It is with great pride and a feeling of accomplishment that your Union brings to you the Local 338 Member Assistance Program. This program has been designed to provide assistance to members in various areas, including: This booklet describes the program s services in more detail, as well as its rules and regulations, policies and procedures. Please read this booklet thoroughly so you understand each program and how to use them. The Union s principal purpose is to serve your needs. The Member Assistance Program (MAP) is one more way in which your Union is working for you. Fraternally yours, John R. Durso 3
3 Financing What about member s eligible dependents? An important element of your MAP is money! Where it comes from and how it may be used should be of interest to you. Who Pays for MAP? The governing body of your Union, the Executive Board of Local 338 RWDSU/UFCW, approved the creation of the Member Assistance Program and authorized the expenditure of the Union s assets to pay for the cost of the Program. The Union s assets come from your dues and initiation fees and from investment income. Eligibility Who is Eligible for MAP? Prescription/patient assistance services and information regarding community resources are available to the member s eligible dependents. Your eligible dependents include: unless legally separated (by virtue of a legal separation agreement, or court decree or judgment) birth to age 26. (A child who reaches age 26 who is physically or mentally incapable of self-support at the time and who is covered under the Plan on the date he/ she reaches 26, may continue to be eligible provided the child remains incapacitated. Proof of incapacitation must be received by the Union If requested, you will be required to submit additional proof of continued disability before eligibility will be extended.) 4 Termination Program Services When will eligibility terminate? Your eligibility for MAP services terminates on the day when any of the following occurs: in a unit covered by a collective bargaining agreement between your employer and Local 338 RWDSU/UFCW; bargaining agreement with Local 338 RWDSU/UFCW; thereof, is discontinued or eliminated; or dures of the program as set forth in this document or by the MAP Prior Assessment MAP provides services intended to assist you when confronted with a number of different crises or serious circumstances, and include: Treatment Referrals Domestic Violence Assistance Referrals If you are facing violence at the hands of a family member or domestic partner, the MAP Director can help assess the situation and make referrals to community services that may be able to intervene on your behalf or provide a temporary safe haven. MAP provides alcohol and substance abuse assessments and referrals to treatment or counseling for eligible full-time members only. The program can provide family members with information about community or other resources offering alcohol and substance abuse treatment, but family members are not covered by the program for assessment, treatment, or counseling.the program includes the following services, subject to pre-assessment by the MAP Director: Director, and MAP s assessment of medical necessity and member compliance with treatment regimen. Payment for Alcohol and Substance Abuse Services When a referral for counseling or treatment is required, there are costs incurred. Such costs are paid by the MAP Program only if: of the member s needs and the services provided are medically necessary contracted with the Local 338 RWDSU/UFCW s MAP program to provide such services and to which the MAP Director has referred the member, and program guidelines as communicated by the MAP Periodic Sessions for Alcohol and Substance Abuse Service Once per week, or as needed according to the MAP Director s assessment, the member must also meet with the MAP Director to review his or her status for reassessment. Such sessions commence upon acceptance into the MAP program and continue until released by the 8 9
6 Mental Health Referrals With depression and anxiety becoming more prevalent, the Member Assistance Program is here to assist individuals who are struggling with mental health issues such as anxiety, stress, depression, PTSD and other related mental health diagnosis. Individuals who suffer from mental health, have access to mental other services to help you in the community. Community Resources professional with social work and substance abuse background. The MAP director has access to contacts and resources in the community that can assist you and your family if you are in need of services and help.
