities many service interventions: Engagement in a uniquely personal positive supportive relationship with a peer; Involvement in an array of peer support meetings; Linkage to a broad range of community based services and natural supports; and Teaching community adjustment and wellness self-management skills. Typically, customers (clients) and the Peer Bridger meet prior to discharge to establish a rapport and trusting relationship to develop respective goals, roles and responsibilities and identify other support needs. The Peer Bridger then works with the individual in the community to transition back into community living. Support services are individualized and can range from (6) six months to (1) one year, dependent upon individual need. Services may be provided for follow along support longer than (1) one year if needed to support the individual to remain successful residing in the community. The program also works in close collaboration with the Genesee County Department of Mental Health, residential providers and other community service entities. CONTRACT AGENCY: NYSARC - GENESEE COUNTY CHAPTER - Co. Acct. # 4322 DESCRIPTION: The Genesee County Chapter NYSARC, Inc. provides a vast array of services for individuals with intellectual and other developmental disabilities in Genesee County. Programs include Supported Work, Transportation, Vocational Case Management, Medicaid Service Coordination, Job Readiness Skills and Job Placement, a variety of Residential options as well as Family Support Services. The Vocational Program and services provide structured, intensively supervised work training and experience. Placements in real full-time competitive work situations are the final step in the continuum for some of the workers who graduate from the supportive work settings. Through these programs many individuals are able to increase their level of independence and live successfully in a community setting with the pride of having earned income. We are presently engaged in the development of entrepreneurial businesses and micro enterprises to assist in meeting the vocational goals for people of Genesee County who also have intellectual and other developmental disabilities. Supplemental funding from the County is critical to our ability to help these individuals develop employment skills with a final goal of achieving competitive employment in our community. 6 CONTRACT AGENCY: YWCA CARE + CRISIS HELPLINE - Co. Acct. # 4324 DESCRIPTION: The Care + Crisis Helpline (CCH), a Program of the YWCA of Genesee County, is a free and confidential 24-hour Crisis and Suicide Intervention Helpline. It is answered by skilled staff, better known as Helpline Specialists and provides a safe and nonjudgmental source of support for anyone in crisis. Crisis Intervention: Provide emotional support, education, assistance and intervention as necessary to all persons in crisis and those impacted by them, with the goal of reducing suicides and self-destructive behaviors. Suicide Intervention: Reduce the incidence of suicide by alleviating despair, isolation, distress and suicidal feelings among individuals in our community, 24 hours a day; to educate the public about suicide prevention; and to help reduce the stigma associated with suicide. Live Chat: Offer Live Chat via YWCA of Genesee County Website as an alternate choice of communication. Member of the Genesee County Suicide Prevention Coalition: The Care + Crisis Helpline serves as the designated helpline number on the Coalition website and promotional materials. In addition, CCH responds to any requests generated by the Coalition website. Information & Referral: Link callers to resources (i.e. support groups, food pantries, transportation, inpatient/outpatient programs, food, prescription, rent & utility assistance, housing/emergency shelter, counselors/therapists, mediation/legal, parenting classes, etc.) On-Call Services: Provide On-Call linkage to Genesee County Mental Health Services (After Hours) & the YWCA of Genesee County Domestic Violence Crisis and Prevention Services (24- hours). Also provide Safe House call checks to clients staying in the YWCA Domestic Violence Safe House. (Evenings & Weekends) Medicaid Fraud Line: Maintains Medicaid Fraud Line and relays reported information to Genesee County Mental Health Services, Genesee County Health Department and GCASA. Law Enforcement Transport Information: Provide a single point of entry for any Genesee County Law Enforcement Agency to contact with information regarding the transportation of a Genesee County Resident to a Hospital due to Mental Health Issues or Crises. Mental Health Emergency Respite Beds: Serve as the single point of entry for client eligibility intakes for respite bed placement. Maintains occupancy status on both the single and double respite beds and sends daily status s to participating service providers Utilization of CCH Contact Information during After Hours: Many Genesee County Service Providers, Therapists, Agencies and Organizations put CCH contact information on their afterhours answering machine message or cell phone voice mail to contact in case of an emergency that cannot wait until normal business hours. DESCRIPTION CONTINUED: Hearing Impaired & Language Translation: Utilize the New York State Relay services to meet the needs of the hearing impaired community. In addition, we have access to over 222 different languages and dialects through the CTS Language line which helps to address any language barriers. Emergency Management Plan: CCH is part of the Genesee County Emergency Management Plan as well as the Genesee County Mental Health Emergency Management Plan

