2 The Primary and Behavioral HealthCare Integration Program (PBHCI) PBHCI is a federal funded grant that was awarded to Kedren Health in The funding revenue comes from the Substance Abuse Mental Health Services Administration (SAMHSA) based out of Washington DC. The PBHCI participants are comprised of clients suffering from a severe mental illness, a need for primary care treatment, and/or the need for substance abuse treatment. The purpose of the grant is to promote the integration of Behavioral Health and Primary Care services to ensure that all needs of treatment are addressed Along with conducting community outreach, and providing education on various behavioral and primary care related health topics such as Tobacco Cessation, Nutrition, Chronic Disease Management, and Healthy Living classes. Participants also receive Care Management services which encompasses all of the facets of case management with an additional focus on integrated health care.

4 4 History and Mission Kedren is a private non-profit corporation founded in 1965 by the late James L. Jones ( ) a child psychiatrist. The original thrust of the organization was community-based psychiatric services for the residents of the inner city. The concept of the community-based mental health services has greatly expanded since its inception and Kedren has been actively involved in this process. Consequently, the agency has recognized and developed new and nontraditional treatment methods which are required to address the unique problems of the inner city of this complex sociological area.

7 7 Consumer walks-in to Primary Care for services. PRIMARY CARE TO OUTPATIENT MENTAL HEALTH SERVICES Consumer s medical home is contacted for Hx/information. If requested, a switch is made to Kedren Health s as their medical home is made. Patient is screened to discern the need for Mental Health, Substance Abuse, Health Education & Housing Services. If MH/BH Services are warranted, referral for appointment is made in COP or AOP. BH Team conducts Triage; if medical necessity is met, consumer is referred for ongoing specialty mental health services. Primary Care is notified of disposition.

11 Peer Support Services Alcoholics Anonymous Founded in Akron, Ohio in 1935 by Bill Wilson and Dr. Bob Smith The idea of peer support was essential to the fellowship from it s inception and it continues to be today. 12 step programs AA is based on the principle of service and one alcoholic helping another.

12 History of Peer Specialist Mental Health Consumer Movement of the 1970s Chronic disease self management diabetes, mental health, heart disease, cancer, asthma, HIV/AIDS, substance abuse, screening and prevention cancer, HIV/AIDS, infectious diseases, and maternal and child health, breastfeeding, nutrition, post-partum depression.
