2 1 Introduction Dear colleagues, We have developed these two sets of instruments following the idea of Dr. Martin Benedict, Karlsruhe/Germany, to create instruments suitable for a minimally invasive approach in otoplastic surgery, which is becoming more and more popular worldwide. The line comprises two instrument sets as follows: - Elevators for right and left ear for creation of the subperichondral tunnel - Scoring instruments for right and left ear for the anthelical weakening. The two instrument sets have been designed following the anatomical shape of the ear and have an ergonomic and slender shape. They are being used in setback procedures of prominent or protruding ears. An overview on the technique and advantages can be found on the following pages. Seite 2

5 5 Medical Background & Indications Diagnostic and Therapeutic Approaches An important part of otoplasty to correct protruding ears is the weakening of the anthelical region of the ear cartilage in order to facilitate its folding and to reduce the risk of recurrence. The anthelical scoring instrument is an elegant device designed for the anterior scoring of the hypoplastic anthelix. It is primarily developed to be used in the Minimally Invasive Otoplasty (MIO), a technique which is gaining increased interest worldwide. Different types of minimally invasive approaches have been published already in the past. The first publication was made in 1967 by Kaye, a Plastic Surgeon in the United States. Another important publication about the Incisionless Otoplasty was made in 1995 by Michael H. Fritsch, an American ENT Surgeon. Dr. Benedict has developed a modified version of this technique. The key part of his technique is the creation of a naturally appearing anthelix by combining permanent buried sutures and a conservative cartilage-breaking technique, both applied minimally invasive without extensive skin incisions and excision. In a MIO, a subperichondral tunnel anterior to the anthelix has to be created with the elevator. The scoring instrument is then being inserted into the tunnel and allows precise cartilage scoring with ist specially desinged cutting head armed with four parallel identical micro-blades. The size of the blades avoids cutting completely through the cartilage with the sequelae of visible edges. The anthelical scoring instrument is suitable for conventional open otoplasty techniques as well. In total there are about setback otoplasties performed worldwide per year. Seite 6