Acct. # 4314 DESCRIPTION: Liberty Hall is an 18-bed Community Residence located on the grounds of the Batavia VA Hospital in Batavia, NY that specifically accommodates homeless veterans with substance use disorders. As certified NYS OASAS program, our Community Residential program differ from the traditional halfway house model due to their highly-structured nature. Our goal is to provide a chance at recovery by providing support, guidance, and life skills training using evidence-based treatment including Trauma-Informed Care practices. Above all else, we are dedicated to fostering independence and building futures for our residents. Our Community Residences provide responsive and individualized service planning. One-on-one and group counseling sessions are primary features at this level of care, as well. The length of stay is individualized and can vary according to a resident s clinical needs, though the average length of stay is three to six months. During their stay, residents initiate pursuance of educational, vocational or employment opportunities as they prepare to return to independent living. Our programs have the unique ability to provide services in a supervised recovery setting, which allows individuals who have jobs to remain employed while participating in a Community Residence. Self-help skills are also stressed, providing a path toward restoring balance in one s own life, along with establishing productive relationships with family members and others. Acct. # 4312 DESCRIPTION: 1) (Program code & ): Genesee County Department of Mental Health funding ($25,450) is utilized by DePaul for the 3 additional beds added to our existing supported housing program located in Genesee County. This is a non-licensed program which secures more permanent housing for adult individuals with a psychiatric disability. Living Opportunities of DePaul, Inc pays an individual s security deposit, startup supplies and furniture, and a monthly stipend to the individual s landlord. Leases are in the tenants name and program staff visit tenants a minimum of one time per month. 2) Supported Housing (RCE) Program Description for Genesee County (2 beds) (Program codes / : $16,966) Living Opportunities of DePaul (LOD) will provide 3 units of Supported Housing to adult individuals with Serious and Persistent Mental Illness who choose to live in Genesee County, New York. These individuals will come from either the Rochester Psychiatric Center, one of its residential programs (such as the John Romano House), or from an Article 28 hospital such as Strong Memorial Hospital, Rochester General Hospital, St. Mary s Hospital, or Wyoming County Hospital. More specifically, 2 individuals will come from The Rochester Psychiatric Center (and/or its residential programs) and 1 individual will come from one of the Article 28 hospitals mentioned above. DePaul will provide the following supports/services to each recipient of this program; a safe, affordable apartment of their choice anywhere in Genesee County (that passes safety inspection standards), full apartment furnishings, a security deposit, a monthly rental stipend, regular staff visits to help the individual maintain his/her place in the community, the development of a Housing Support Plan, and collaboration with other service providers such as Psychiatrists, Therapists, Health Home Care Managers, peer supports and other Community based supports that are consistent with the individual s needs and choices. LOD staff will also provide some assistance with daily living skills training and ongoing advocacy as needed (such as negotiating disputes or disagreements with landlords, etc). As mentioned above, all individuals enrolled in this program will be linked to a Health Home Care Manager. Ideally, the referring hospital will make the referral to Care Management but if that does not occur, LOD staff will make the referral to the Genesee County SPOA. Once Care Management is in place, LOD staff will work collaboratively with the Care Manager so that there will be a clear delineation of roles. Referrals for this program will originate from the above mentioned hospitals and will be submitted directly to the Genesee County SPOA office/department. The SPOA will then disseminate the referrals to the LOD Intake Coordinator. Each individual will be screened (by an LOD Intake Specialist) within 3 business days of receiving the referral from the Genesee County SPOA Department. In remaining consistent with the OMH guidelines for this program, DePaul anticipates accepting at least 80% of all referrals received for placement in to this program.

