20 2011 Samples of Comments from California Callers who Completed QA Surveys 127 days free of gambling. Help line counselor was great! 5 months 12 days no gambling. He feels good and strong, attended approximately 4 meetings due to job conflicts. Spending more time with his children and is fixing up a "classic" car. 80 & 70 years old, wife still likes to go with her friends. She has slowed down, but not stopped. Caller is not gambling anymore, girlfriend spends her own money, so they are not in debt. He doesn't feel at their age that there is much he can "make" her do about it. Caller is in the car now as we talk taking a nap while his girlfriend is in the casino gambling. "Absolutely great, only the best to say about the service." Attended 1 GA meeting, but never returned, "I made excuses not to go. I need to go back, damage is done right now." Attended a meeting together, caller found it helpful. Fiance has not been back to casino to her knowledge but feels he knows it will cause a major problem in their relationship should he return to gamble. Attended meetings for a bit, but stopped as he didn't feel he was " that bad" but did enjoy the meetings. Does like to periodically do it, but not in trouble with it. They do not take their credit cards or more than $40 with them to the casino. Did some reading and also bought some books himself to read on gambling. Been working a lot, so hasn't followed up. Gambling has decreased since calling the help line, "help line was great." Caller and husband attended 1st meeting and "got the idea." Help line was helpful to her family. Caller attended a few meetings, but did not feel comfortable "airing her dirty laundry". She has started to work more and keeps herself busy. Caller attended a few meetings, took the info and applied to his situation. Caller continues to financially support him, however she feels confident that he is going to make progress and stop. She is controlling his finances at this point. He is seeing a Counselor in Oregon who has reported that he has no control of gambling due to brain injury. Counselor suggested he go under conservatorship, caller does not want to do this as it is too controlling and he wouldn't want to do it. Caller did not follow up on anything, but instead kept herself busy and changed her routine. Caller did not follow up on anything, Father is still gambling. Caller did not follow up on the resources provided, however he did read the information that was mailed to his home. He reports he is no longer gambling. Caller did not follow up with anything, but has kept herself busy and has no desire or energy to go to casino. Caller has not followed up due to conflicting work schedule. Caller stated his work schedule has changed and will now attend a meeting in his area. Caller hasn't followed up on anything but stated just the call to the help line was beneficial. 21 2011 Samples of Comments from California Callers who Completed QA Surveys Caller stated he attempted to attend the meeting in Temecula, but could not locate the church. He pulled it up on his computer while on the phone and thinks he might of wrote the address down wrong the 1st time he called. He plans to try attending tomorrow and will call if unable to locate. Caller was provided counseling info again tonight. He felt the help line was great when he called the 1st time, very understanding and supportive. Caller stated the help line was helpful, but she did not follow-up, she just moved out of the area and is no longer able to go to casino. Counselor at help line was good, seeing a counselor in CA now and doing well. Counselor at the helpline was great, very helpful. Counselor that he met with in person was hard to understand, went for about 5-6 sessions. All in all he feels all aspects were helpful to him and life has improved for him. Counselor was great, very friendly and supportive. Counselor was great, very helpful but GA meetings conflicts with his schedule. Help line is an incredible resource to have. Counselor was polite and helpful. Son is no longer gambling. Counselor was very informative and educational. Decided in May that she was done, stopped gambling and drugs all in the same day. Has had a promotion at her job, her child is back with her and is moving to a better home. "Thank you all for your support and being there when I needed you, it was needed." Doing better, attending mtgs. Took down Annette Taylor's name for counseling. Due to lack of transportation not able to follow up. Still gambles, but not as much. Friend has attended a couple GA meetings, caller plans to attend with him and talk with facilitator in regards to starting a new meeting. Friend has attended a few GA meetings. Counselor was great and very supportive of him during the call. Caller was very satisfied with the call and the help provided. "Good and bad days" reaching out to people in her church and at work for help. Has been attending 1x week when not attending cancer treatments. Has been attending GA since calling in January and has been slowly catching up on her bills and is reportedly not gambling. Has moved since calling, so the gambling has decreased due to lack of access. He did not follow up at this time and is still gambling. Will call us back to follow up at another time. He says that things are going "just fine now." He stated he was to have been sent info, but never received it. Offered to send, but he declined. Help line was great, didn't follow up but feels talking with Denise was great and helped him to see his issues and problems. He has stayed away from gambling. Help line was helpful and he attended a few mtgs. Still gambling at this point. "I found Christ and through the good Lord have been able to stop it all." Page 20 Quality Assurance

22 2011 Samples of Comments from California Callers who Completed QA Surveys "I would rate the service a 10 out of 10!" He is doing very well. Is currently reading the packet of literature. Last went in end of November, casino scares her, out of control and doesn't want to go back down the same road as she did with the drugs and alcohol. The lady that talked to me was wonderful, very helpful. Mother resisted and didn't move with her as planned. "Not that big of a problem, I still want to play". "Really bad, really bad." Reports attending 1 GA meeting and seeing a counselor 2x, but doesn't understand why things are not getting better. Reported that she attended 1 meeting, but didn't like it and then did not return. She did not follow up with counselor but claims she is not gambling. Reportedly her sister called the help line after this call and has followed up attending mtgs. Caller stated that the help line was "instrumental" in helping her family. Reports that he is attending GA, has gambled 1x since calling the help line. He felt the help line was helpful and has since "came to my senses." Sandi was wonderful, very helpful and understanding. See's counselor weekly for past 3 months, not gambling, help line was very beneficial. Service was great when she called, received the info right away. Husband is doing better at this point. "Service with you guys is always great, with me, not so much." Hasn't followed up anything at this point, now hanging out with a new friend who also gambles. Slowing down with the gambling, but still going. Son has been provided the info, he needs to follow up on it. "I can lead him to it, but I can't make him do it." Son is 38 yrs old and doesn't listen to her. Caller still pays for their joint credit card, encouraged her to close the account. Son is attending meetings. Spoke with Jerry several times for C2C and has not gambled since March. Currently in the hospital for illness. Still gambling but not as much. Still gambling for living, but is doing better mentally. he participated in the UCLA program and feels he has it under control. Not gambling as much, seeking other employment. Still gambling, hasn't follow up on anything, can't stop, but doesn't do anything to stop. Talking with the counselor at the help line helped, he listened to what was said. Page 21 Quality Assurance
