3 Health & Wellness Student Ambassador Outcome Goals Short Term Students are more aware of resources, services and programs Students are more aware of the benefits of maintaining personal wellness Students apply for public benefits (CalFresh and/or MediCal) Mid Term Increase in the number of students accessing resources, services and programs and receiving public benefits Increase in the number of students who work to improve their wellness Long Term Students who access resources, services, programs and/or participate in wellness activities will: Complete educational goals Transition off public benefits and earn a higher living wage Improve health and wellness

4 Ambassador Description Peer-based outreach, education and one-on-one support to raise awareness of the availability of health and wellness resources and the value of healthy behavioral choices Coordinate workshops on health, nutrition, mental health and wellness Network and partner with campus resources Organize and deliver presentations, campus resource tables, health and wellness fairs Establish partnerships with local community resources; organize clinics to increase access to these programs Coordinate and collaborate with existing on-campus student support programs

5 Work Plan Activities Activity 1: AmeriCorps Members will meet with college faculty & staff to learn about campus resources and services. Research and become familiar with on-campus resources, programs, services and events related to health and wellness. Engage in meetings with college staff who implement programs and services to learn more about how students connect and access these resources, services and programs.

Example: Create plan on how to promote the resources through one-on-one outreach, resources tables, presentations, and print materials/

8 Work Plan Activities Activity 2: AmeriCorps Members will meet with Health & Wellness community partners. Learn about existing community partners with your campus such as food banks, mental health counselors, public benefit coordinators, food and nutrition, etc. Meetings, phone calls, attend local trainings. Colleges can also recommend community partnerships, topic areas, etc. for members to explore. Members will work with community partners to implement activities, bring new resources, expand existing efforts around health and wellness.

9 Work Plan Activities Activity 3: AmeriCorps Members will provide general education to students on health and wellness. Raise student awareness of both on-campus and community partner programs, services and resources through: Campus resource tables Presentations Events Health and wellness fair Print materials Organize guest speakers Wellness workshops Food Bank information in grocery bags CalFresh application events Research & deliver curriculum Move night & discussion topics De-stress finals events

10 Work Plan Activities Activity 4: AmeriCorps Members will provide one-on-one outreach to students on wellness. Raise student awareness of both on-campus and community partner programs, services and resources. Inform faculty, staff and students that Student Ambassadors are available to meet with students to talk about various resources Create open office hours Once per week resource table introductions to faculty and staff with Members

11 Work Plan Activities Activity 5: AmeriCorps Members will provide one-on-one application assistance and follow up support for public benefits. Meet with county CalFresh coordinators or CalFresh outreach organizations. Attend CalFresh application assistance trainings. Will be provided with links and information regarding AB1930 CalFresh Student Eligibility and work rule exemptions. Webinar will be scheduled later 2017 or early 2018 Set individual meetings with students to help them with CalFresh Application Support follow-up outreach to students

19 May is Mental Health Matters Month Mini-grants 1 per college participating in the AmeriCorps Student Ambassador Program with Health & Wellness focus area Plan an activity or event for May is Mental Health Matters month Beginning in January 2018 Ambassadors will complete a short form and description of planned activity or event and how the funds will be utilized Gift card to college $500 VISA Implement activity in May Complete short report form

20 CRISIS TEXT LINE CCC partnership with the national crisis text line, launched in May 2017 Provides free, 24/7 support for CCC students in crisis via text Text COURAGE to Trained crisis counselors CCC Crisis Text Line materials

22 HEALTHY TRANSITIONS Healthy Transitions provides curriculum and training activities for young adults transitioning out of foster care and focus on increasing awareness and information about the many facets of wellness and self-care and the importance of healthy habits.