10 CONTRACT AGENCY: GCASA- Co. Acct.# 4313 DESCRIPTION: The Genesee Council on Alcoholism and Substance Abuse (GCASA) operates four programs which provide behavioral health services: Outpatient Treatment, Residential Services, Prevention and Employee Assistance (EAP). The main Outpatient Clinic is in Batavia, NY with Satellite offices in LeRoy and Indian Falls. Clinical treatment services are also offered to the jail, drug court, and probation clients. Specialized services are also offered to applicants and clients of the Department of Social Services. Further specialized services are offered to women, adolescents, people struggling with comorbid psychiatric disorders, patients struggling with abuse and addiction to opiates, tobacco, and gambling through several individual based protocols including medication assisted therapy, DBT, CBT, relapse prevention, motivational counseling and 12 step facilitation. The Atwater Community Residence is comprised of 17 beds for people in need of a halfway house for 3-6 months to support their substance abuse recovery. The Residential Services program also provides 19 supportive living beds in 4 different locations in Genesee County for people who need long term residential support of one year or longer. GCASA provides a variety of prevention services which includes classroom education, environmental strategies, as well as a Children of Alcoholics (COA) program for children who come from families where substance abuse is a problem, an alcohol and drug awareness program, server training, retail compliance checks and specialized programs to seniors, pregnant women, tobacco cessation and smoking prevention, gambling prevention and advocacy in a variety of settings and for healthy community living free of chemical abuse. GCASA also partners with the nationally award winning Drug Free Community's Coalition whose purpose it is to lower the incidence and prevalence of substance abuse in the county as well as a New York State OASAS grant for a development and training Resource Center. GCASA's Employee Assistance Program has contracts with over 30 organizations and companies in the Genesee/Orleans/Wyoming County area which covers over 4,600 employees and their family members. This program provides counseling and consultation for personal, family, mental health and substance abuse problems to decrease work related problems such as absenteeism, poor performance, low morale and low productivity. As part of the Governors Medicaid redesign, Performing Providers Systems were created around the state to reduce the amount of unnecessary admissions to hospitals. GCASA is a member of the Finger Lakes Performing Provider System (FLPPS) and is participating in several of the projects which involve behavioral health providers. GCASA has transitioned to a paperless medical record which integrates scheduling, billing, as well as assessment and treatment documentation. GCASA's electronic medical record and billing system is equipped to meet the requirements of the all current payers systems and will meet the requirements for the transition to managed care. 11 CONTRACT AGENCY: MENTAL HEALTH ASSOCIATION - Co. Acct. # 4321 DESCRIPTION The Mental Health Association in Genesee Co. promotes mental wellness through education, advocacy and support for our community, thereby improving quality of life and instilling hope. Our services include the following: The Place To Be Social Club, provides a safe environment for individuals with a diagnosed mental illness to gain social skills as well as have healthy social interaction. Activities include social outings, an Art Room and Gallery, Snack Shop and Kitchen. These activities and programs are designed to give individuals an opportunity to gain skills that will assist them in being productive community members. It also aids those that choose to return to the work environment with job readiness skills, such as preparing and serving food, taking inventory, purchasing, retail, and customer service to name a few. Transportation is provided to our members without charge to meet their needs so they may live successfully and independently in the community. Transportation to appointments, social club, employment and shopping are a few ways this is achieved. Warm Line- the Warm Line program is well established and in its eleventh year of operation. This program is a peer to peer, non-crisis telephone service that provides support and encouragement. It operates seven evenings per week, 365 days per year from 5:30-8:30 pm, when other supports are not available. Peer listeners are provided training opportunities as well as a stipend for a completed shift. Visions Of Hope Recovery Center This is a peer-driven program designed to assist individuals in finding their spark of life. Together they work on a selffocused plan toward wellness. By focusing on personal strengths and interests, individuals can have a healthy empowered life. Community Trainings / Support Groups- the MHA provides educational trainings to the community on various topics, including safetalk, Mental Health First Aid for Adults and Youth, Wellness Recovery Action Plan, and Hearing Voices that are Distressing. Support groups include: Life Bridges Trauma Support Group and the Depression/Bipolar support group. Family Support and Consultation- provides information, education and support to individuals who have loved ones with mental illness so they can learn to cope and adapt to meet their own needs as well as those of their loved ones. Family education groups & monthly NAMI Family Support Groups are provided. Suicide Support and Education- The MHA is involved in the coordination of efforts of the Genesee County Suicide Prevention Coalition and coordinates safetalk trainings and provides postvention support to those who have lost a loved one to a suicide through the SOS (Survivors of Suicide) support group. The MHA also coordinates and hosts the Regional Suicide Prevention Coalition meetings with the OMH Suicide Prevention Office of NY.
