7 Jack PCL Brace Dynamic knee orthosis to relieve the PCL Concept The Jack PCL knee brace is a unique solution for PCL injuries, offering a dynamic anteriorly directed force to hold the leg in a neutral position so preventing any potential for posterior sagging. The force set through the hinge is individually adjustable to the size and weight of the patient s leg and is appropriate for both conservative and postoperative treatments. The Jack PCL knee brace is also routinely used in cases of multi-ligament trauma, treating conservatively PCL and MCL tears and so minimising other surgical interventions at a later stage. Jack PCL Wearing Time Experience has shown that if the brace is only utilised for two months there is a risk of instability re-occurring, since the collagen is still under re-vascularisation. We therefore recommend that the Jack PCL brace is worn for 3-4 months. It is important to explain to the patient the principles behind the product so that he/she is convinced of the necessity to wear the brace 24 hours a day for the full rehabilitation period. If the patient removes the brace at night, or loosens the straps, the effect will immediately be lost and the graft or isolated tear is then exposed to gravity and most likely stretching. For a conservative treatment, the Jack PCL brace should be applied within the first three weeks post trauma. Rehabilitation Active muscle exercises are recommended right from the start, using quadriceps training to avoid too much muscle atrophy. For hamstring training the patient is placed in a prone position and then the brace can be removed. We recommend the ischiocrurals to be trained from the hip area with the knee in 20 of flexion and the hip in hyperextension to avoid unfavourable force vector. Partial weight bearing of 15kg to 20kg is used for the first 8 weeks. There are clear rehabilitation protocols laid out in the LaPrade clinical study referenced below. 9 Principles of the Jack PCL Brace 1 The Problem: A Rehabilitation of the knee joint after reconstruction of the posterior cruciate ligament heavily loads the reconstructed ligament, especially when the foot is resting on the floor and the leg is hanging through, as in sitting position diagram A, or if the ischiocrural muscles are contracted during active flexion of the knee as in diagram B. The consequence is elongation of the transplant and recurrence of posterior instability. Jack PCL 2 The Goal: In order to prevent the graft from elongating, posterior sag of the knee must be avoided during the rehabilitation phase until the transplant is re-vascularised and matured using a counter force of corresponding size during this 3 to 4 month period, as in diagram C. B During conservative treatment, the constant support and protection of the ruptured ligament is essential to avoid the ligament healing in an elongated condition. 3 The Solution: Using the Jack PCL brace, the posterior draw position is avoided independent of the degree of flexion of the knee joint. 11 DynaCox The hip concept Concept The original DynaCox hip orthosis was the first dynamic hip abduction brace. The treatment concept is a functional combination of bandage and orthosis. The bandage provides compression and helps reduce excessive flexion of the hip. The orthosis offers containment by clever use of the counter forces generated by its pre-tensioned design. The brace is constructed from three basic measurements and ensures a semi bespoke fit, allowing simple fine tuning for the clinician when supplying the brace. With the leg piece constructed at an abduction angle of 20, even when the patient works against this force by adducting and flexing, the brace responds with a counter force through the femur pad to give the required containment. This close fitting orthosis allows more normal movement and improved compliance, whilst helping return confidence to those unfortunate to have had, or who are at risk of a dislocation. Rehabilitation The normal wearing time for DynaCox during the rehabilitation therapy is 24 hours a day for 12 weeks and should be reviewed at appropriate stages throughout the treatment. The combination of well thought out biomechanics and component design significantly reduces the risk of dislocation, whilst allowing normal muscle action to promote rehabilitation. The three point principle stabilises the hip joint, so that a static abduction position is replaced by a dynamic abduction guidance. The patient may therefore place the leg involved actively below the load line of the body weight and still enjoy protection from dislocation. The abductors are then put under strain with every step and atrophy can be avoided. 13 Options We can replace the standard TRI-Flex joint (Flexion limited from 70 but no limit on Extension) with a ROM-Flex joint which controls the maximum angle of flexion and extension of the hip. This range can be adjusted by the clinician in increments of 15, with settings from 0 to 90. We can replace the standard Lateral Side Bar with a Short Side Bar for patients who have a femur less than 36cm long. (femur length = distance from knee crevice - greater trochanter). It is also possible to purchase additional bandages separately. 15 Special features of the new DynaCox evolution Modular hip brace concept The new, modular hip brace consists of just four components, pelvic belt, joint, condylar clasp and bandage. The flexible belt now allows for the whole brace to be laid out almost completely flat. This feature is especially beneficial when fitting the brace with the patient in a lying position. 1 The snap fit system for the pelvic belt allows for quick and simple adjustment for all treatable hip circumferences (70cm 150cm) DynaCox Hip Concept Click & Lock Closures The Click & Lock closure system makes it possible to adjust the length of the orthosis without the use of any tools. This easy adjustment is possible where the joint meets the pelvic belt and where the condylar clasp with lateral bar meets the femoral pad. This secure connection also makes the brace tamper proof Adjustable Condylar Clasp The newly designed condylar clasp has a flexible anterior cuff and adjustable condyle pads. This allows maximum adjustment in fit across an entire size range and complete control in this critical area with optimal fit both laterally and medially. The soft cushioning material on the inside of the cuff and air pockets on the inside of the pads avoids pressure points and maximises compliance. 5 4 New Bandage with Innovative Knitting Technology The cut of the bandage has been optimised to adapt better than ever to the patients anatomy. 17 CDS Controlled Dynamic Stretch Dynamic spring loaded orthoses Introduction Dynamic spring loaded orthoses can be used to treat joint contractures caused by both neurological and orthopaedic conditions. The shortening of the tissue surrounding the joint decreases the range of motion, affecting the patient s everyday life. Therefore, the CDS concept aims to increase the range of motion without pain by applying a constant, appropriate low load prolonged stretch (LLPS). Contraindications: bony blockings, osteoporosis, thrombophlebitis Treatment The CDS principle, Controlled Dynamic Stretch, applies a dynamic low load prolonged stretch to the structures limiting ROM in order to stimulate the growth of the shortened tissue and increase the range of motion. The combination of LLPS and active movement stimulates blood circulation in the contracted tissue in order to return it to a normal level of weight bearing and mobility. Through this physiological contracture treatment, fast results with high compliance are achieved. Firstly an exact diagnosis of the ROM deficit is assessed and the first therapeutic goal is defined. In order to achieve a plastic remodelling of the contracted tissue, treatment times from 30 minutes to several hours are required. Spring tension and degree are adjusted according to diagnosis and sensitivity of the patient. At the beginning of the treatment the goal should not exceed 5 but can be increased as treatment progresses. The brace can be worn at night and during all resting phases of the day. CDS Contracture The shells are constructed to provide the highest wearing comfort and excellent pressure distribution. The adjustable limitation of the redression area protects the tissue from detrimental, painful overstretching. The spring tension can be adjusted individually and is shown by an indicator. The easy, safe handling increases the acceptance by the patient.
