Innovation in Organ Donation Darren Malinoski, MD, FACS Professor of Surgery Oregon Health & Science University
Disclosure Darren Malinoski, MD Grant funding: NIH Laura and John Arnold Foundation Outside relevant work: Honoraria / travel multiple Organ Procurement Organizations (OPOs)
Innovation in Organ Donation Improvement versus Innovation Tweaks versus Transformation? Science of Donor Management Donor Management Goals Mild Hypothermia RCT
BACKGROUND Need for greater quality and quantity of organs available for transplantation National average of three organs transplanted per donor (OTPD) Inconsistent donor management practices Lack of scientific foundation Paucity of RCTs Risk adjustment models lack robust donor critical care data
BACKGROUND Checklists have demonstrated utility in several arenas Standardized critical care endpoints Donor Management Goals (DMGs)
OTPD and DMGs: Current state in 2007
BACKGROUND UNOS Region 5 DMGs Initially utilizing published recommendations, clinical experience, and expert opinions, representatives from 8 OPOs created a DMG Bundle Meeting the bundle = achieving any 7 of the 9 DMGs Critical Care Endpoint DMG 1. Mean Arterial Pressure mmhg 2. Central Venous Pressure 4 12 mmhg 3. Ejection Fraction (EF) > 50% 4. Vasopressor use 1 and low dose 5. Arterial Blood Gas ph PaO2:FiO2 (P:F) > 300 on PEEP = 5 7. Serum Na < 155 meq/l 8. Blood Glucose < 180 mg/dl 9. Urine Output > 0.5 cc/kg/hr
DMGs Assessing the Impact Phase 1 Phase 2 Phase 3 Phase 4 (The Web Portal) Design Retrospective Prospective Prospective Prospective # of DMGs in Bundle Dates 2007 Time points collected 10 9 Modified 9 Modified 9 Prior to organ recovery (OR) July 2008 July 2010 At authorization hrs into case Prior to OR August 2010 February 2012 At referral At authorization hrs into case Prior to OR March Current At referral At start of case At initial allocation Prior to OR Number of donors 320 SCDs 914 DBDs 1684 donors >10,000 donors
