1 Orthopedic Physical Assessment with Special Tests Shoulder COURSE DESCRIPTION Detailed video demonstrations of tests and procedures common in musculoskeletal assessment are supplemented by high-quality full-color clinical photographs and illustrations of each test and assessment for the Shoulder. LEARNING OBJECTIVES Module 1 Selected Movements- Shoulder Correctly instruct patients to perform active movements of the shoulder which include elevation through forward flexion, elevation through scaption, elevation through abduction, medial rotation, lateral rotation, extension, adduction, horizontal adduction/cross flexion, horizontal abduction, circumduction, and scapular retraction and protraction. Identify positive indications of positive active movements of the shoulder consisting of elevation through forward flexion, elevation through scaption, elevation through abduction, medial rotation, lateral rotation, extension, adduction, horizontal adduction/cross flexion, horizontal abduction, circumduction, and scapular retraction and protraction. Correctly instruct patients in performing Apley s scratch test. Identify positive indications of Apley s scratch test. Correctly perform passive movement tests on the shoulders. Identify positive indications of passive movement tests on the shoulders. Correctly perform a posterior capsular tightness test. Identify positive indications of a posterior capsular tightness test. Correctly perform resisted isometric movements tests of the shoulders. Identify positive indications as well as clinical notes of resisted isometric movement tests of the shoulders. Module 2 Special Tests for Anterior Glenohumeral Instability Identify and describe the difference between Type I and Type II anterior instability found within the shoulder. Identify and describe relevant signs and symptoms of translational instability and endrange instability. Compare and contrast the reliability, specificity, and sensitivity of special tests for anterior glenohumeral instability (anterior crank test, crank/relocation test, fulcrum test, load and shift test, Rockwood test). Identify positive indications of a fulcrum test. Module 3 Special Tests for Posterior Glenohumeral Instability Identify and describe the relevant signs and symptoms of a load and shift test- posterior. Correctly perform a load and shift test- posterior. Identify positive indications of a load and shift test- posterior. Correctly perform a Norwood stress test. Identify positive indications of a Norwood stress test. Correctly perform a posterior apprehension or stress test. Identify positive indications of a posterior apprehension or stress test. Identify and describe the suspected injury, epidemiology and demographics, relevant history, relevant signs and symptoms, and mechanism of injury for special tests for inferior glenohumeral instability. Correctly perform a sulcus test and/or a Feagin test. Module 4 Special Tests for Impingement Identify and describe shoulder impingement syndrome, with specific focus on Neer s three stages of shoulder impingement. Describe the mechanism of injury for both anterior and posterior shoulder impingement. Compare and contrast the reliability, specificity, and sensitivity between the Hawkings- Kennedy impingement test, Neer impingement test, and posterior internal impingement test. Correctly perform a Hawkings-Kennedy impingement test. Correctly perform a Neer impingement test. Correctly perform a posterior internal impingement test. 3 Module 5 Special Tests for Labral Tears of the Glenohumeral Joint Identify and describe various special tests for labral tears of the glenohumeral joint. Describe the suspected injury, epidemiology and demographics, relevant history, relevant signs and symptoms, and mechanism of injury for labral tears of the glenohumeral joint. Differentiate between the reliability, specificity, and sensitivity of the various special tests for labral tears of the glenohumeral joint. Correctly perform the active compression test of O Brien. Identify positive indications of the active compression test of O Brien. Correctly perform an anterior slide test. Identify positive indications of an anterior slide test. Correctly perform a biceps tension test. Identify positive indications of a biceps tension test. Correctly perform a clunk test. Identify positive indications of a clunk test. Correctly perform a biceps load test. Identify positive indications of a biceps load test. Correctly perform a labral crank test. Identify positive indications of a labral crank test. Correctly perform a SLAP prehension test. Identify positive indications of a SLAP prehension test. Module 6 Special Tests for Scapular Stability/Special Test of the Acromioclavicular and Sternoclavicular Joints Define and describe scapular stability. Explain the suspected injuries and epidemiology and demographics for scapular stability. Identify and describe the relevant history and relevant signs and symptoms for neurological signs and symptoms with regard to scapular stability. Describe and differentiate the mechanism of injury for scapular stability, with a specific focus on type I, II, III, and IV dysfunction. Correctly perform a lateral scapular slide test. Identify positive indications of a lateral scapular slide test, as well as clinical notes and cautions. Correctly perform a scapular load test. Identify positive indications of a scapular load test. Correctly perform a scapular retraction test. Identify positive indications of a scapular retraction test. Correctly instruct patients in performing a push up test. Define and describe acromioclavicular joint separation, including epidemiology and demographics, relevant history, relevant signs and symptoms, and mechanism of injury. 4 Correctly perform an acromioclavicular crossover, cross-body, or horizontal adduction test. Module 7 Special Tests for Muscle or Tendon Pathology Define and describe injuries to muscle or tendon tissue including suspected injury, epidemiology and demographics, and relevant history. Identify, describe, and differentiate between the relevant signs and symptoms of injuries to muscle or tendon tissue, which include tendinitis/tendinosis, tendon ruptures, muscle strain/tear (third-degree strain), as well as general signs and symptoms. Identify and describe the mechanism of injury for injuries to muscle or tendon tissue. Compare and contrast the reliability, specificity, and sensitivity of special tests for muscle or tendon pathology. Correctly perform and identify positive indications of Speed s test (biceps or straight arm test). Correctly perform and identify positive indications of Yergason s test. Correctly perform and identify positive indications of a supraspinatus ( empty can or Jobe) test. Correctly perform and identify positive indications of an abdominal compression (belly press or Napoleon) test. Correctly perform and identify positive indications of a bear hug test. Correctly perform and identify positive indications of a lift off sign. Correctly perform and identify positive indications of a subscapularis spring-back, lag, or modified lift-off test. Correctly perform and identify positive indications of a lateral (external) rotation lag sign (Erls) or drop test. Correctly perform and identify positive indications of a lateral (external) rotation lag (infraspinatus spring-back) test. Correctly perform and identify positive indications of the test for trapezius weakness. Correctly perform and identify positive indications of the test for serratus anterior weakness. Correctly perform and identify positive indications of the test for rhomboid weakness. Correctly perform and identify positive indications of the test for latissimus dorsi weakness. Correctly perform and identify positive indications of the test for pectoralis major and minor tightness and contracture. Module 8 Special Test for Thoracic Outlet Syndrome Define and describe thoracic outlet syndrome, including its suspected injury, epidemiology and demographics, relevant history, and mechanism of injury.
