3 Family Reunification, Recovery & Resiliency Program serves multi-trauma-abused women with co-occurring chemical dependency & mental health challenges who are experiencing homelessness whose parental rights have been challenged by Child Protective Services, and their children under the age of 7. We provide housing, services & support for recovery, mental health stability, & family reunification, as well as interventions to prevent return to addiction, violence & homelessness once in permanent housing.

9 1. Financial assistance to secure the best possible location the Phoebe can reasonably afford. 2. Through this intervention alone, the mother will not have to seek poor housing options, live with dysfunctional family members, or past violence partners to have a place to live that put her at risk. 3. Being a support system for the Phoebe when the environment triggers a negative response or poses a risk to relapse. 4. Continued development with housing partners and service providers to develop and sustain a solid communitybased support system around the Phoebe in her new home community. 5. The Phoebe Forever Aftercare element takes the woman through her first year of sobriety in the community, helps her confront fears, temptations and barriers and provides safety net funding until she can build a small reserve to survive small setbacks without compromising her housing status. 6. When the women leave a safe and stable environment, they are at risk of ceasing medications and habits that have contributed to important gains. By sustaining the length of time they are able to manage their mental health and gain skills in a supportive environment, their resiliency strengthens.

10 Since 2015, we have served 22 women 90% (20 of 22) chronically homeless remain housed 16 opened a checking account 6 women are currently employed 8 are currently in school 12 are actively engaged in counseling 15 have gained increased custody 10 of the women have gotten their CPS case closed 15 have increased income 1 is opening their own business 5 out of the 10 women on mental health medication have remained stable on their medication 100% of women that did not continue mental health medication have relapsed
